After tightening a casing connection, the employee put EE's hands in the derrick before the pipe settled, pinching EE's thumb between the string and the table. Resulting in a half inch laceration.

Employee was testing a limit function on a drill and got EE's leg pinned between the rod support and drill steel. Employees knee became swollen. EE was seen by urgent care and discharged with instructions to rest it and ice it for 3 days.
The operator of the excavator hit the left rear corner of the 605 haul truck with EE's bucket while swinging to dump the bucket load into the truck.
An employee's finger was pinched resulting in an avulsion that required a skin graft surgery.
While attempting to place a bearing onto a shaft using a hydraulic press the shaft slipped causing it to make contact with the guard. The guard then struck the employee.
Employee felt pain in their shoulder while using an impact wrench on 5/14/2021. On 7/9/2021, it was determined that the employee had a tear in the shoulder consistent with the 5/14/21 incident.
While attempting to make a lift a picking eye broke causing the lifting chain to strike employees arm
Employee was working out of a manlift to remove wear plate. Tension was taken out of the rigging and the crane motion stopped and then started back in motion. Employee immediately tried to move the man-lift to get away from the pick. The picking eye failed dropping the wear plate. Struck the employee in the side of the face causing two fractures that do not require surgery to fix.
Employee was conducting maintenance on a haul truck. While utilizing a torque gun on a wheel lug the employee pinched pinky finger between the torque gun and the wheel well. The pinch resulted in a laceration and fracture to the tip of the pinky finger.
An employee's hand was contacted causing a laceration.
Technician was tightening bolts on a final drive. The technician put the 1" impact wrench on a bolt to finish tightening it. The impact wrench was in reverse, causing the bolt to back out and the technician's right pinky finger to be caught between the impact wrench and final drive.
While winching down the lifter got between rings on the mast ladder. Winched back up and hand was between winch line and rig mast.
The incident occurred while washing a work truck with a high pressure water cleaner located at the mine operator's vehicle washing facility. While washing the truck, water under high pressure exited a section of the hose, passing though rubber gloves, resulting in an "injection" injury to the individuals right index finger.

Chipping of Nordbak material out of crusher when a piece of material fell off and pinched thumb between fallen material and air hammer that was being used to removed material.
While removing a differential from a 789C haul truck the crane was moved off center of load to swing differential out of axle housing. Differential got hung up and when it came free the differential swung farther than expected and hit EE's causing laceration and contusion to right leg below knee.
An employee was moving a wheel barrow full of floor dry and pinched their finger in between the handle and a metal bin causing a laceration. The employee was transported to the hospital where they received 10 sutures to close the laceration.
Pull starting a diesel water pump when the motor locked on pull back.
A miner was installing resin bolts in active heading, when he went to set the bolt on the rail of bolter, he hit his finger on the bolt plate and when he reacted, he pulled his hand back causing a laceration to right index finger, requiring stitches.
No easily verifiable conditions contributed to the accident. Employee reported wearing all proper PPE throughout his shift.
As an employee was turning on heater valves on a dozer, their hand slipped and their glove got caught in the pulley. The employee's right thumb was smashed in the spring next to the pulley. The employee was transported to Nye Regional Medical Center were it was determined that their thumb was broken and they received 6 stitches.

Worker was holding sign when Backhoe operator went to move Backhoe and back of Backhoe swung and hit his finger
Employee was air arcing on a mantle liner, when a piece of molten slag burned through his pant leg and fell into his boot. The slag caused 2nd and 3rd Degree burns to his ankle before he could get his boot off.
A mechanic suffered a cut to the hand requiring 13 stitches while installing carriage bolts on a bucket of a 966 loader. He was holding the nut with his left hand while holding the impact wrench in his right hand. Once the wrench was activated, the nut spun freely slicing his Mechanix glove and cutting the employee's palm. The mechanic received a full duty release for work
While a hydraulic ram was being retracted the employee's left thumb was pinched between the ram head and its main housing.
Employee was drilling holes with a mag drill and power failed which released magnet and caused the drill to drop hyper extending the thumb.

As an employee was using a spring attached to a hand held drill to clean out the valve stem on a haul truck tire, the spring broke. The spring contracted and became embedded in the employee's left hand little finger. The employee was transported to Nye Regional Medical Center where it was determined that the employee's finger was fractured and the employee received two stitches.
As an employee was cutting a piece of steel out of a crusher, the steel shifted and contacted their right leg causing a laceration. He was transported to the hospital where they received eight stitches and seven staples.
He did not have both hands on the grinder when using it with a cut off wheel. The grinder kicked back and cut his left thumb.
As driller was pulling sample, sample became caught and stopped. Driller then continued on to next hole. Driller then coughed and felt a twinge in his back. Called shifter and was taken to ER.
Employee was cutting through a 12" SDR 11 pipe that was being supported with a backhoe bucket. When the employee had cut apprx. 3/4 of the way through the pipe it broke and struck the employee in the face. The employee suffered a lacerated and broken nose.
As the employee was operating a rubber tire dozer, they began feeling pain in their neck and back. The employee was seen at the hospital and released to full duty. At a follow up appointment on 11/26/08 the employee's status was changed. Employee was ordered off of work until 12/26/08.
Loss of power and failure of machines ability to stop, also the operators steering away from a paralell course with the incline of the slope.
