While performing drilling operations, drill pipe fell and struck the operators right hand causing injury to the right thumb which required surgery.
After unloading, EE drove to the cleanout to ensure the tubs were empty. Using a sledgehammer to tap the side and vibrate the remaining contents, it rotated in EE's hand, catching EE's glove, twisting EE's left thumb. EE was fitted with a splint to immobilize the thumb, was referred for a follow-up appt. w/ ortho and released to work w/o restrictions.
Pulley bearing failed and caught fire leading to drive tower/belt #1 fire.
Until the fire investigation is complete, we are unable to determine the cause of the fire to the truck.
Fueler lost grip of hose while retracting, allowing end of hose to strike their face.

An employee was in the process of re-inserting HQ split tube into a core tube. During the task, the split encountered ice inside of the tube and the employees hand slipped while applying pressure to the split tube. the sharp edge of the frozen split tube sliced through the employee's glove, resulting in a laceration to the employees left thumb.
While a Hitachi EX 5600 hydraulic shovel was digging in a shot pattern it is suspected it contacted an undetonated booster.
Driver statement: The tarp was in a bind, driver climbed up on the front of the trailer to help work the tarp over whatever it was hung up on. Where the tarp pole connected to the motor, it broke loose, and struck my finger.
Grinder slipped while in use, causing laceration to leg.
While IP was lifting drill rod to string, the rod slipped. IP reached down to grab rod and placed finger under pin end of rod pinching between box end and pin end of rod.
Employee was cutting sheathing from wires when tool slipped and cut EE's hand through EE's gloves.
Removing GET from lip of bucket. Was using a pry bar and soft face hammer to work the old tooth off with the crane attached. The tooth was moving and I was using the hammer to walk the adapter forward. When hitting the adapter a piece of the corner of the adapter splintered off and hit my left inner thigh.

While replacing a chain link on the drill head crew was using a chain come-along to pull the slack out of the chain to install new link when the come-along slipped from its anchor point coming in contact with then assistants knee causing a cut.
Employee was moving the core tube from the drill rods to the table. As they approached the table, the core tube released unexpectedly dropping onto their hand on the table. Equipment malfunction contributed to the core tube releasing. Employee left site to seek medical attention, and was promptly released after examination.

Operator had a fall of the same level after stepping on a rock walking to equipment
While attempting to place a bearing onto a shaft using a hydraulic press the shaft slipped causing it to make contact with the guard. The guard then struck the employee.
Operator was lifting a cover to close a ball valve, when the lid slipped out of their hand causing the lid to pinch index finger.
While attempting to remove a rock from the push arm of a rubber tire dozer the operator felt a strain in right arm.
Mechanic removed new ripper tip from rack and went to place the ripper tip in the service truck when mechanics finger was pinched between the ripper tip and tailgate.
Case began as a first-aid only case on 05/25 and was modified to reportable injury on 07/07. Laborer using a vice grip clamp attached to the liner to help guide the liner. The employee tugged on the liner and the clamp came loose. The employee lost their footing and fell backward on the slope. The employee braced themselves with their right hand and received a strain to the wrist.

Our permitted landfill built on an old waste rock dump had an unplanned fire occur. The source of the fire is unknown. No damage was caused by the fire and no personnel or equipment were exposed.

While driller was changing a hammer it became entangled in the collar on the deck and when the hammer broke loose the employee got their finger caught in the jib hoist line causing a laceration.
Mechanic pinched finger while removing light bracket from a dozer.
Mobile equipment mechanic was loading 5600 shovel teeth onto service truck, the mechanic slid one shovel tooth over to make room for the additional shovel teeth, in doing so the shovel tooth slid on the frosty deck and pinched mechanic's left pinky finger against the truck mounted vice, causing a laceration which required three sutures.
While positioning a steel plate, the operators hands slipped and the plate fell on their foot.
An employee was using an auto retracting knife with hook blade to cut through a liner weld to inspection it As EE cut the liner the blade met resistance at the weld seam. EE pulled harder to cut the weld seam. The blade cut through the weld seam and struck EE in the left leg. EE got a 2"" cut which required 8 stitches and a tetanus shot.
Operator pinched finger between drill and bit basket
While replacing swing relief valves on a hydraulic shovel, employee stepped off one level of a cat walk onto another approximately eight inches without realizing. The elevation change caught the employee off guard and they fell onto a grease tank contacting the left arm.
While walking a Hitachi 470 excavator a fire started in the rear of the machine. Operator safely exited the machine after shutting it off and engaging the parking/hydraulic lever. No injuries.
Employee was cutting a length of electrical cable for installation in CIC-2. The employee was using a Klein cable splicer's knife to cut the zip tie holding the end of the cable secure to the reel. When the blade cut through the zip tie and the blade struck back of the employee's wrist cutting through the cut gloves that EE was wearing.
While operating in the dig face a Hitachi 5600 shovel caught fire causing extensive damage. The operator was able to safely exit the machine with no injuries. The investigation of the cause was inconclusive.

Employee was using a utility knife to cut a hose, the knife slipped and cut employee's arm. Resulting in stitches.
Fueler noticed flames coming out of the engine compartment while driving on the tailings facility causeway. Employee reacted as trained and proceeded to call a MAYDAY, put the truck in park, set the park brake, shut the engine off and initiated the fire suppression system, and exited the truck. In the process of walking away from the truck flames reappeared.
After lifting a wheel chock employee felt an irritation in the right shoulder.
Technician was tightening a bolt for a hose clamp under a 793F Truck, when dust fell and hit near their right eye. They felt debris enter the eye, so they proceeded to flush eyes several times. The debris was unable to be removed locally, so they were treated at a local clinic for removal.
Employee was removing an exterior weld bead from a 10"" HDPE pipe using a cold chisel. The employees hand and cold chisel slipped contacting EE's left forearm causing a laceration requiring 3 stitches. Employee was immediately released to work with no restrictions following medical treatment.
Haul truck driver strained back during pre-operational check of equipment.

Technician was tightening bolts on a final drive. The technician put the 1"" impact wrench on a bolt to finish tightening it. The impact wrench was in reverse, causing the bolt to back out and the technician's right pinky finger to be caught between the impact wrench and final drive.
A dozer caught fire without injury to operator. After further investigation the most probable cause of the fire appears to be left to right side battery cable direct short to transmission cooler hardline, igniting transmission oil.
Portable heater caught fire while being fueled.

Person fell to ground while walking- injuring person's left ankle.
While working to remove the lower corner liner of a mill, a corner liner above slid down impacting employees fingers.
While driving a pin from a CAT 16M grader mechanic missed the pin driver and struck left hand with the hammer. Initial X-rays from the ER did not show a fracture. After the visit with the orthopedic specialist on Sept. 6th the mechanic was diagnosed with an oblique fracture to the 4th metacarpal.
While walking on even ground in the pit area inspecting a haul truck a mechanic stepped on a small rock causing the leg to slip resulting in what was believed to be a strain to the knee. After continued discomfort a CT scan was ordered and read on September 7th which showed a non displaced hairline fracture of the tibia.
Employee was starting to slide a rod away from the grating so the rod sling (clam shell) could be removed. The employee's right foot slipped and the employee injured knee.
Employee was carrying a chain jack, lost grip, re-grabbed the jack hitting two top front teeth with the top end of the jack resulting in both teeth being replaced on 4/13/2017.

While lifting a shift cylinder employee felt a strain in wrist.
Employee was kneeling down fixing the foot clamp jaws. When EE tried to stand up EE felt pain in lower back and lost strength in legs and fell to knees. When EE stood up the second time EE felt pain in lower back.
Over-liner Aggregate collection - samples carried via 5 gallon buckets (70-80lbs) being walked to pickup truck without assistance (mechanical or human). Did not report injury until 10/10/2016.
Miner was driving a haul truck. After the second load, miner says they hit a bump, causing back pain. The validity of the claim is in dispute.
An unplanned fire occurred in our permitted land fill that is built in a rock waste dump excavation. No damage or impairment to any property or people.
The employee stepped out of a crew van onto the ground causing a sprained left ankle resulting in restricted days.

Crew was tripping out collars. Employee had pulled the collar sling off and set it down on the deck when his foot slipped off the 4x4 blocking. He fell and twisted his ankle.
Technician fell to the ground and broke his fall with his left arm and as a result, fractured a bone in his left wrist.
While walking up stairs he felt a shift in his back and it started hurting. He was not carrying anything at the time. He was taken in to be checked out. On the 29th it was still bothering him. During his follow up, after not getting better he was restricted to light duty for 2 days and given a prescription
Welder was welding 30"" pipe and needed to adjust pipe. Fire watch, injured worker tried to manually roll the pipe to complete weld. After lunch the injured worker complained of back soreness. He was taken to clinic for back pain.
Employee reached behind the seat to retrieve the passenger side seat belt when his finger came into contact with a sharp object causing injury to his right index finger.
Steel fell on to the employees left foot causing injury to the top of the left foot.

The operator fell while accessing a motor grader causing an injury to the left hip.
A blast hole driller was placing a drill bit in the holder and pinched the tip of the pinky finger on the left hand.
Employee were working on installing pump and landing plate fell and contacted thumb.
The injured employee was driving a service truck down into the pit to work on a loader. While driving slowly (reported to be approx. 5MPH) due to wet and rough road conditions, the employee was jarred around and said truck's seat bottomed-out, hurting his back. A Licensed Health Care Professional diagnosed the injury as: #847.2 - Sprain, Lumbar Region.
While walking on uneven ground, the IP had stepped on a rock that had rolled over and allowed his leg (Knee) to hyper extend, and then landed on his backside. At the time the IP felt some pain in the knee but the pain dissipated and he thought no injury had occurred until a few days later the pain returned. It was later determined the IP compressed the meniscus in his right knee.
While re-threading a pull down chain on a drill, the employee strained their lower abdomen.

Employee was swapping over engine and radiator and was removing old hose from lower cooling line on trk467 793D haul truck. He was using a pocket knife and cutting away from himself and had both hands pushing one on top of the other for better leverage and the knife slipped out from under hose and cut right little finger.

The driver was backing down an access road to the stage 4 heap leach. While backing the truck lost power causing the air brakes to weaken/fail. Near the bottom of the slope the truck began to turn side hill and rolled. The driver was belted in but received minor laceration to the top of his head from striking it in the cab.
The dozer operator was assisting a mechanic change a ripper tooth on his cat and a piece of metal caused a laceration in his leg when the tooth was struck by a hammer.

Dewatering personnel was on the ground near the area where a decommission electric cable was being moved when the cable stuck him in the lower leg.
EE was working with a wrench to loosen a bolt off a 777 haul truck while doing so ee was putting his full body weight into it when the wrench slipped off the bolt. He fell forward striking his face on the 777 haul truck cutting his bottom lip. He was checked out in town, received 2 stitches and returned to work the same day.
EE was task training on a 777 F series with another EE as the passenger.He was backing haul truck under the Cat 993 loader when the haul truck was rocked violently. He states he was catapulted by the air seat into the headliner of the cab. Other EE was in the training seat and immediately his back started hurting.
Per statements IEE who is an experienced haul truck op. was training another EE on a haul truck. EE was on 2nd day of training as he was backing up to a loader operator had the bucket raised so the truck could be loaded, as the truck got into position apparently the bucket was dropped quickly which jarred the truck and hurt his lower back. EE is on light duty.
According to IE , he was working on tightening a mirror on a truck in the lay down yard when the wrench he was using slipped off the nut striking him in the mouth chipping his tooth. EE reported the incident that afternoon and refused medical treatment. EE RTW with no lost time.
While clearing poured slag from a table, a piece cracked as it cooled, throwing a small piece at the employee. The piece passed under the employee's safety glasses, striking him in the right eye. The employee received a corneal abrasion to the right eye.
Tech was looking for device next to door as she walked in and the door closed faster and harder than expected and left fingertips were caught in door. The door had an oversized closure spring because of strong winds. Skin and fingernails were torn and bones of fingertip later diagnosed as fractures.

Individual was removing chock blocks from his truck. He dropped one onto his right foot immediately behind his steel toe. Individual received a fracture to his right 5th toe.
While performing cleanup a mill operator slipped and fell to the ground.
Replacing ripper on D-10, went to rotate ripper, my left foot lost footing and my weight followed and my R/leg didn't. Bent R/knee out to the side as I hit the ground. I rolled over to put it back in place. Went to shop to see ERT's @ Ft Knox.
While using a hydraulic torque wrench employee had the tip of their left thumb smashed.
After using a ban saw employee had metal shavings on his glove. He removed his safety glasses to itch his eye. When his eye started bothering him he washed it out. Later that night it continued to bother him so he went to the hospital where they removed a piece of metal.
EE was adding drill pipe to drill load rack, load was not centered when picker from magazine and swung out pinching his little finger.
A scrap steel bar was left on a cutting table after not being used. I placed a small piece of 1/4"" plate on the table. The vibration caused the steel bar to fall on my steel toe boots. I received 3 stitches on a small cut on the toe, and a fracture on the knuckle of the big toe.

Person was assisting with retrieving a water pump from a well. Pump wire unravelled from tooling catching person's hand between wire and well casing.
Person was walking up stairs while carring a 1.5 in. by 2 ft. long piece of pipe. Person tripped on the grating of the stairs. When reaching for the handrail, two fingers were caught between the pipe and handrail.
Employee slipped on ice and braced himself on parked pickup truck, injuring his left shoulder.
Worker pinched her right thumb in the door jam of a CAT 938G loader while opening the door to the cab. Worker was climbing into the machine when the injury occurred.
On January 27, 2011; RTD354 (rubber tire dozer) was cleaning the 1990 floor while the shovel in the area was temporarily down. While making a push along the highwall at 12:17 AM, the operator heard a loud noise which he assumed was a front tire failure. When he investigated the noise, he found what appeared to be a partial booster on the ground.

While exiting a door to the outside employee slipped on a piece of siding which was covered in snow causing some discomfort in his left knee. The employee was not initially in any pain and did not seek medical attention but after several days the knee began to bother him. Employee sought medical attention on 01/22/2011 and then received restricted work activity.
While a hydraulic ram was being retracted the employee's left thumb was pinched between the ram head and its main housing.
While using a screwdriver to tighten a screw the employee ruptured their right biceps muscle.
Employee tripped on a communication wire causing him to sprain his left achilles tendon.
Employee was hooking up light plant to back of truck manually when the lamp plant fell pinching finger between tung and light duty pickup hitch/tailgate. Resulted in scraping and potential tendon injury of left ring finger (center knuckle). No broken bones.

Operator drove front left tire of a ten wheeled gravel truck into a hole covered with ice causing the truck to roll on its left side.
While sliding a heavy object employee felt a sharp pain under his right shoulder blade.

A haul truck driver was beiginning an uphill climb in third gear. The excessive speed caused his CAT 785 seat to bottom out after cresting a hump. This resulted in a lower back strain to the driver.
EE was removing hard hat to wipe brow and left eye became irritated. Incident report filled out and ee eye flushed with eye wash. Later in the shift eye irritation remained. EE sent for to clinic where a foreign particle was removed.
Employee injured his left pinkie finger while replacing a belt on a pump.
He did not have both hands on the grinder when using it with a cut off wheel. The grinder kicked back and cut his left thumb.
As driller was pulling sample, sample became caught and stopped. Driller then continued on to next hole. Driller then coughed and felt a twinge in his back. Called shifter and was taken to ER.
While picking up 10 pound chock block, employee experienced sharp pain in his lower middle back.

Employees were tripping drill steel into the hole, injured employee was in the racking board tied off by the self retracting lifeline (SRL). While lowering the drill steel, the traveling block contacted the SRL cable pulling the employee to the floor of the racking board and twisting his ankle.

The employee was walking out of the mechanical room, which is under construction, when he lost his balance due to slippery surfaces and started to fall. During the fall the employee put out his hand to gain balance & came in to contact with a steel girter. The girter had a sharp edge that lacerated the employees left forearm (10 stitches).
A small 1/4"" piece of metal became lodged under the skin of employee's little finger of right hand while fellow employee was hammering bucket wear metal package off Hitachi 3600 shovel bucket they were rebuilding.
While moving a light plant EE pulled on the telescopic jackstand and felt a pain in his right arm. The arm began to stiffen and EE was taken to the doctor.
Loss of power and failure of machines ability to stop, also the operators steering away from a paralell course with the incline of the slope.
While using a 4"" grinder, the grinder slipped and wheel ran across left thumb
While cleaning and inspecting new 57"" wheel hardware, injured was standing in front of elevated wheel hardware held by a mobile tire handler he was operating and the hardware weighing approximately 1500 Lbs. slipped from the handler gripping pads and struck injured on the right foot, resulting in crushing injury to right arch and first three toes on that foot.
Employee was walking across shop yard and slipped on ice, twisting his left knee. Employee did not fall but had pain and swelling in the knee.
Employee was swinging a 20 lb. sledge hammer, driving a piece of all thread to remove pan liner bolts from the SAG mill. The hammer glanced off the all thread and pinched the employee's finger causing a laceration which required sutures to close.
Employee was thawing a frozen downhole pressure line on a drill. There was some pressure on the line which released when the hose was removed. The pressure and ice forced the employee back and off the ladder he was working to the ground. The employee had a laceration to the back of the head with sutures to close, and bruising of the left shoulder.
Refinery operator was hooking an Electrowinning basket onto the bracket attached to the crane. One side of the basket came loose, the basket swung and pinched the employees left middle finger between the basket and the crane hook. This resulted in a fracture of the finger tip and a laceration that required sutures to close.

Employee's hand was caught in main line cable and pulled into winch drum, which pinched fingers of right hand.
Employee was on the staircase leading to the basement where the end of a clogged sump hose was located. Compressed air was turned on to remove the blockage. When the obstruction gave it caused the hose to whip. Employee was struck on the left elbow causing a fracture.
As Mechanic was tightening a bolt the wrench slipped and cut across the employee's left thumb. Laceration required stitches.
Lab employee reported experiencing discomfort in his right shoulder as he was pulverizing samples. Employee was diagnosed with a neck strain and muscle spasm.
Ice may have built up in the hose due to extreme temperature. Although whip checks were installed on both ends, there were no guards keeping the middle portion of the hose from moving in a violent manner.
Employee was performing detox duties when foreign material got into his right eye. Employee was diagnosed and treated for chemical conjuctivitis. Employee was wearing safety glasses.
A haul truck operator experienced numbing pain in his neck after a large rock, along with smaller material, was unloaded from a shovel into his truck. The large rock caused jarring which contributed to this injury.
Mechanic was lifting a 5-gallon bucket of oil when he experienced a pain in his lower back. Employee has been diagnosed with an acute LS strain and muscle spasms.

Employee was performing maintenance activities on a motor grader and while stepping off the machine twisted his right knee. Upon further medical evaluation the employee's injury required surgery on 2/23/2007 to repair a torn meniscus.
Employee was attempting to change a rim liner bolt on the Gyratory crusher. Employee had accessed the crusher dump pocket and slipped on loose material located on the rim liner. The Employee fell into the crusher and sustained a fracture to the Tibia Plateau. Employee was utilizing fall protection equipment to perform the task.
Operator was climbing the ladder to the cab of a haul truck when he slipped and fell. Employee had been working with the lowboy, handling the chains and binders. Operator commented that his gloves were slippery due to the lowboy chains and binders. Employee was diagnosed with a bruised tailbone.
A production employee slipped and lost his balance while positioned on the tracks of a dozer. The employee fell onto the dozer tracks hyper extending his right wrist.
Shovel Operator was cleaning the pit floor when the bucket of the shovel hung up on a rock causing the shovel to lunge forward. The employee was jarred in the seat when this occurred and sustained a strain to the lower back. Employee was diagnosed with a LS strain and placed on modified duty.
A maintenance employee was repairing a water leak on a drill rotary head utilizing a 36 inch pipe wrench. While pulling on the pipe wrench the employee felt a sharp pain in his lower stomach. The employee was seen by a physician and diagnosed as having an abdominal strain.
Mechanic was replacing a lip protector on a bucket of an hydraulic shovel. A second employee struck the lip protector with a metal hammer which caused a piece of metal to fly off striking the mechanic in the mouth cutting his lip.
Employee was loading and unloading ore sample boxes on 11/08/2006 when he recalled feeling soreness in the upper back area. Injury was reported 11/14/2006. Employee was diagnosed with a Thoracic LS strain & spasm.
Employee was performing a walk-around on a haul truck, when he slipped on the ice and fell. Employee sustained a sprain to his right shoulder and has been placed on restricted work activity.
Employee was cutting an ""I"" beam with a torch. The cut piece was not secured and fell pinning the employees right ring finger against another ""I"" beam. This caused a fracture to the finger.
There was not a single, instantanious incident associated with this injury. Excessive lifting of drill bits over a period of time has been claimed as a contributing factor.
EE was stripping foam off of HDPE pipe when the knife he was using slipped and cut his finger resulting in 2 stitches. He was trying to remove too much at one time.
Employee was performing wash down duties with a 2"" water hose and developed a pain in his right elbow. This was reported to his supervisor on 9/30/2006 at 1:55 pm. Employee sought medical attention on 10/02/2006.
There was not a single instantanious incident for this injury. It was difficult to determine the date the injury occured or the task which contributed to the injury. Employee was performing crushing and pulverizing tasks within the lab. The repetitious movement thus resulted in a pain in the shoulder area. Employee sought medical attention on 9/29/2006.
Operator developed pain in lower back at the end of shift. There was no single instantanious incident that contributed to injury. Supervisor reported the equipment, roads and operating areas were in good condition. Employee sought medical attention on 9/23/2006.
Mechanic sustained a laceration to the right hand while moving a torch cart. The employee was walking backwards with the cart when his hand was pinched between the handle on the cart and a wall mounted piping valve. The employee received four stitches to close the wound.
A haul truck driver fell from a distance of 8 ft while boarding a haul truck. While climbing the ladder on the truck the employee's lunch pail, which he was carrying over his right shoulder, hung up on the housing of the air filter box. Employee lost his balance and fell away from the ladder.
Employee was operating a D10 Dozer in the Downstream Random Fill. Employee removed his seat belt and exited through the right side door of the dozer cab to signal the haul unit operator when he slipped and fell to the ground. Operator taken to Fairbanks Memorial Hospital for treatment. X-rays show fractured wrist.
Employee sustained an injury on 8/05/06 while performing crushing and pulverizing tasks. Employee was diagnosed with tedonitis and returned to work with no restrictions. On 9/17/06 employee reaggravated the injury and was placed on resricted activity.
An operator of a CAT 789C haul truck backed onto a berm which gave way at a waste dump. The haul truck and operator traveled down the slope of the dump and came to rest upside down on a lower dump. The operator was wearing the seat belt at the time of the incident. The emloyee was treated for lacerations to the scalp and released the same evening from the hospital.
EE was changing out a hydraulic pump & lacerated his finger on a coupling while attempting to stand the pump on end. EE rec'd 5 stitches to close the wound.
Employee was picking up a badger flow meter when he felt a pain in his back. Pain subsided and he continued to work. When he lifted a pallet and felt the pain again he then reported it to his supervisor.

An employee was removing a radiator from a dozer when the bottom of the radiator became hung up on the crank pulley. The mechanic wiggled the radiator causing it to come free pinching his left hand between the radiator and the hard nose of the dozer.
