Welding on D-12 drill. Was welding retaining straps to head and steel and flashed eyes before welding hood was lowered.

WORKER WAS TORQUEING THE INSIDE WHEEL NUTS ON A CAT 77 USING A 1" AIR WRENCH. THE WRENCH WAS NOT "SEATED" PROPERLY ON THE WHEEL NUT WHEN THE AIR WAS APPLIED, RESULTING IN IT TURNING & PINCHING HIS RIGHT THUMB AGAINST THE VALVE STEM GUARD. THE WORKER REQUIRED TWO STITCHES IN ORDER TO REPAIR THE CUT.

FOREMAN WAS DRILLING. REACHED UP TO TURN OFF MACHINE. RIGHT HAND WAS STRUCK BY SHARP ROCK (SOFT BALL SIZE) WHICH DISLODGED FROM ABOVE AND IMPACTED HAND RESTING ON DRILL. BARRED DOWN BEFORE DRILLING. FOREMAN WAS WEARING ALL REQUIRED SAFETY EQUIPMENT.
