The employee was laying on the shop floor using a cutting torch to remove a bolt from a truck when a spark or small piece of slag went in his left ear. He gave himself first aid for a burn in his ear. He then went to the clinic as a precaution. He was examined, given a prescription for ear drops and he returned to work the with no restrictions.

The employee was using a cutting torch while changing screens. The hose snagged as he moved, causing the torch to pass over his arm resulting in a burn on his left forearm.

EE WAS DOING REPAIRS ON DRAG CHAIN. DRAG CHAIN HAS A GUIDE IDLER & IDLER WAS OUT OF ADJUSTMENT IDLER IS MOUNTED ON A BRACKET MADE OUT OF ANGLE IRON. USING A CABLE HOIST/PULLER TO SECURE IDLER DLER, EE CUT BRACKET WITH TORCH. IDLER & BRACKETWAS HANGING ABOVE EE'S HEAD & WHEN BRACKET WAS CUT THE CABLE HOIST FAILED TO HOLD IDLER & IDLER & BRACKET ROTATED DOWN, STRIKING EE IN THE MOUTH

EMPLOYEE WAS KNEELING ON METAL PLATE WHILE REPAIRING LOADER. ANOTHER EMPLOYEE WAS UNDER THE PLATE DRILLING UPWARD. THE DRILL CAME THROUGH THE PLATE STRIKING INJURED IN RIGHT KNEE. SUTURES R EQUIRED TO CLOSE WOUND.
