Over-exertion (Not Elsewhere Classified)

The employee was changing skirting rubber. They stepped up on a piece of the framework to look in a hopper and when they stepped back down their foot landed on a piece of material and they twisted an ankle. It was initially treated as a sprain, but after not improving as quickly as expected, an x-ray was done on 4/22 and it was determined that there was a fracture.

Over-exertion in wielding or throwing objects

The mine employee was using a 4' sleever bar and a 4 lb. hammer to remove mud that was built up under the Stamler drag chain. EE felt a pain in right shoulder. EE still had some pain later, so EE was taken to the clinic on 5/17/2024 and returned to work with no restrictions. On 7/26/2024 EE was seen by another doctor who evaluated the MRI and put EE on light duty.

A haul truck driver with a history of asthma told the excavator operator that EE was having an asthma attack.

The employee was fabricating a base for a motor and gearbox using a steel plate 24""x24""x3/4""thick. It was on a steel work table in the shop. While repositioning the plate the employee smashed the end of left index finger between the steel plate and the table top. The finger was split open and required six stitches to close the wound.

The miner spent about 2 hours kneeling during the process of replacing skirting rubber. This aggravated a previous knee injury and miner was instructed to do no kneeling and avoid using ladders for at least a week.

Fall to the walkway or working surface

The miner was walking on a catwalk, tripped and tried to catch self injuring a shoulder as they fell down. Miner initially thought they were ok and did not report the accident to their supervisor until 4/27/2021. After the initial medical examination and a follow up, physical therapy was recommended.

The maintenance team was installing a new screen when a 6 inch square steel tube supporting the screen tipped and struck the miner's hand catching the hand between the tube and the catwalk. It resulted in a fractured left thumb and laceration on left hand.

The employee was unrolling a roll of conveyor belt. The roll was going crooked and they tried to slide the roll of belt to straighten it and strained their back.

The mine employee was removing metal shingles from the No. 1 kiln and cut left hand on one of the shingles. It required 6 stitches.

The employee was helping an over the road truck driver dump some material from an end dump trailer. When they were done, EE tripped on the pile and grabbed the back of the trailer to steady self at the same time the driver latched the tailgate. EE's hand was caught under the tailgate latch crushing a bone in the center of EE's hand.

Struck by falling object

One miner was preparing to move a welding trailer to another location when another miner dropped a piece of metal from the location where EE was working on top of a kiln. The metal struck the first miner in the left hand causing a cut and swelling. It was determined at the clinic that a bone was also chipped and EE will be on light duty for about three weeks.

The employee was repairing a water line. It was a very hot day and employee was working in a hot part of the plant. Employee developed heat stress. Employee was driven to the clinic for treatment.

The employee was working with others to dismantle a roll crusher. After using a wedge to separate some parts, he was removing the wedge and somehow struck the tip of his right pinky finger between the wedge and the frame of the crusher.

The employee was helping to pull a large electrical cable into a cable tray. During the process, he thought he pulled a muscle. He reported it to the safety manager on 10/02/2015 and on 10/13/2015 it was determined that he had a hernia.

The employee was using a large hammer to remove some rusted bolts while repairing a roll crusher. He missed the bolt he was aiming at when he swung the hammer and hit the handle of the hammer on the crusher. His right index finger was injured and required stitches to close the wound.

The employee was adding hydraulic oil to the dump truck when he slipped from where he was standing and fell. The fall resulted in him twisting his ankle. He thought it was a sprain, but when he went to the clinic to get checked they found his ankle was broken.

The employee was laying on the shop floor using a cutting torch to remove a bolt from a truck when a spark or small piece of slag went in his left ear. He gave himself first aid for a burn in his ear. He then went to the clinic as a precaution. He was examined, given a prescription for ear drops and he returned to work the with no restrictions.

The employee was using a cutting torch while changing screens. The hose snagged as he moved, causing the torch to pass over his arm resulting in a burn on his left forearm.

Employee was using a utility knife to trim a belt. He set it down, and when he picked it up he grabbed the wrong end and cut his hand.

Employee was making his rounds and tripped. He reached out to keep from falling down and his hand came in contact with the Number 4 kiln hood. He sustained a small 2nd degree burn on the side of his left hand.

After loading a truck, the employee was walking back to the scale room and stepped on a clinker and injured his foot. He went to the doctor two days later and was informed he had a hairline fracture, but no treatment was necessary. At a follow up 6 weeks later, it had not healed and it was determined that surgery would be necessary.

Worker was loading a truck under the tanks and when he closed the door, he caught his finger between the lever that operates the door and a piece of the framework under the tanks.

Workers were dismantling the intake pipe on the number 4 coal pulverizer when the pipe shifted and caught the worker's hand between the pipe and the structure, resulting in a small bruise and cut on his right hand.

Two workers were replaceing a guard on return conveyor. One worker jerked guard towards himself, pinching other worker's left hand between guard and motor pulley. This caused a laceration to the middle finger.

SUNDAY HE WAS CLEANING PLUGGED KILN CHUTE DURING THIS TIME HE EXPERIENCED SHARP PAIN AND HAD A BULGE ON HIS RIGHT SIDE, IN THE GROIN AREA. EMPLOYEE WAS TAKEN TO CLINIC THURSDAY MORNING & DIAGNOSED WITH A HERNIA. HE IS SCHEDULED FOR SURGERY 01/09/02.

EMPLOYEE WAS STRUCK ON RIGHT SIDE OF FACE AND HIS HEAD HIT A LADDER. EMPLOYEE HAS A BRUSIED JAW & A LUMP ON LEFT SIDE OF HEAD. TAKEN TO CLINIC WHERE HE WAS EXAMINED, HAD X-RAY OF JAW. NO BROKEN BONES. HE IS OFF WORK 12/11 DUE TO SHIFT CHANGE.
