While removing liners from the Coal Mill, a fragment broke off while hitting it with a rivet buster. A worker attending nearby was hit by the projectile, which was embedded approx. 2cm into their right thigh. Employee is scheduled to have it removed in surgery on 02/12/26. Employee is currently on restricted duty and another method for liner removal is being evaluated now.

The employee went to put an air lance down the pipe, when doing so the air came back faster than expected and the material went right over safety glasses and into the employees eyes. The employee was not wearing EE's face shield.

Employee was cutting the head of a bolt to remove a machine guard with a portable grinder. The grinder kicked and struck the employee in the lower left thumb causing an injury that required six stiches.

Using a crane and hopper to load balls into finish mill. An employee tried to connect the crane hook to the clevis. The operator lifted the load before employee was done attaching the hook.

Employee stopped grinding and leaned forward. Dust/debris that had built up on hard hat or faceshield bracket, fell down EE's face and into EE's right eye. Eye was washed out & employee felt eye was okay. On 12/18/2017, employee reported that eye had gotten worse; red from irritation, causing pain and blurred vision. Employee was taken to Quick Care for foreign body removal.

When clearing the preheat tower, an employee was using a jack hammer to try and clear a large build up. When it came loose, the combined weight of the jack hammer and the chunk twisting as it fell sprained the employee's shoulder. Employee was seen by a doctor that day and placed on restricted duty.

The production crew was in the process of switching silos. EE was checking a leak on the #4 cooler when the transport line to the silo started blowing out cement dust. EE did not have respirator on. EE quickly put it on but had already inhaled some of the dust. EE reported burning in chest & shortness of breath. Sent to ER for medical attention.

Employee was cleaning the preheater. EE was using a water lance to break up a chunk. When it broke loose, it hit the bar of the water lance. The lance was positioned under the employee's armpit and pointed upward. The impact jolted the water lance with intense impact on underarm. The employee was escorted to the ER where EE was treated for a dislocated shoulder.

The ground was damp & muddy. The employee was using a jackhammer to clean out the screens on the Southwest side of the plant, when the jackhammer got stuck in the mud. The employee attempted to pull it out. As he was pulling, he heard his back pop.

Employee is a machinist. He was cleaning a bolt on the wire wheel machine. The bolt pulled into the machine and employee's finger was pulled in.

Employee was standing by the door frame. He and his crew had been lifting and placing a piece of table liner through the door and into the mill. The liner became stuck in the door frame. The hoist operator backed it out. As it backed out it turned unexpectedly towards the employee. He backed away but it trapped his arm between the load and the door frame.

Employee was scaling tower #3 with air lance. Large chunk of material sloughed off and jarred air lance. Employee dropped air lance and tried to move away. Air lance handle hit employee above right knee. Employee did not notifiy anyone of incident until going to doctor on 10/16/2008.

Employee alleges that he was welding inside a chute and received a couple small welding burns on his legs. He did not report it. One week later he stood up from a desk and felt a sharp pain in his left knee. The next day he was admitted to the hospital with an infection in his knee. He alleges the infection came from the small welding burns.

EE was welding on the 807 elevator housing. two ee were vacuming with industrial vacum hose. The hose whipped suddenly and hit him knocking him down. He was unconscious for 30 seconds-2 minutes.

Employee injured right thumb while poking to remove buildup in the preheater tower riser for kiln system #2. Employee's thumb was caught between the outside door latch and the air lance handle when he encountered a soft spot in the buildup.

Employee was air lancing #2 preheater feed shelf when a large chunk of material fell and hit the 3/4" x 18' long air lance and knocked the lance from his right hand. He held onto handle with left hand as air lance spun in a clockwise motion - twisting left arm out & around. Lance then came loose from left hand, spun and struck left wrist with other handle.
