Employee was roof bolting. EE had put in finish and push steel. As EE began to drill the hole, loose shirt sleeve got entangled in the drill pad.
While employee was installing a roof bolt a damaged roof bolt struck the employee on the left side of face cutting it beside the left eye. Employee received 4 stitches. Came back to work the next day.

EE was roof bolting and had a piece of drill steel in his hand and was dropping boom of bolter and the boom caught the drill steel and pushed it down on his right foot. This fractured his 2nd toe next to his big toe.

Employee was loading tray on off side a roof bolter. When the bolter moved up it ran over his left foot and bruise it.
He was climbing up in #4 face & moved miner over to the right side. He could not get the ripper head to start. He trammed the miner back out of the face. When the miner came toward him he moved forward to keep from being hit and was pinned between the miner and the rib. It hurt his left arm and torso.
He was grinding a bolt on a shuttle car with a grinder. When it kicked back and hit him on top of his right hand, which cut his hand. He received 5 stitches on top of his hand.
A rock slid off buggy, 2' X 1' X 3" and fell on his left foot. Broke his little toe.

Running roof bolter in #7 entry first bolt in the first row. He started to drill when the drill still bent and struck him in the left arm above his elbow broke it. He was also thrown : Back against the rib and hit his head that caused a small cut above his left ear.
Employee was using torches cutting out head jack bushing when some slag popped up got in the corner of his left eye. he had safety glasses on.
Drill steel was hung in hole in top. He was on his knees, placed hand on boom to pull himself up and while he was doing this the steel came loose falling and hitting his hand. Broke his middle finger on left hand.
Was pulling bolter steel out of hole. It caught left hand pinky finger between steel and bolter. Had 3 stitches, came back to work next day. No lost time.
EMPLOYEE WAS PINNING THE TOP, HAD HIS HAND ON THE BOOM, BOOM CAME DOWN & MASHED HIS RIGHT HAND, BRUISING IT. NO LOST TIME.
Installing 10 foot rope bolt, he spinned the bolt to get it to go up. The bolt head broke. The wrench came back and hit him in the mouth cutting his lip, breaking out a tooth and loosening three more.
EMPLOYEE WAS INSTALLING ROOF BOLTS. WHEN HE TURNED TO GET A BOLT HE FELT THE BOOM HIT HIS LEFT FOOT. IT BROKE HIS MIDDLE TOE.
Ee was turning left hand nich with miner. He was on his knees on right side of miner when a rock fell out between bolt and rib hitting him on shoulder then falling on his right foot cutting off his 3 little toes.
