Fall was found on 3rd North West Sub Mains starting 1 break inby spad #10338 at the beginning of the inter section and stopping 1 break outby spad #10377. Fall is dangered off and supported as per roof control plan. The fall will not be cleaned up.

Employee was in the right side return airway coming off of overcast when EE slipped on steps and hurt back and neck.

Roof fall in the return from 55 number 50042 and 50052 in number 4 entry.

Employee was recovering belt structure when employee felt pain in employee's back.

Employee was getting into a scoop and twisted back.

Employee was operating a shuttle car. When EE hit a hole in the road, EE hit head on the canopy of the shuttle car.

Putting up curtain board and the drill steel got hung in drill hole. Employee was trying to pull drill steel out and felt pain in left shoulder.

Employee was operating continuous miner cutting in 1 Left when a piece of draw rock fell and hit EE on top of right hand. 14 stitches.

Employee was working on belt line. Employee was prying on a belt stand and strained left shoulder.

Employee was walking across section and hit head on a low place in roof.

Employee was helping make a belt splice and change a roller out. Employee was pulling on belt roller when employee felt pain in employee's back.

Employee bent over and picked up some roof straps and felt pain in neck and left shoulder.

Employee was working on a belt move. Using a utility knife to cut belt. The utility knife slipped and it cut right knee. Employee received 3 stitches.

Employee was working on belt structure. EE was pulling on it by hand and felt pain in back.

Employee was getting into scoop and twisted right knee.

Employee was getting out of the shuttle car and foot slipped on a rock and twisted right leg and right hip.

Employee was getting off of a buggy when EE's foot got caught on the buggy and EE fell into the coal rib and hurt EE's left shoulders.

Employee was using a sledge hammer breaking apart a miner chain when a piece of metal sheared off & hit employee on right pointer finger knuckle.

Employee was getting into scoop when left leg slid back and knee popped. Felt pain in it.

Employee was using a pry bar to change a roller. When employee pushed up on the bar they felt pain in their back.

Employee was shoveling coal at Nole belt tail piece. When EE felt pain in left shoulder.

Roof fall 60 feet inby spad 50041 on outby Return of MMU-006 in the No 1 Southwest Submains along the No 2B conveyor belt. The fall was 8 feet thick and 150 feet long.

Employee was changing a tire on a shuttle car. EE was working the tire off the wheel unit, the tire fell on EE's right foot.

Roof fall was at outby Return of MMU 006 at No7 entry 20 feet inby spad #50039. Fall is 90 feet in length and 9 feet thick. Second fall located 20 feet inby spad 50056 is 60 feet in length and 8 feet thick.

Employee was unplugging a mantrip from the charger when they slipped and fell backwards hurting their back.

The Dover conveyor escape hoist located in the primary at the bottom of the fan shaft. The main guide rope was found broken allowing the cage to come in contact with the shaft wall. The guide rope has been repaired.

Employee was putting up flypad boards & hit the wrong lever on the roof bolter causing the canopy to hit employee on the right side of head.

EE just finished roof bolting. EE backed the roof bolter out of the heading. EE was standing by the center of the machine when a piece of draw rock fell between the roof bolts 4' thick 1' wide 2 1/2' length. Striking ee on right side of hand cutting index finger, 43 stitches.

Employee was using an insert puller to remove an insert on the miner. Part of the puller broke and came back and struck employee on the left ear. Employee received 20 stitches on the left ear.

Trip was making test run and lose control fuse caused brake to set when brake set battery lead burnt off.

Employee was working on a tracking node for the tracking system. EE was standing on a 4 wheeler to perform the work. EE lost footing and fell off it. Hurt back, right arm and right leg.

Employee was hooking up chain and pull rope to pull conveyor chain back in place when the chain mashed middle finger on left hand. Employee received 2 stitches.

Employee was operating shuttle car. Going from feeder to the miner tire went over a rock in roadway bouncing ee into the canopy of the shuttle car. Hurt ee neck.

Employee was roof bolting. EE had put in finish and push steel. As EE began to drill the hole, loose shirt sleeve got entangled in the drill pad.

Employee was on rubber tired man trip when the right front tire on the man trip came off the track causing the steering wheel to spin. The knob on the steering wheel hit EE's right wrist.

Employee was shoveling tail piece when EE felt pain in their left shoulder and neck.

While employee was installing a roof bolt a damaged roof bolt struck the employee on the left side of face cutting it beside the left eye. Employee received 4 stitches. Came back to work the next day.

Employee was hanging roof bolter cable and felt pain in lower back.

Employee was in the mine office. Due to a medical condition EE blacked out and fell on a metal folding chair and hurt back and right shoulder.

Employee was using a knife to cut a piece of belt line. The utility knife slipped and cut the top of EE's hand. EE received 4 stitches on top of hand.

Employee was unhooking a supply car on the bottom of the slope. EE was using a slate bar to unhook the coupler when the bar slipped and EE hit pinky finger on a miner nut on the end of the supply car. Cutting EE's finger, EE received 7 stitches.

Employee was loading structure into scoop bucket when EE pulled left arm and twisted back.

Employee was breaking a metal band loose from a new roll of belt line. When the band was cut it flew back sticking EE in the left forearm, cutting it 8 staples

Employee was using a utility knife to trim a belt splice when knife slipped and cut the side of EE's right wrist. EE received 7 stitches.

Employee was pulling slack up on air hoses from impact wrench and tripped over rail and twisted back.

Employee was loading oxygen tank on flat car and felt pain in back.

Employee was working on belt structure. EE was packing a top roller when EE felt pain in back.

Employee was roof bolting. EE was walking back toward the back of the roof bolter to get more roof bolts when EE tripped over a piece of draw rock and twisted left ankle.

Employee was tramming a scoop when something flew into EE's right eye, dust or dirt.

Employee was lifting a VFP box for a miner, up to put on a supply car and felt pain in his back.

Employee was pulling up bolter cable slack and rehanging it, a scoop came through fly pad and caught cable and pulled it down on employee shoulder, hurting his back & groin (pulled muscle)

Making a mechanical splice in belt. He was taking roller structure off of belt rails, the top structure flipped over. Mashed his left ring finger - it fractured his finger. (No lost time - was off 11/26/15, 11/27/15 & 11/28/15 for Thanksgiving).

The mine failed to operate due to a stuck contactor. MSHA Hot Line was called. The contactor was repaired and the hoist ran through all tests as was put back in service at 2:45 pm.

Employee was roof bolting. He was pulling drill steel out of drill hole when he dropped the steel. It hit his left forearm cutting it. He received 2 stitches. Came back to work the next day. No lost time.

Employee was running shuttle car. Hit bump in roadway. Hit hard hat on canopy, hurting his neck. Then he was getting out of shuttle car he fell hurting his knee.

Employee was hanging roof bolter cable. The cable popped and received electrical burn to right hand.

Employee was shoveling belts when he tripped and fell hurting his back and right leg.

Employee was walking on section when he hit a cable bolt with the side of his head. Felt pain in his neck.

Employee was shoveling at long fork No2 belt head. when he slipped and fell and felt pain in his back.

Employee was hanging miner cable and his right shoulder started to hurt. Employee worked until 10/15/15. Went to the doctor and they took him off work.

Employee was walking to the power center when he hit his head on a fly pad board.

Employee was operating a man bus that struck a timber lying beside the track. The timber came inside deck on the man bus and struck him in the right thigh.

Roof fall on 006 section number 4 entry belt line. 9100 feet from the face. 10' high 9' in width and 100' in length. Fall will be cleaned up.

Employee was shoveling belts and felt pain in his neck and left shoulder.

Employee was helping changing belt line he put chain across his shoulder and reach across belt and felt pain in his back.

Employee was packing a saminco box when he slipped in mud causing him to fall to his knees. He felt pain in his lower back.

Employee was hanging miner cable. Felt pain in his back. Employee worked until 7-10-15 then went off work.

Employee was using a utility knife to cut a belt line. He was pulling it towards himself when cutting the belt line. The knife hit his right leg. He received 14 stitches on his upper leg.

Employee was changing a pop off valve on a miner cutter head. When oil plug was removed, oil blew out and got in employee's eyes.

Employee was moving miner cable and felt pain in his back.

Employee was washing the hood out on the miner. The door on the duck work fell down on his hand. It knocked the hose out of his hand. The hose flew back and hit him under his left eye and cut it. He received 5 stitches.

EE was lifting a rotation pot from a roof bolter. When he felt pain in his right shoulder. EE worked until 3/30/15 then went off work.

Employee was hanging miner cable. The hanger slipped and the cable struck him on the right shoulder.

Employee was roof bolting top. When the drill steel touched the top a piece of draw rock 8"" x 12"" fell and hit him on the hand.

Employee was walking between the shuttle car and rib when EE tripped over some loose rock and fell and hurt back.

Employee was operating a scoop at the end of the track scooping up a motor to load on a flatcar. He backed the scoop in a break, when he did he backed over a timber 6x6x8 ft long. His head hit the side post of the canopy on the scoop.

Employee was helping make a belt splice. When he picked up a belt roller he felt pain in his right shoulder.

Employee was scooping up gob with a scoop, when a piece of structure that was in gob hit him on the right shoulder and right side of his neck. bruising his neck.

Employee was shoveling Gob out from under stacker belt when he felt pan in his back.

Employee was using a chain ratchet to pull chain together and pulled muscle in his back.

Roof fall in No 2 Entry starting in intersection at spad No 8454 ending outby 3rd cross cut spad No 8438. Fall was 5' in height, 20' width, 210' long. Fall will not be cleaned up. The entrance to fall is dangered off and supported as per roof control plan. Fall was in old works.

Employee was helping set a jack under a beam when he slipped and fell and cut his left thumb. He received 5 stitches.

Employee was laying track rail when he slipped and fell on a track tie cutting his right knee he received 6 stitches.

Employee was walking by the belt line when he tripped and fell and hurt his left knee.

Employee was getting off slope car at mane hole No 4 when he slipped and twisted his right ankle.

Employee was changing speed reducer on a shuttle car. The speed reducer slid off the forklift and fell and hit his left middle finger - cutting it. He received 8 stitches.

Employee was changing bottom roller on belt line and felt pain in his right shoulder and neck.

The mine hoist failed to operate due to blown fuses. Called MSHA hot line at 10:52 PM. The fuses were replaced and the hoist ran through all test and put back in service at 12:00 AM.

Employee was roof bolting when a piece of draw rock fell out and hit him on his left arm and cutting it he received 14 stitches.

Employee was moving miner cable and felt pain in his back.

Employee stepped on the track and twisted his left knee. Track was wet were it had been watered.

EE was walking down No.& Entry when he ran into a torque tension roof bolt with his head. Felt pain in his neck and back.

Employee was changing bottom roller on belt line he did not have roller locked in place and it dropped out and hit the top of his right hand.

Employee was pulling fly pads off of curtain board when he felt something pull in his back. He felt pain in the middle of his back.

Employee was unloading rope bolts off of the fork lift and strained his back.

Employee was walking towards the roof bolter when he stepped in a hole and twisted his right knee.

Employee was roof bolting when the drill steel broke striking him under his chin causing a laceration - he received 6 stitches.

Employee was getting out of Roofbolter when he hit his right leg against the deck of Bolter and cut his leg. He received 5 stitches. Came back to work the next day.

EE was taking conveyor chain apart on miner with a hammer when he struck the miner frame with the hammer and left pinky finger between miner and handle of hammer. It fractured his little finger. Came back to work the next day.

Employee was using a utility knife to cut a piece of conduit when he felt something pop in his right wrist. Employee did not go off work until 11-22-12.

Employee was roof bolting when letting down the canopy it caught his left hand on bolts in his tray with the canopy it mashed his middle finger and index finger on his left hand.

EE was recovering belt structure he took a step forward and felt pain in his right foot.

Employee fell off of a 4-wheeler. When he fell he fell on his S.C.S.R. and fractured 2 ribs on his left side.

Employee was opening air lock doors and the doors shut on his right arm. it fracture his right forearm.

Employee was walking with some roof bolts when he slipped and twisted his back.

Employee was pre-shifting when he stepped on a nail.

Employee was unloading supply's off of a rail car when he tripped over the rail and twisted his right ankle.

EE was unloading structure off of a forklift when a piece of rail fell off and hit him on top of his left foot bruising it.

Employee was prying with a pry bar when he got his thumb pinched between pry bar and pin on the miner. Accident was not reported until 9/19/12.

Roof fall has occurred one entry over from spad #2218 outby the 2B overcast in the intake in the 1st left southwest parallel. Fall was 20 feet long, 6 feet height and 16 feet in width. The fall will be cleaned up.

EE was walking beside power box when a piece of rock fell out between bolts and hit him on his back. Strained his back and bruises.

Employee was loading a boom lift jack onto a tide car when he lifted jack he turned around and pulled muscle in his back. Employee worked until 7-11-12 then went off work

Employee was loading a bag of rock dust on fork lift when he stepped in front of fork lift and the tire ran over his right foot.

At 3:35 p.m. the mine hoist failed to operate due to a blown fuses. The fuse was replaced and hoist run through all test and put back in service at 6:30 p.m.

Employee was picking up belt structure and he felt pain in his lower back. Employee worked until 8/3/12 then went off work.

Employee was standing next to roof bolter when the roof bolter operator moved the bolter forward. The tire on the roof bolter hit employee's left foot.

EE was pulling on roof bolter cable and felt pain in his stomach. EE worked until 6/6/12.

EE was roof bolting when he bent over to pick up a roof bolt he felt a pain in his stomach.

Employee was working on a tail shaft using a pry bar. The pry bar slipped and the bar hit him on his left shoulder.

Employee was putting a joint of pipe on fire deluge at No.8 head. Slipped and hurt his right knee.

Employee was pushing track derailer down and mashed his fingers between derailer and the track. Middle finger and ring finger on his right hand.

Employee was working on a shuttle car using a chain ratchet. The ratchet slipped and the employee fell back against the shuttle car hurting his lower back.

EE was operating a miner when a piece of draw rock fell off of the conveyor and cut his lower left leg. He received 9 stitches came back to work the next day.

Was operating roof bolter, when a piece of draw rock fell out between bolts hit him on the right arm and right leg. Bruising it rock was 2' wide 2' long and 1"" thick.

Roof fall 50 feet outby survey station 25970. The fall was 20 feet by 60 feet by 4 feet high.

Employee was moving roof bolter up. Employee was on the off side of the bolter when a piece of draw rock fell out between bolts and hit him on top of his head and neck. Draw rock was 2 1/2 feet wide 5 feet long and 2 inch thick.

Employee was roof bolting when a piece of draw rock fell and hit him on his left knee. Bruising on his left knee.

Employee was roof bolting when his right index finger was pinched between pin steel and pot.

EE was changing out suction hose on the roof bolter he put his left hand over pot to check suction when his right hand slipped hitting joy stick allowing pot to raise and pinch left hand between pot and drill steel. It fractured top of his left hand.

Employee was operating shuttle car when he hit a hole in the roadway it bounced him up and hit his head on the canopy of shuttle car. Hurt his neck.

Employee was operating buggie when he hit a hole in road way and felt pain in lower back.

Employee had drilled a hole for roof bolt. When he took bottom section of drill steel loose, the top section of steel fell out of hole striking him on his right index finger cutting it. He received 4 stitches.

Employee was setting bits on the continuous miner. He was changing the inserts when he hit his middle finger on his left hand. He received 5 stitches. No lost time.

Employee was pulling roof bolter cable out from under loose coal when a piece of coal fell on his right hand bruising his fifth finger.

Employee was operating shuttle car when he ran into rib and bruised his left arm.

Employee was cutting a band that was around roof bolts when he cut it the band came back and cut his left leg he received 2 stitches.

EE was getting onto the man trip when he twisted around to get on he felt a pop in his left knee.

Employee pulled slack up from power box on the right miner cable when the rope slipped up on his hand and cut his right thumb. He received 5 stitches on his thumb.

Employee was roof bolting when a small piece of draw rock 1"" thick 6"" long and 6"" wide fell and struck him over the left eye cutting him. He received 4 stitches came back to work the next day.

Employee was operating a shuttle car when he hit a hole in road and it bounced him up into canopy hurting his left shoulder.

Employee was using a utility knife to cut belt line when the knife slipped and it cut his right knee. He received 5 stitches. He came back to work the next day no lost time.

EE was building brattice when he smashed his right index finger with a hammer he received 12 stitches on his finger.

A piece of draw rock fell out between bolts and struck him on top of his head.

Roof fall uccurred in the last open cross cut intersection of the number 1 entry. The fall was approximately 19 feet in width, 6 feet high and 35 feet long

Employee was tramming a scoop in #5 heading when his back started to hurt him.

Shuttle car operator was taking up his cable when the cable tripped employee causing him to fall and hurt his back.

EE got off of the mantrip and said his back started hurting.

Employee was building stopping and felt pain in his right wrist.

Take up pin on continuous miner struck EE on top of his left foot.

EE was at feeder dumping the shuttle car and a piece of draw rock came out of the car striking his right index finger. EE received 5 stitches and had no lost time.

Shoveling tail piece felt pain in left shoulder and neck.

The slope hoist had an electrical malfunction it was found while doing pre shift.

EE was using a utility knife to cut tape off of his boot. The knife slipped and it cut his right index finger. EE received 4 sticthes had no lost time.

Employee was roof bolting when the drill steel got stuck in the roof. Then the steel fell out of the roof striking him on the right hand little finger. He received 17 stitches on his finger.

EE was walking across section when he tripped and twisted his left knee.

Employee was walking beside the roof bolter and twisted his right ankle.

EE was roof bolting and had a piece of drill steel in his hand and was dropping boom of bolter and the boom caught the drill steel and pushed it down on his right foot. This fractured his 2nd toe next to his big toe.

He was trying to push drill steel up in roof top and strained his back.

Employee was hanging bolter cable. He tripped over cable and hit his head on the coal rib and cut it on the left side above his ear. He received 19 staples. No lost time. He came back to work his next shift.

EE was pulling up on uncoupler on the man trip and felt a pop in his left shoulder.

EE was roof bolting when his glove caught on drill steel and twisted finger he had a laceration on 2 middle fingers on his left hand. 14 stitches.

Employee was using a jack to raise a belt head. The jack bar slipped and hit him in the face and fractured his left lower jaw.

Employee was loading a welder and cable on a 4 wheeler and felt pain in lower back.

EE was cutting belt line with a utility knife when the knife slipped and he cut his left leg above his knee. He received 7 staples. Came back to work on 9/1/09 no lost time.

Employee was riding on a mantrip when they ran over a rock or hit a hole in the roadway and he twisted his back.
