Fall was found on 3rd North West Sub Mains starting 1 break inby spad #10338 at the beginning of the inter section and stopping 1 break outby spad #10377. Fall is dangered off and supported as per roof control plan. The fall will not be cleaned up.

Employee was changing a tire on a shuttle car. EE was working the tire off the wheel unit, the tire fell on EE's right foot.

Employee was unplugging a mantrip from the charger when they slipped and fell backwards hurting their back.

The Dover conveyor escape hoist located in the primary at the bottom of the fan shaft. The main guide rope was found broken allowing the cage to come in contact with the shaft wall. The guide rope has been repaired.

EE just finished roof bolting. EE backed the roof bolter out of the heading. EE was standing by the center of the machine when a piece of draw rock fell between the roof bolts 4' thick 1' wide 2 1/2' length. Striking ee on right side of hand cutting index finger, 43 stitches.

Employee was using an insert puller to remove an insert on the miner. Part of the puller broke and came back and struck employee on the left ear. Employee received 20 stitches on the left ear.

Trip was making test run and lose control fuse caused brake to set when brake set battery lead burnt off.

Employee was working on a tracking node for the tracking system. EE was standing on a 4 wheeler to perform the work. EE lost footing and fell off it. Hurt back, right arm and right leg.
