Employee was finishing up the right side of his cut while loading a buggy when he felt something hit him in the leg causing a laceration to his lower left leg.

Putting up 8 ft bolts when bolt got hung up on drill steel pinchers. The up pressure was hit in an attempt to get the plate unhung. Holding on to the plate, the bolt bent, pinching his finger against the drill steel pincher; requiring 12 stitches.

EE & other were running water thru the spray plaster machine to flush it out & the nozzle plugged up. When the nozzle cleared itself, the hose jumped out of work hand & sprayed him in the face. The force knocked his glasses off & got material in both eyes. Transported to hospital,had eyes flushed & given antibiotic.

Employee was attempting to straighten a bent 8' bolt. The end of the bolt was in the hole and when he shoved on the bolt he caught the end of the ring finger between head of bolt and drill chuck causing a fracture.

Employee was spraying stopping with fosrock when hose blowed off end of sprayer striking employee in the face resulting in material going into both eyes. Medical reportable only employee was released (material flushed from eyes and drops put into eyes)

Injured was operating a roof bolter on the left side. As he was dropping the operator's canopy, the moveable wing caught on a roof bolt plate. The injured was talking to the other driller and had put his hand on top of the rigid canopy. When the canopy came free from the roof bolt plate, it caught his finger.

INJURED WAS ATTEMPTING TO CANGE A BROKEN SPRING ON A RUBBER TIRED MANTRIP. HE RAISED THE MANTRIP WITH A FORKLIFT & WHEN HE WAS LOOSENING THE SPRING BOLTS, THE MANTRIP TEETERED & THE END DROPPED CATCHING INJURED'S RIGHT HAND BETWEEN THE IMPACT WRENCH AND THE GROUND. WHEN HE PULLED HIS HAND FREE, HE LACERATED THE BASE OF THE MIDDLE FINGER. TRANSPORTED TO ER & RECEIVED 7 STITCHES.

EE WAS ADVANCING CONTINUOUS BRIDGE SYSTEM AND WAS POSITIONED AT THE END OF THE TAILPIECE. THE CONTINUOUS MINER OPERATOR REVERSED THE CONTINUOUS MINER, FORCING THE EDGE OF THE DECK ON THE BRIDGE INTO THE TAILPIECE, WHICH RAISED IT INTO THE MINE ROOF. THE EE STRUCK THE MINE ROOF WITH HIS HEAD, CAUSING A STRAIN TO THE NECK.
