Operator was pinning top pulled wrong lever and let pot on bolter down and his rt shoulder
Low top pinner got hung. Operator tried moving pinner. While pulling with scoop, pinner slid sideways fowling him against the control inside deck.
Employee stated that when he started to pick the pinner cable up to move it, the operator took the reel up and the cable struck him in the face.
Employee using air sander to clean blade on grader. Sanding disk came apart, piece struck employee in the stomach.
EE stated that while operating the roof bolter he lowered the roof bolter pot onto his foot.
Employee was cutting a bolt down with a torch when a piece of slag fell through the corner of his glasses. (Started missing on 4/19/05 and returned to work on 4/22/05)
EE stated that while tramming the miner to set up in eight right the miner was crossing over a hump in the roadway, after topping over the hump the boom end of the miner swung to the left striking the EE on the right shoulder. (The boom on the miner was swung to the left while tramming.)
EE STATED THAT HE INJURED HIS LOWER BACK WHILE LIFTING THE ROOF BOLTER SHIELDS.
EMPLOYEE STATED THAT HE SUFFERED FLASH BURN TO BOTH EYES WHILE WELDING.
