Showing all 10Roof bolter helper (injured employee) was pulling slack cable off the roof bolter cable reel. The roof bolter operator accidentally started to wind the cable reel while the bolter helper was pulling it, causing the cable to jerk the bolter helper and strain his back.
While bolting he had his hand on roof bolt. Swung head in hitting his hand on canopy.
Bolter was backing up ee left foot got under back bumper coming off ledge.
