Employee operating dual boom bolter. While running roof bolter, dropped head on left foot. Smashed on top of metatarsal. Broke bone in top of left foot.
Employee was resting left arm on top of bolter ATRS, started bolter and ATRS raised to the roof. Left arm was crushed between roof & top of ATRS. Broken left arm above elbow.
Was straighting out roof bolt to install, pinched end of little finger between bolter and bolter head.
BOLTER OPERATORS STARTER STEEL WAS STUCK IN TOP. HE ATTEMPTED TO DROP DRILL HEAD DOWN TO PUT HIS DRIVER STEEL BACK ON. WHEN STARTED STEEL FELL OUT OF TOP, STRIKING HIM ON HIS LEFT HAND
