While breaking bead from a loader tire, with the porta power ram, the ram slipped and smashed the left thumb between the ram and the frame of the loader. This caused a finger laceration, fracture of distal phalanx of finger and open nailbed injury.

Showing all 8Employee was working with 4 other employees to start a water well. When the well was started the employee was looking at a flow meter when it failed and popped off the pipe it was mounted on. The flow meter hit employee on the right side of their face causing lacerations on forehead and around right eye. Employee was sutured to close the wound and has an orbital fracture.
Employee was bent over to grab a sample of fines next to a blast hole drill when a piece of metal fell from the mast. The piece metal struck employee in the top backside of EE's hard hat and then off EE's shoulder. EE was taken to hospital and diagnosed with a concussion.
EE was putting a 12 inch pipe in the 412 fusion machine and ee skin on the top of ee left ring finger was pinched between the pipe and a pin on the machine. A small laceration was the result and it required 3 stitches.
Employee was welding and cutting on a retainer pin in windy conditions. He felt something get into his eye around his safety glasses. He was seen by an optometrist where a small metal flake was removed. He was given a prescription for an antibiotic drop.
Employee is not sure how but a piece of metal got in his eye while driving to the shop. He had been grinding earlier and perhaps had something on his face. He was seen by an optometrist and a piece of metal was removed and a rust ring removed. He was taken off work for 2 days.
Employee (E) was removing cutting edge bolts on a dozer blade with an impact wrench. The bolt head backed out of the hole while it was spinning and cut through the (E's) glove causing a laceration to the left index finger. (E) received medical attention and it took 15 sutures to close the wound.
EMPLOYEE PLACED LEFT HAND BETWEEN BOTTOM AND MID SHELL OF CRUSHER TO ADJUST GASKET WHILE MID SHELL WAS BEING LOWERED BY CRANE ONTO BOTTOM SHELL. EMPLOYEE'S LEFT HAND WAS CRUSHED. (POST-SURGICAL EVAL.: PROBABLY WILL REQUIRE AMPUTATION OF MIDDLE, RING & LITTLE FINGERS, EACH AT PROXIMAL PHALANGE.)
