Employee stated that as EE was installing a rib bolt EE felt a sharp pain in EE's middle / left lower back.
The employee was installing bolts and went to lower the canopy when the seal of the canopy jack blew out and dropped down suddenly. The bottom of the canopy hit them on the top of their head and stoved them up.
The employee was attempting to push in the bottom rib bolt. While at the same time EE was holding the wrench to seat the bolt head. EE then spun the wrench, and EE's glove was caught between the wrench and bolt head.
While installing a roof bolt, the employee raised the drill pot to insert the roof bolt into the wrench. The employee's glove was caught between the wrench and the bolt and when the rotation was bumped it twisted their hand around the wrench fracturing wrist.
The washdown hose on the miner was coiled up on the rear of the machine with the valve end drooping over from the top of the machine to the floor. The employee was in the process of cutting coal when the frame of the machine caught the ball valve and broke it in two. The sudden release of water pressure caused the broken shut-off valve to strike the employee in chin.
Note: The employee did not begin missing work until 11/2/2021. Description: After drilling the inside bolt hole the employee inserted the glue and as EE was inserting the bolt into the hole EE felt a pop in EE's left shoulder.
While the employee was drilling to install a roof bolt, approximately 3' into the hole rock broke out around hole and fell hitting the drill pot and reflected into EE's right leg. The rock was approximately 30" x 12" and 3" - 1" thick.
Employee had removed the bottom plug from drill pot to work on suction. Employee placed right index finger inside the pot to check for debris and while doing so they inadvertently contacted the joy stick causing the drill pot to rotate catching the finger between the cleaner pins at the bottom of the drill pot.
Employee was in the process of moving the continuous miner from #9 entry to #6 right face and in between the #8 and #9 crosscut employee was pinned between the conveyor boom of the continuous miner and the coal rib.
While roof bolting, the employee placed the roof bolt on the push plate of the drill pot in order to push the bolt up far enough to place the bolt head in the drill chuck. The up pressure and the fast feed feature before the bolt was in the hole. This caused the bolt to bend and pinch right middle finger between the bolt and the drill mast.
The employee had EE's hand on the canopy post and inadvertently lowered the canopy catching EE's right middle finger between the collars.
As EE was lowering the drill pot, the pot pulled off of the wrench and the wrench sprang back towards EE and struck EE's chin.
Employee was mining the 2nd cut out of 3 lt. crosscut when a piece of rock bounced out of the shuttle car EE was loading and struck EE in the mouth.
Employee had positioned side of the machine to install the next row of bolts. EE then extended the slide of the canopy out and attempted to raise the canopy higher. EE unknowingly left finger on the top side of the canopy which pressurized against a rock brow causing a fracture to right pinky finger.
EE drilled the starter hole and then continued to put the pusher and finisher in the hole to complete the hole depth. An attempt was made to fast feed the steel into the hole. In doing so the steel bowed at the same time they hit the rotation. This caused the steel to strike EE in the palm of the hand.
The employee had drilled through one end of a roof strap while against the mine roof. As EE backed the steel out of the hole, it caused the strap to fall, swing around, and hit EE in the side of the face.
EE was installing belt hangers. He stated that the chuck was coming out of the wrench and while holding the wrench while pushing upward his finger was caught between wrench and chuck.
Employee unplugged course refuge bypass chute that was blocked by a crib block. After removal, employee was standing on the floor below watching and listening to ensure start-up would be successful. Unbeknownst to the employee, spillage had occurred on the floor above. During start-up, a piece of rock fell and hit the employee on the head.
The employee was loading the roof bolt machine. EE was setting a handful of bolts on the machine as it moved forward to the next row. The tire ran on top of his foot.
Employee states while drilling the foot jack slipped off of a rock and caused the canopy to hit his head.
The employee was grading bottom with the CM. As he was trimming the bottom the roof "bumped" and drew his attention to the roof, the CM cutter drum was still cutting the bottom and while looking up a piece of foreign material went underneath his glasses.
The employee was placing the drill steel into the drill pot. He was unaware that his glove had become pinched in-between the wrench and the steel. He initially thought his hand was clear, he started to rotate the steel which pulled his hand/glove around the steel.
The employee was bolting roof. The roof bolt nut had become stuck in the drill chuck. He attempted to remove it by placing a piece of drill steel against the mine floor and up into the bottom of the pot. He exerted downward pressure with the joystick and inadavertently maneuvered the joystick into the rotation phase and the bolt spun and hit him in the ear/face area.
The injured was stepping out of the deck of a roof bolter, it was on top of a rut. When he stepped out of the bolter, it slid over against his foot/ankle. He did not start missing work until 5/29/2012 due to this condition.
The injured and another employee was putting a ram jack for a scoop onto a Johnson ride, as they were setting it down it slipped and caught his fingers in-between the jack and the ride.
The injured was operating the bolter to install a fly board for the face of #1. Curtain was hung across the intersection to keep air in the face. After he drilled his hole and was pulling the steel down he used his hand to help bring it out and the air blew the curtain into the driller station catching the steel and wrapping his arm into the steel.
Injured was sawing a timber when the blade became stuck. He forcefully tried to pull it back towards him. The blade came out of the timber unexpectedly and struck him on the left forearm, causing a laceration that required 10 stitches to close.
The injured was installing a roof bolt, he was spinning the the drill chuck and bolt counter clockwise so that the bolt head would seat down into the drill chuck prior to pushing the bolt up. He was holding the bolt as it spun; his glove was snagged by the bolt which jerked his hand/thumb around the bolt.
The injured was installing the outside roof bolt with a curtain hook on top of the plate. As he pushed the bolt up, he said he left the plate around 6-8 inches from the roof. He then tried to adjust the direction of the curtain hook. The unused re-coil from the up pressure broke the jacket of glue, pinching his left index finger.
Injured was tightening wheel lugs on a DBT Coal Hauler. The bolt he was tightening broke, causing him to fall forward towards the hauler. Injured struck his mouth on the hauler, causing a large cut on his lower lip that required 2 stitches to close.
EE was bolting top, he had put extension on the steel and was raising it into the hole when the steel pressurized in the hole and it sprung into the drill chuck hitting his hand.
Injured was tramming bolter into #4 Entry to bolt 4 right Crosscut. He moved from deck to "inch" tram controls. As machine pivoted around, a curtain board on machine caught on a rib and sprang towards operator, stiking him in the mouth. Busted upper lip and possible fracture of facial board.
Injured was usaing a torch to cut wheel straps loose from wheel of rail car. While doing this a piece of hot slag went in his ear causing a burn. Sought medical treatment on 3/23/09.
Employee put in the last outside bolt. He raised the jack and a rock fell off the canopy and hit him on the head and neck.
Roof bolt got fouled in top, upon dislodging bolt EE accidentally hit rotation which spun bolt casusing bolt to strike EE on the right forearm. EE right forearm was fractured.
Shoulder strain due to drilling. Not immediately reported.
