Employee stated that as EE was installing a rib bolt EE felt a sharp pain in EE's middle / left lower back.
Sitting in the maintenance office and cleaning EE's ear with a pocket screwdriver. The chair had wheels and EE went to lean back and felt like EE started to fall; EE's reaction to balance self caused EE to poke self in the eye with the screwdriver.
The employee was using a chain ratchet (3-ton) to secure a tailpiece into the scoop bucket. As EE ratcheted it back the body of the ratchet twisted and the handle pinched EE's hand between it and the scoop bucket.
The employee was carrying a top roller to install for section advancement and stepped into a divot in the bottom. EE's ankle popped and twisted in the process.
Employee states that while cleaning up rock a piece of top measuring approximately 5" wide, x 6" long, x 5" thick roof fell from between the bolts and struck EE on the leg.
Employee states as EE was walking down entry EE either stepped in a hole or on a rock and twisted EE's knee. EE also states EE felt EE's knee pop.
The employee stated that EE became dizzy and fell off the ladder while plastering a stopping. The ladder was 4', and total height in area was around 9 1/2', EE fell between 2' and 3' to the ground.
EE was walking to the back of the machine to put oil in and slipped while crossing the cable.
While bent over putting oil in bolter a piece of roof rock fell and hit them on their arm/shoulder. Because they were bent over in the walkway of the machine, the rock fell perpendicular and broke across the machine. The initial rock measured approximately 77" long, 36" wide, and 2 ½" thick. Mining height in this area is 13'.
On January 29th, 2026, Mine management personnel received info about an injury that happened on December 3rd, 2025. Employee received a laceration behind EE's right ear from a roof bolt that required 3 stitches. The employee did not notify mine management of the incident, went home and went to the clinic on EE's own without notifying mine management of the injury.
The employee was installing bolts and went to lower the canopy when the seal of the canopy jack blew out and dropped down suddenly. The bottom of the canopy hit them on the top of their head and stoved them up.
A roof fall occurred near Spad # 34462 on Panel D1 (5A Belt Break #38) of the active Section No. 5 at 10:55AM. This fall was in the LOB where the miner was loading at the time. The fall is approximately 20ft X 35ft X 7 to 8 feet thick. The fall has covered the right continuous mining machine.
A roof fall was found near survey station 33820 at the 23 cross-cut on the B Submains on 4 section. The fall measured approximately 18', x 18', x 10' high.
Employee was working on a machine and was moving crib blocks. While moving them, the tech felt a pain around their navel and there was some slight inflammation.
A roof fall that was not previously considered reportable under the criteria is now being reported due to MMU equipment being installed in the area. At Spad # 30889 an unintentional roof fall occurred above the anchorage zone. The fall measures 20'x20', x 7-9' high.
EE was replacing the pin on the scoop lift jack pin. EE leveraged the jack with a chain and slate bar and was going to check alignment. EE used the left middle finger when the jack fell catching their finger.
The employee states that that while operating the Romac Water Truck it began to get stuck (Hung-Up) so EE looked backward to try and back up. While still looking backward EE shifted back into the forward gear which caused EE's face to lunge forward and hitting EE's face into the dash.
Employee was unplugging the ref chute on 7th floor. As EE was attempting to dislodge the material with a slate bar, the tip of the bar slipped off and caused EE's prying arm hand to pinch between the bar and the chute door.
Power was lost off of the main grid. An employee was discovered to be a couple feet from the bottom on the elevator. Generator was started around 5:46 and they were lowered and exited the elevator at 5:49.
An unintentional roof fall was found by the weekly examiner near spad 30803 at 8:03. The area was cribbed and dangered off on all approaches. The fall was approximately 8' in height, and 20' long and wide.
A sudden PLC fault allowed slack to form and force the No. 4 hoist rope over into No. 1 hoist rope on the sheave wheel. The elevator was 30 feet from the mine floor and 8 miners inside. Shortly after the elevator was able to be manually lowered to get the miners off. It was placed back in service at 1:30PM.
A fire was discovered on the number 16 bolter while making a preshift exam in the number 9 entry of the number 4 section. Fire was extinguished by using fire extinguishers, rock dust, and water deluge.
Note: The original date of injury was 8/6. The employee continued to work until November 22 at which time the results of an MRI revealed that time off work would be needed. Description: Employee smacked the drill steel against another piece of steel while rib bolting to get it unstopped. EE felt a sharp pain radiate from EE's shoulder up into EE's neck.
Note: The original diagnoses was an infection. It was not diagnosed as a hernia until after seeing a specialist on 5/28/2024. Description: While dragging a timber down the offside of 3C head EE felt a pain in EE's groin area. The timber measured 6"x6"x10'.
When coming off the section to outside the mantrip hit a hole and caused the employee to bounce up into the canopy and back down.
While adjusting a wire mesh panel, a piece of top rock fell and broke through the previously installed and next panel of mesh and hit the employee's left hand/finger.
The cord to the elevator door switch was damaged. This caused the elevator to not work from 11:30PM until 1:35AM on the 20th. There were no employees when this occurred.
Employee was passenger on a rubber tire mantrip exiting the mine. The operator of mantrip steered closer to rib to avoid a hole in roadway at which time employee as passenger had their foot slip off the framing of the man trip causing EE's foot to get caught on a ledge and jerking EE's leg backwards.
The employee was holding up the curtain to allow the scoop to travel bucket first down the No. 4 entry. As the scoop trammed forward the deck of the scoop hit the injured employees left leg.
The employee was attempting to push in the bottom rib bolt. While at the same time EE was holding the wrench to seat the bolt head. EE then spun the wrench, and EE's glove was caught between the wrench and bolt head.
One strand of the No.6 traction cable broke and unwound causing the strand to bunch up on the drive motor sheave wheels. This caused the rope gripper brake and the car safety wedge brakes to engage. There were 4 individuals on the elevator when this occurred. No injuries were incurred.
The employee was changing the bearings in the #24 tow motor. The machine was blocked, and a crib block placed under the hub and a bottle jack placed under the trunnion. After changing the top bearing, the employee raised the trunnion with the bottle jack to remove the crib. The trunnion slipped off the jack and caught EE's right forearm between the hub and the crib block.
Employee was installing the fork back onto the forklift. While trying to get it into place EE's left-middle finger was caught between the back-clip of the fork and the cleat of the fork-frame. EE received 2 stitches in the left middle finger.
As the EE was loading the roof bolter they stepped off the back of the machine to get more supplies and sprained an ankle.
The employee was carrying two full buckets of miner bits and tripped on a rock. They fell into a rib and felt pain in their back, legs, and pelvic region.
Employee was in the process of positioning the drill station to drill a corner bolt on the outside of the entry when a rock/kettle bottom fell from the roof, sliding down the roof screen and struck EE in EE's back. The strike to EE's back pushed EE forward causing EE to hit mouth against the canopy.
While bolting, the employee was placing a piece of mesh on top of the ATRS and adjusting it with EE's hands. A rock fell and caught the inby corner of the mesh and caused the mesh to spin around and strike EE in the mouth area and knock out 2 teeth.
While loading a top roller into a scoop insert, EE stated felt a pop in lower back.
Two employees were hanging bottom roller on structure during a belt move. The roller was being lifted simultaneously by each employee with the bottom belt laying loose over top. The injured employee felt a pop in shoulder. Note: The employee worked until 6/12 when employee was then required to have surgery to repair damage sustained during this action.
While traveling up 3A belt conducting an on-shift the EE's ride broke down. When EE stepped off and began to stand up EE lost balance and fell back into the ride. EE's left foot was caught between the frame of the ride and the belt frame causing a fracture to the ankle and fibula.
The employees were pulling in/out 1000' of belt. While the injured employee was pulling the redwing (AKA Duck-Bill) over by hand to keep the belt straight the redwing slipped off while under pressure and struck the employee in the forearm.
The employee was removing the lower ATRS jack with a slate bar. The jack was partially stuck and when EE was prying up, EE felt a sharp pain in the right bicep.
The employee was walking from 3 towards 2L at the Paint Creek Shaft site when EE slipped/fell on a ledge and tried to catch self with a chain-fall hoist chain that was hanging from the roof. In the attempt to catch self from falling a burr on the chain cut EE's palm.
While in the process of changing scoop batteries when they were lowering one side of the lid their fingers were caught between the battery lid lock and battery tray.
The employee and crew had stretched out an old piece of high-voltage cable for repair. When the EE stepped out of the forklift, EE's ankle rolled and fractured two bones in their foot on an old high-voltage cable plug.
While employee was stepping down off a Caterpillar 740 articulating truck to refuel, EE's foot slipped on the last step. EE caught self with right arm and felt pain in right shoulder and the pain also radiated to EE's neck. Note: Employee continued to work until 10/11/2022 when surgery was needed.
While installing a roof bolt, the employee raised the drill pot to insert the roof bolt into the wrench. The employee's glove was caught between the wrench and the bolt and when the rotation was bumped it twisted their hand around the wrench fracturing wrist.
Two employees were re-stacking a stopping that had been being used as the diesel regulator. There were 2-3 rows of block left attached to mine roof that became dislodged as the witness was straightening blocks already stacked in place. A block fell causing a fracture to the injured EE's left pinky finger.
Employee was tramming the #2 scoop thru a set of fly pads in the #7 entry when EE ran over a rock and it bounced EE into the canopy causing EE to hurt back. After an MRI was done it was determined it was found that EE has degenerative disk disorder.
The washdown hose on the miner was coiled up on the rear of the machine with the valve end drooping over from the top of the machine to the floor. The employee was in the process of cutting coal when the frame of the machine caught the ball valve and broke it in two. The sudden release of water pressure caused the broken shut-off valve to strike the employee in chin.
