The roof bolter operator was drilling a hole for a roof bolt when a thin piece of shale slid down the steel (4 1/2 inches by 9 inches, paper thin to 1/4 inch thick). Before the operator could stop the rotation, the shale broke loose hitting the operator on the tip of the nose. Stitches were placed in the nose and no work was missed.

Employee was using boom pressure on the roof bolter to get steels together when they moved suddenly striking right hand. Employee was treated at the ER where x-rays revealed a fracture. Employee was taken off work with restrictions preventing them from performing their normal duties.

While trying to remove the outside roof bolt that had bent entering the roof strata during the bolting process, the operator used the boom to remove the roof bolt and start over. In doing so the operator swung in pulling the pin and a piece of roof strata down with the boom, catching their left hand between the tray and the roof strata. Started missing work on 2/6/2026
Employee was roof bolting when partner told them they were advancing to the next row. EE acknowledged them, but as the bolter trammed forward they reached for pins placing left foot under the bolter tire. X-rays were negative for a fracture and EE was given a full release. EE was taken off work the following day as they were having pain preventing them from performing normal job.
Roof bolter was reaching into pin tray to retrieve a steel when a small head came from the roof smashing the tip of ring finger on right hand. Resulted in a fracture and started missing work on 10/29/25.
The roof bolter was drilling a hole for EE's rib pin when a rock fell hitting the canopy swinging in and striking EE's right shoulder, lower back and EE's right knee. The rock pushed EE's knee into EE's boom causing a contusion and swelling to the knee. Started missing work on 10/17/2025.
Employee was dropping the bolter cable as it was tramming. EE was unaware that EE had stepped between the bolter and the rib and as the pinner swung it struck EE's lower left leg resulting in a fracture. EE began missing work the following day 9/26/25.
Employee was drilling a hole when a piece of draw rock fell striking EE's right forearm causing a laceration. EE was treated at the ER where EE received four sutures and a full release with no restrictions. Returned EE's next scheduled shift.
Employee was pinning when a piece of draw rock fell striking EE in the middle of EE's back on the right side. EE finished EE's shift but when EE came in the next day EE wanted to be checked by a physician. X-rays were negative for fractures but EE said EE was unable to work so EE was taken off work beginning 8/6/25 as EE was unable to perform EE's normal job.
The operator of the bolter was tramming past the supply trailer and pinched EE's 4th and 5th fingers on EE's left hand between the trailer and the roof bolter. Resulting in 3 stitches in the pinky finger and a fracture to the ring finger. The employee started missing work on 7/28/2025.
While drilling a hole for the outside pin a head dislodged from the roof hitting the roof bolter operator in the right leg just above the ankle causing a fracture. Started missing work on 6/12/2025.
Employee was installing a pin and a head came out of the roof, striking EE in the right arm. On 5/21/25 EE was diagnosed with a fracture to EE's right elbow and was taken off work as EE was unable to perform EE's normal work duties.
Employee was dropping EE's canopy with EE's right hand while placing EE's left hand on some boards near the front of the tray. The canopy pinched EE's left pinky finger and when EE reacted with a tug, EE pulled the nail off.
While the mechanic was cutting a pin out of a lift jack on a diesel scoop with a torch, the grease port was exposed letting the grease get hot and shoot out hitting the mechanic in the nose, cheek, and bottom lip causing a burn. Employee started missing work on 5/12/2025
While in a tight space laying on EE's side working on a bolter boom the mechanic was using a die grinder to clean a hole for a pin when the die grinder jumped back hitting the left forearm causing a laceration that took 4 sutures. No work was missed.
Roof bolter was moving when the reel got tight pulling the cable down hitting the roof bolter in the side of the hard hat causing a laceration that required 4 sutures. No work was missed.
Employee was putting pin steels together when EE applied upwards pressure causing the steels to pop the webbing of EE's left hand between the thumb and index finger. EE was treated by the mine nurse where EE received seven sutures which prevented EE from doing their normal job and began missing work on 2/13/25.
Roof bolter was trying to connect two steels together. The steels were touching each other with a little boom pressure and when EE pulled on the one in the chuck, they popped together hitting the right thumb causing a fracture. Worked on 02/05/2025, started missing on work 02/06/2025.
While the opposite operator had EE's hand on the T-bar, the operator was spotting a pin outby, dropped EE's boom hitting the ATRS pinching the opposite operator's hand between the pad and T-bar. This caused a laceration that required 12 stitches and started missing work on 1/16/2025.
Opposite operator roof bolter was drilling a hole for a roof bolt when the operator pinched right index finger between a board and the drill pot. Started missing work on 12/20/2024.
Employee was installing wire mesh on the roof during pinning cycle. EE was holding the mesh with left hand as EE started a hole. The steel snagged the mesh causing it to twist on EE's left index finger and cut it resulting in six sutures. EE was given a full release and returned to work 12/18/24 for EE's regular shift.
The miner operator was cutting some hard coal and a piece of steel off of the miner bit hit them in the neck. This required 4 sutures and no work was missed.
While using a cut off wheel on a grinder to cut a lock on a golf cart, the grinder kicked back and cut the mechanic on the top of EE's left hand, requiring sutures.

A grinder was dropped by a belt mechanic while the blade was engaged, hitting a two man diesel ride, it bounced off of the jake and hit the index finger on the right hand of a spectator causing three sutures. No work was missed.
Operator dropped the boom when hitting the pin that was made up to put in the roof, the board that was on the premade roof bolt slid over and the boom caught the board which hit the operator in the right jaw causing a decade tooth to break on the upper left side. Missed one day of work on 10/25/2024
While trimming a piece of metal off of a scoop with a cutting torch a piece of hot metal bounced up getting behind the cutting glasses burning the left eye.
The employee was standing on the opposite side of the bolter and the operator repositioned the bolter running over the toes on the right foot of the opposite operator causing a fracture to the big toe and the third toe. Started missing work on 09/25/2024.
EE attached a whip check to a Hilti drill bit that was hung up in the locking collar of the drill. EE and another employee picked up the drill and began to pull on the bit. Accidentally the drill was engaged causing the whip check to wrap around the bit and therefore pulling tight on IE's right index finger cutting it requiring stitches to close.
Employee was lowering the boom to start second steel when the steel caught between the boom and employee's right foot. The steel was pushed into the foot causing a fracture at the base of fourth toe and resulting in eight sutures. Began missing work 9-11-24.
Employee was drilling the second steel up. The second steel broke and a piece of the steel hit the rib and then hit EE in the back causing a 1" cut that required 5 sutures. No work was missed.
Employee was loading gob with a continuous miner and a rock that was on top of the miner slid down the tail of the miner and onto the shuttle car and then slid off the car onto the employee's left foot. Started missing work on 6/21/2024 due to a fracture in left foot.

Employee was using a porta power trying to break the cab canopy loose on a roof bolter. The electric porta power was loading up when it shot out of the cab contacting the employee in the mouth. Resulting in a laceration to the upper lip with 3 sutures and 3 teeth fractured.
Employee was swinging the boom out when a piece of rock fell out hitting the boom causing it to bounce off striking them in the right knee. Employee finished the shift and went to see their doctor the next day who put them off of work on 01/17/2024.
Employee was starting EE's steel when a piece of rock went over EE's safety glasses and irritated EE's right eye, EE finished the shift and went to the eye doctor the following afternoon. The doctor gave EE restrictions and EE missed work on 12/19 to 12/20/23 returning to work on 12/21/23.
Employee was bolting when some gob fell out hitting their right middle finger causing them to receive 4 stitches. They returned to full duty 10/25/2023.
