The employee was moving a slusher. The Slusher caught on the wood deck floor causing the slusher to raise up approximately 12" on one side. He released the slusher brake which allowed the slusher cable drum to free-spool out cable, which then allowed the Slusher to fall down onto his left foot.

Employee was drilling a breast down round with a jackleg. When he pulled out the steel he strained his right shoulder. The employee went to the doctor 10 days later on 4/3/12. He was able to continue his regular duties. He had shoulder surgery on 9/1/12 and is on restricted days. More details are on file with the MSHA Denver office.

Employee was operating a Jackleg collaring a hole in the back for bolting purposes and while in the process of doing this, the machine buckled and the employee held on to the drill. The machine fell to the rib and pinched his Right Index Finger between the machine and the rib.

Miner was drilling a face round with a jack-leg; he went to move the leg forward to continue drilling, when he started moving the leg forward he felt a sharp pain in his lower left back.

The employee was operating a jackleg, while inserting a split set bolt. The pivot point gave way, pushing employees right hand smashing the fingers between the cob and rib.

While blasting holes in the 4550 N. Block #8 Stope the drill steel hung up and pulled the machine out of the miners hands causing the machine to spin around towards the miner. When the machine spun around it struck the EE's left elbow.

Employee was drilling a back hole, with a stoper. While drilling the employee hit a bolt and bound the stope machine. Employee was trying to break the machine loose, causing the employee to pull his left shoudler.

Employee was driving a bolt with the Jackleg, grabbed the maching to move it to a new hole. When he went to set the jackleg the corn-cob was in contact with his body. was operating enough to thrust towards the rib smashing his left thumb.

While pulling core tubes, the EE was guiding wire line onto spool w/hammer. His gloves were pulled into the wireline drum where his 3 middle finger of (L) hand were caught in the cable wrap. I n a jerking reaction, the EE pulled back to free his hand and subsequently amputated his 3 middle fingers below 1st knuckle. At hospital, required amputation of middle finger to 2nd knuckle.

Removing old bolts from cutting edges. Bolt came down out of lock. Reached up with hand to hold bolt in place, spun nut with impact, bolt fell down resulting in cut to left hand.
