Miner was fin-hoeing blasted ore from top of the muck pile.He created an opening to the backside of the muck pile releasing residual gases trapped from the blast.The miner breathed these gases,became dizzy and retreated from the muck pile and sat down when he momentarily lost conciousness.A co-worker helped him to the man way.The injured miner was given oxygen and left the stope.

The employee was conduction his work place exam. While inspecting the back he tripped over a rock and fell. When he put out his hand to catch himself, as he fell his right palm landed on a rock cutting though his glove, causing a laceration to his palm.

The miner was checking on the backfill foam in the stope. He heard a leak coming from the bull hose in the man way, two sets down from the stope level. While he was climbing down the hose came undone & squirted backfill foam on him, knocking his glasses off.

After scaling a loose rock from the back, the miner drilled a hole so he could install a 6' split set bolt. when he went to reposition the jack-leg another rock fell from the back and struck his left hand causing a laceration and a fracture to his hand.

When the employee reached the second landing in the manway a 3' x 12' piece of timber fell over and struck her on the hard hat. The employee was released to full duty at the time of incident. The employee was still sore and went back to the doctor, at that time she was placed on modified duty.

EE was working in a cribbed manway in the 5050 8N, he started to feel light headed and said he blacked out for a split second and fell approximately 10' to 15' to the landing. EE sustained a bruise to his left leg from the fall.

Injured employee and helper were bolting the ribs in the Alimak raise when a rock fell out of the rib, hitting him on the left hand. His hand was on the jackleg which contributed to the severity of the injury.

The employee was moving a slusher. The Slusher caught on the wood deck floor causing the slusher to raise up approximately 12" on one side. He released the slusher brake which allowed the slusher cable drum to free-spool out cable, which then allowed the Slusher to fall down onto his left foot.

Employee was drilling a breast down round with a jackleg. When he pulled out the steel he strained his right shoulder. The employee went to the doctor 10 days later on 4/3/12. He was able to continue his regular duties. He had shoulder surgery on 9/1/12 and is on restricted days. More details are on file with the MSHA Denver office.

After the employee finished scaling he was fin hoeing at the face when a rock fell from the back. The rock hit his hard hat then rolled down his back causing a laceration in the middle of his back.

Employee was operating a Jackleg collaring a hole in the back for bolting purposes and while in the process of doing this, the machine buckled and the employee held on to the drill. The machine fell to the rib and pinched his Right Index Finger between the machine and the rib.

Employee stepped out of a tractor onto a rock twisting her left ankle. The employee received a full duty release from the Emergency Room @ the time of incident and performed her regular job duties up until the time of surgery. The surgery on her ankle was on 08/24/2012 and is restricted days only, no days away from work.

Employee was cutting plastic thread protector off of a new drill steel with a utility knife. The knife slipped and the employee cut his hand that was holding the drill steel. The cut required 2 staples in the palm of his right hand below his middle finger. An attempt was made to submit this on 2/10/12 but this system was down for maintenance

The underground trainer was in the process of training a new employee on the secondary Alimak escape way. They started back down when the Alimak stopped on the rail. The underground trainer went to the lower deck to start trouble shooting the problem. Upon further investigation it was determined the Centrifugal brake system had a brake shoe failure.

Miner was barring down a piece of loose in the 5050 Vein 20. While scaling, a rock approximately the size of a baseball came down and hit his scaling bar and then struck him on the hat which in turn was pushed down onto the bridge of his nose causing a laceration that required stitches.

Two miners were coming down the Alimak Raise when the top motor on the Alimak failed. The Alimak was then stuck in place where it could not move up nor down. Attempts were made unsuccessfully to disengage the motors so that the units controlled free wheel system could be used to come down.

EE had just finished drilling a round when he set his jack-leg drill up against the rib so he could start cleaning his lifter holes. The machine was not secure when it tipped over and landed on ee right hand causing a laceration between the ring and little finger that required stitches.

EE was using a 4' pipe wrench, in the drill doctor shop, to loosen up a part on a jack-leg drill leg. When EE put pressure to the wrench, the wrench slipped causing EE to lose his balance forward and when he took a step to catch himself, he twisted his left ankle.

EE was drilling a face round with a jack-leg machine. When ee finished drilling a blast hole in the burn, he started pulling the drill steel out of the hole and the steel puller bounced up allowing the drill steel to come out of the drill causing ee to lose his balance and he fell backwards on his shoulder.

In the 5190-4100N Access, the SW Nose Pillar had sloughed down and went beyond the existing ground supports anchor points. Mud seams are present in the area and are believed to have contributed to this occurence.

On March 1st, 2011, EE was connecting pipe to the concrete pump at the 5050 East Lateral Draw Point #2. EE was connecting a steel pipe to a rubber concrete hose. While holding the lines in place and attempting to close the clamp around the connection, EE felt a pain in his back.
