I.E. was standing on the longwall pan line when 1 of the recovery bolters turn on EE's cable reel causing the bolter cable to hit Employee in EE's lower legs causing EE to fall hitting EE's left arm on the pan line. Employee had 8 stitches in EE's left arm.

I.E. stated that they had a rock slide from between the screen wire and the roof striking employee on the left side of their nose. X-ray shows a fracture and the employee received 5 stitches on their nose. Employee has a full release.

I.E. stated their loose-fitting sleeve was caught between roof bolt and chuck pulling the sleeve off the shirt. Employee had full release with no restrictions until 3/26/2025 they were complaining of back pain due to incident and doctor took them off work.

I.E. stated they was helping roof bolters put up rib wire after the top was bolted, after roof bolter tacked up rib wire i.e. turned their back to the rib and a rock fell striking them in the right shoulder. I.E. went to the hospital for X-ray, no broken bones or dislocations. Doctor released employee on restricted duty.

I.E. stated they were cutting top LW chain on the LW head drive after the chain had broken, all chain tensioners were released, and slack was in chain. When I.E. cut a piece of chain that was on the head drive the piece had stored energy and hit I.E. on their right forearm. I.E. went to the hospital and received 4 stitches.

An excavator operator went to swing the boom of the excavator & as the operator swung the boom the bucket came into contact with an 18" plastic pipe lying next to the excavator, this caused the pipe to move suddenly striking IE on the left side lower leg. IE was transported to a Hospital and had surgery to lower left leg that was fractured.

The IE was using a lever hoist to pull a piece of casing into a borehole. As tension was being applied a weld broke on the hoisting lug causing it to come lose and strike the IE in the back of the head.

I.E. was removing a damaged top roller from the take-up of the slope A belt, when I.E. cut the damaged roller loose it popped up and caught right index finger in between the roller frame and conveyor belt rail, causing a fracture to the finger. I.E. started missing work on 07/12/2021 after a follow up with an Orthopedic surgeon. The Doctor said the finger is healing good.

IE pulled 2 tubs of longwall chain onto the pan line using a shield hauler. IE was in the process of pulling the 3rd tub on. A 1/2" chain got caught in the tail sprocket binding the longwall chain. IE stepped out to see what was stuck. In the process, winch rope came undone, slinging foundry hook. Foundry hook ricocheted off chain tub than hit IE, breaking 2 bones in lower leg.

The IE was using a cutting torch to cut a piece of pin holding a jack in place on the tail of a CM. In the process of blowing the cutting material away, a piece of the metal got in EE's eye.

Injured person was roof bolting when the drill steel clogged causing EE to lower the drill box to see if EE had suction at the chuck. When EE put a hand on the chuck the drill steel fell from the roof striking and fracturing left ring finger.

Employee was installing chain hangers in #1 entry on HG7. When EE raised EE's boom on the right side of the bolter, a 5ft steel became caught on the boom fan. When it dis-lodged it struck the employee on the right elbow causing a laceration requiring 5 stitches.

Employee spotted a DA ram on shield feet. After the shear left employee was securing the ram to the shield feet standing in soft bottoms beside the pontoon. During this time positive set came thru and set the shield catching employee's right foot under the headgate pontoon, fracturing the big toe on employee's right foot.

Employee was in the process of removing the bolts from the right side tram motor on a Double Boom Roof Bolter, when employee's ratchet slipped causing employee to jam employee's right arm on the mine floor. This resulted in a strain to employee's right elbow. Employee continued to work up until 3/6/2017.

I.E. was in the process of installing a roof bolt, he was holding the bolt with his left hand while raising the pot with his right hand. While stretching to place the bolt head inside the wrench he slipped and hit the rotation. This resulted in I.E. wrapping his left hand around the bolt which caused a fracture to his left thumb.

Employee was in the roof bolting process and had drilled his hole and was bringing his drill steels downward out of the hole with the steels rotating. he reached up and put his hand around the steel to keep down pressure on the steel and his glove caught on the steel which caused the injury.

I.E. had been shoveling at shield 210. When the shearer arrived at the tail to cut out the I.E. was removed by the more exp. workers until the cutouts could be made. After the cutouts the shearer left the tail area and SRB was started. The I.E. was standing with his back to the shield when the SRB advanced the shield pinching his right foot between the shield toe and the relay bar.

Employee had ran dozer out of fuel on raw coal pile. After restarting the dozer the employee was going to get fuel. Employee backed up knowing to turn right & turned left accidentally causing the dozer to slide into a feeder that was being fed by another dozer. The employee states he hit his head when dozer came to rest.

Employee was installing angle brackets with a roof bolting machine, while putting the roof bolt into the hole, he heard a pop in his right elbow. He was working in Headgate #2 unit in the number 2 entry. He has continued to work until July 11th, after seeing his doctor, he was put on restrictive duty, and has been off since that time.

Employee was pushing up a roofbolt when wrench bent breaking the bolt, the plate flew back striking the employee above the right eye, causing a laceration that needed 3 stitches. Employee was wearing eye protection. He received the stitches and returned to work.

The mining machine was loading material into the shuttle car and a piece of rock slid across the load and was headed towards the employee in the cab of the shuttle car, he saw it coming and held his hand up to stop the rock, the rock cut his right hand, in the palm area at the base of his small (pinky finger). He went to the hospital and received 5 stitches and returned to work.

On Jan. 28th,2010 at 1:45 a.m. EE got his right foot under the cat track of the beam setter as it was tramming forward. He was transported to the hospital for examination and x-rayed, nothing was broken, he was told it was only bruised and was sent home to keep it elevated. He started missing work on 01/29/2010.
