Cutting outboard bearing on a 2 ton fan on south side of building. Bearing would not slide off and had to cut off with grinder. Outside cuts were made. When working on last inside section, the grinder blade shredded causing pieces of blade to hit top of the employee's left hand. PPE worn: hearing protection, face shield, mechanic gloves, safety shoes, and safety glasses.

Employee was drilling holes and a small piece of metal flew out and got by employee's safety glasses and into his eye.

Air would not bleed off air compressor line (though both gages read 0 psi) so employees turned attachment and it blew air out and hit employee in the face. Employee sustained ear pressure/discomfort.
