Employee LOTO his machine saw a belt not turning reached in pulled on the belt, the belt started in the wrong direction and pinched his finger causing a laceration and torn ligament. Employee was taken to ER and received seven stitches and was allowed to return to work with no lost time.

Bagging Hydrated lime bag blew off spout after bag clamp did not engage spraying employee with lime and resulting of some lime into eyes. Employee had on all PPE. Eyes were flushed out and employee was taken to Berkshire Occupational Health.
