A miner performing water blasting activities in the preheater tower riser area when handling the water lance and when the water supply valve was opened got a kickback force causing the gun section of the lance to strike on EE's right ear, knocking EE to the floor. EE was transported to the hospital, where EE received eleven stiches in the ear and was treated for a concussion.

EE was assisting with a drag rope endo when the eye on the tugger cable snapped. The employee was contacted by the drag rope, resulting in two lacerations that required stitches.

The miner was assigned the task of changing the drive pulley on Coal Mill Line 1 main drive ID fan when around 11:45am EE's left index finger was caught between the sheave and drive belt. Medical attention was provided. No fractures and 22 stitches were given to EE's finger. Miner returned back to work on same day with no restrictions.

EE was attempting to clean spilled material with a dingo skid steer with one hand when EE's left hand was cut by a metal plate used to hold a motion sensor. Medical attention was sought and the EE received stitches and placed on restricted duty of no use of the left hand.

The EE was working on a rotary feeder in the CKD area when EE's hand got caught in the rotary feeder. The EE's hand was injured and EE was taken to the hospital for further treatment.

EE was in the process of assisting the adjustment of the travel chain. A hydraulic ram was used to tension the chain. During the process, the spacer was ejected, striking the employee and causing a laceration requiring stitches.

While finishing up the repair of a handrail by grinding the weld smooth the grinder kicked back and impacted the welding mask the employee was using causing the mask to flip up and the grinder to strike the employee on the left cheek

While changing the belts and motor on fan 611-FN007, employee made contact with the belt while it was in motion, causing it to run EE's hand through the fan sheave and received 8 stitches in EE's right index finger.

The employee was water blasting the Preheater tower at the microwave level, when material falling in the lower riser hit the lance causing it to rollback hitting employee's chest area. Employee was using all pertaining PPE and after medical evaluation and CT scan results was diagnosed with contusion in the front wall of thorax.

While disconnecting the D-ring from the cable tractor, the D-ring rolled forward onto the EE's foot. A bone in the EE's foot was fractured.

The employee was using a magnetic drill to drill holes in metal plates. When EE attempted to clean the metal shaving, EE's gloved hand became entangle in the drill bit causing a traumatic left thumb injury.

The employee was water blasting the North east side of the pre-heater tower when the material fell on the employee. The blow lifted the outer layer and landed on the inner penetrating it and landing on employee's neck. Employee was wearing proper PPE for the task.

The employee was on the pre-heater tower water blasting the microwave area when EE's coworker accidentally triggered the pressurized water lance onto EE's right foot. The pressure make the water to penetrate the boot causing 1st and 2nd degree burn on EE's right ankle/heel area.

A maintenance employee supporting production to ball Finish Mill #6 was the flagman situated at the top of the mill giving instructions to the crane operator to place the grinding media drums in the mill door. EE placed hand between the drum and the device used to carry them, the drum shifted and smashed left middle finger.

While tightening bolts on a gearbox housing ee was positioning the hydraulic wrench on bolt 8 out of 16. While placing in position the hydraulic unit moved slightly catching ee left thumb between the hydraulic wrench and the gearbox housing causing a cut that required 4 stitches.

While water blasting in riser located at preheater tower, material fell down on tool jerking it upward. His hand was pushed upward by the handle of the water blaster tool into an air cannon nearby that smashed his finger.

While operating a water blaster on pre-heat tower, the lance kicked backwards and struck him in mouth. Apparently, material inside chute fell on end of lance causing it to kick.

The excavator walked in front of the truck, then reached over and put the bucket in the bed of the truck and began to walk backwards, pulling the truck out. When the truck was freed, the truck came forward, striking the windshield of the excavator. The truck struck the walking levers of the excavator which caused it to walk forward. Excavator then pushed truck til it went off.
