Employee was adjusting the drag cable on the drum when EE's finger became pinched between the wire rope and the drum. This resulted in a fracture with stitches to the left index finger.

Employee was walking on ground surface by conveyor when EE slipped and fell twisting left ankle.

While completing a bucket exchange, EE was inserting a pin into the hitch extension. A finger became pinched between the head of the pin and the hitch extension. This resulted in a nondisplaced fracture of the distal phalanx of the right ring finger.

Employee reported the bed of EE's 777 truck slammed while retracting after dumping the load at the main crusher. EE did not feel any pain until April 28th, 2023. The initial report of injury did not require any restrictions, however, on EE's second visit, after an MRI EE received restrictions on 05/08/2023

Employee claims they had some issues with their back about three weeks ago but reported it today.

A flat bar fell onto their left-middle finger

While finishing up the repair of a handrail by grinding the weld smooth the grinder kicked back and impacted the welding mask the employee was using causing the mask to flip up and the grinder to strike the employee on the left cheek

While troubleshooting A/C system on a locomotive, EE left hand index finger came into contact with a cooling fan. Resulting in cuts that required stitches.

The employee was using a magnetic drill to drill holes in metal plates. When EE attempted to clean the metal shaving, EE's gloved hand became entangle in the drill bit causing a traumatic left thumb injury.

EE using a 992K Loader in the pit area that has a joystick as the controls when EE felt pain in both hands. Medical attention was sought and EE was diagnosed with carpal tunnel syndrome.

The employee was attempting to throw a switch on the rail yard when EE lost EE's balance and sprain EE's left ankle.

Employee was dismounting a loader when EE felt pain in the groin area. EE was taken to the hospital and diagnosed with Inguinal hernia but the doctor stated that it was undetermined if it was job related.

EE was attempting to turn a starter upright inside of a 988H Loader engine's bay when EE felt pain in left arm.

EE was loading used wire rope into a dumpster. Forklift become unstable and turned over. EE received a laceration to the head that required medical attention, stitches. EE returned to work on the next scheduled shift. MSHA site was down for service Feb. 15 thru Feb 19.

On 11-15-2018, employee reported pain in lower back while lifting and carrying crusher liners. On 11-16-2018 employee was medically evaluated and put on restricted duty.

EE was using a sledge hammer to drive a pin in. The sledge hammer was harden steel. The pin is soft blow material. As the pin seated the hammer made contact with the area around the pin. This resulted in a small piece of metal entering ee's lip. When the metal was removed it required 2 stitches.

Employee was exiting a diesel truck. As they stood and twisted to exit the employee felt something in their L thigh. Employee was sent for diagnosis. Dr.stated no treatment needed. EE reported pain persisted. Sent for follow ups. EE missed first day of work on July 5, 2017.

A loose rock bounced off the top deck of the screen and landed on the lower level striking employee on the back of the hard hat.

A foreign object fell into an employee's eye. Initially employee reported this as a First Aid.

Employee was setting a panel on a sand tank when it slid off and pinched his right middle finger.

Employee was closing the doors on a rail car. Lever used sprung back and pinched employee's left hand middle finger,

Employee lifted a 30 Lbs. sieve sampler at the QC lab. He felt pain in his lower back.

During the PM day on unit 105, (Marion 7820 Dragline) an electrician was replacing the light fixtures on the boom when the fixture slipped and caused a small cut on his right hand on the middle finger at the knuckle. After providing first aid the employee was taken to see the Doctor who placed two stitches on the wound.

Employee was cutting a piece of rubber, wearing gloves, when he received a small laceration on his left thumb.

Employee, was using his lanyard when he slipped off. He then tried to grab to the lanyard to recover himself from falling. While pulling the lanyard he felt a snap on his right biceps.

EE was placing safety cone behind truck. During this task he felt a sharp pain on his back. He reported the incident and an entry was made on the First Aid Register. He came back the next day Fri. and the next day Sat. On Monday he complained his back was bothering him. He was taken to the Dr. who put him on restricted duty.

A mechanic from the Mobile Shop received a laceration in his left hand. He was changing a seal on a PTO. The tool that he used slipped off his hand. He received the cut on the palm of his hand close to his thumb.

Employee was walking on the north lateral side while looking up to the 62 shaker. Hit his head with a X cross member. He was walking in a forward motion.

Employee pushed down his left foot onto the rotor at the primary crusher while twisting his upper torso. Employee reported incident on 03/22/2013

Strained chest muscle shoveling material.

Conveyor rolled back catching employees leg between back of chute and conveyor.

Smashed middle finger with pipe.

slipped off of step of dozer and twisted ankle.

While working to install a piece of rail, EE struck a rail clip with a hammer, which glanced off and hit his left index finger.

A rock fell from loader bucket and hit his legs.

Employee was dismounting from the left shoe of the dragline, while the machine was stopped, by using the 4-step ladder of this side of the dragline. He lost his grip from the last step (approx. 3 feet high), falling backward and landing on his right elbow.

Tightening head on kelly bar on drill with wrench, wrench slipped and employee moved akwardly and felt pain in lower back.

Employee was closing a water valve on a water pump. While closing the valve the flat bar slipped causing him to loose balance, fall forward onto the valve sticking him in the ribs. He reported it on First Aid and informed his supervisor. He continued to work until 1/28/2008 when he informed his supervisor that the pain had continued and we seeked medical attention.

Employee glared at welding arc and eyes began to burn.

EE & other were replacing impact idlers. They were in process of installing fourth idler. When they both simultaneously lifted the idler it slipped and fell. He then attempted to prevent the idler from falling & had his finger pinched in between the frame of the conveyor & the frame of the falling idler. He was wearing gloves.

Employee was walking along the peak of the excavated stockpile in order to measure heights and calculate volume of the pile for production. Employee lost his balance when material gave way underneath him while climbing. Employee rolled down the entire face of the stockpile approximately 90 feet before coming to rest.

Tripped over welding curtain and lost footing and twisted knee/ankle.

EE was assigned to replace a bearing on the lower bend pulley (BP) of Conveyor 22B. Crouching on the catwalk with his back to the midrail he attempted to remove the bearing cover by pulling on it, at which time it slipped from his hands causing him to fall backwards against the midrail. The midrail broke and he fell aproximately 12 ft landing on his left shoulder.

While replacing screens, one fell and landed on his foot.

EE was walking with a 5 lb. GPS rover pole in his hand. He looked at the GPS to give directions. He was walking on fallen trees. The bark was smooth, wet and slippery. His foot slipped off and rested on soft and uneven ground 18"" to 24"" below, causing him to sustain a sprain of his left ankle. This Report was filed on 11/20/06, I called MSHA to inform internet problem.

Employee's left index finger was smashed between jib-boom and platform on drill rig.

EE was moving between two screens, (limited access) and steped on a rock approximately 4"" in diameter and turned his ankle.

EE WAS REPLACING THE HIGH PRESSURE FITTING ON THE AC COMPRESSOR ON A CAT 16H GRADER. ON HIS WAY DOWN THE LADDER HE MISJUDGED HIS LAST FOOT PLACEMENT FALLING BACKWARDS HITTING THE BACK OF HIS HEAD ON THE CONCRETE FLOOR. THIS RESULTED IN A LACERATION ON THE BACK OF HIS HEAD. HE WAS TREATED AND RELEASED FROM THE PALMETTO GENERAL HOSPITAL.

He was attempting to dislodge a rock in the winged tail pulley with a long piece of flat bar. He pulled down on the bar, the end of the bar rolled out of the tail area, the flat bar smashed his left index (pointing) finger resulting in a broken bone, 5 stitches. He was wearing gloves on his hands at the time.

Employee was greasing rollers on dragline with an air accuated grease gun. Hose developed pin hole & grease filled his glove and (L) thumb because of extreme pressure. Employee had surgery to remove grease from around bone. He is off work presently.

He walked down the first set of stairs from 992 G cab level to the ballast block level without any incident. He faced the engine compartment, held on to both handrails and using his right foot, took one step down. The right side support bracket and bolts were broken. As the step dropped, employee knocked his right shin against the walkway edge.

Employee was climbing down from the fines recovery dragline and when he stepped onto the ground from the ladder he twisted his right knee.

While installing a ladder on the 4600 dragline, used in the plant for cleaning sand, it slipped from his right hand, catching the tip of the left pinky finger, causing 2 cuts to either side of his finger. (He was a First Aid on the 16th of Feb. He was taken to Workers Compensation on the 21st of Feb., at his request, where he received treatment.)

Employee was walking by the plant shop towards his truck. He tripped and fell on his back against a steel sand tank screw that was placed in that area for fabrication.

Employee slipped and hit his face on a rock while trying to move a cable for the dragline after the hurricane.

Employee was changing a bearing on the stedman with a portapower jack and while pushing off the bearing the ram popped out and hit him in the jaw area. NOTE: (Employee did not seek any treatment at the time due to mild pain and did not seek medical treatment until 8-29-2005: and was then taken to workers Compensation for treatment)

Employee was working near the primary feeder, when he slipped and felt a pain in his back.

The employee was taking a survey point on the excavated material Pile on Lake 6 north.As he started to descend the pile, getting towards the bottom, a rock 10"" to 12"" in diameter rolled down the pile and struck him in the back of the legs, causing a bruise and an abrasion on the calves of his legs.

Moving a cable tree when the chain used to move the tree slipped and pinched the employee's finger.

Employee was washing out the primary pan feeder in a kneeling position. When he finished the task, he raised up; hitting the top of his left shoulder, striking an I-beam.

Employee was walking up the stairs to his work site when he stepped on a rock and slipped down onto his knee causing a strain requiring physical therapy and medication as treatment.

Employee was pulling power cable with a cable puller for the 7820 dragline, when the puller slipped off the cable, causing him to fall backwards, straining his shoulder.

Employee was working on the anvil ring on the VSI crusher, when as the ring was being lowered, his left hand was caught between the ring and the inside edge of the crusher, causing lacerations to his hand.

Employee was walking towards his truck in the plant, when he stepped on a rock and twisted his ankle.

Employee was changing friction brake blocks on the 480W electric Dragline and got some type of dust in his eyes.

Employee was working in the dragline. He was climbing down the shoe ladder and lost his footing falling 4-5 feet to the ground. Landed on his feet, but felt a strain to his lower back. Working restricted duty and receiving medication and physical therapy.

Employee was changing a top deck screen on a shaker and while adjusting the position of the screen, he felt pain in his lower back.

Employee was removing plastic zip ties from computer cables and in doing so cut left wrist with knife.

Employee was building a cement block wall. After a period of time, he noticed pain in his left wrist as he continued to place the cement blocks.* Became reportable 11/17/04 after follow-up visit to Dr. & EE was given medical treatment*

Employee was changing out a box liner when the liner slipped and hit his (L) leg agove his ankle requiring stitches.

Got dirt in eye while working inside Stedman crusher.

Employee tripped over hose and strained his shoulder upon falling.

Employee was marking parts on a pallet in the warehouse, when he rose up to walk away and felt a sharp pain in his lower back.

Employee slipped and fell hitting his leg inside 480W Electric Dragline while changing hoist cable. (Note: Employee did not seek medical attention until 7/19/2004)

EMPLOYEE WAS UNLOADING A 20 LB BOX FROM A DELIVERY TRUCK AND WAS SETTING IT DOWN ON THE FLOOR OF THE WAREHOUSE WHEN HE FELT A SHARP PAIN IN THE TOP OF HIS RIGHT FOOT.

The employee strained his back while attempting to move an aggregate sample bucket stand closer to his vehicle. (Important Note) Employee sought treatment for injury on July 21st ,2004.

EMPLOYEE WAS PASSING METAL PLATE THROUGH SHAKER WHEN A SHARP EDGE CUT THROUGH HIS GLOVE CAUSING A LACERATION TO THE RIGHT PALM WHICH REQUIRED STITCHES.

EE WAS GRINDING ON A SIDE WALL HOLDING GRINDER OVER HIS HEAD IN FRONT OF HIM. HE HIT THE FRAME OF UPPER DECK CAUSING GRINDER TO JUMP, TWISTING HIS ARMS CAUSING GRINDER TO STRIKE HIS WRIST, LAC ERATING IT REQUIRING STITCHES.

EE WAS MOVING HAMMERS IN THE STEDMAN WITH A PRY BAR WHEN THE HAMMER JUMPED OVER THE RAIL. EE GRABBED THE HAMMER TO KEEP IT FROM FALLING OFF THE TABLE AND STRAINED HIS BACK.

EMPLOYEE WAS CLIMBING THE LADDER TO ACCESS THE CAB OF THE 785 HAUL TRUCK WHEN HIS RIGHT HAND SLIPPED OFF THE HAND RAIL CAUSING A BACKWARDS FALL TO THE GROUND.

WHILE CLEANING OUT ACCESS HOLE, PARTICLES FROM THE ENGINE HOSE WENT INTO THE EMPLOYEES LEFT EYE.

WHILE REMOVING A CYLINDER ON A P&H DRAG LINE A BOLT FLEW OUT AND HIT RIGHT CHEEK OF EE CAUSING LACERATION.

EMPLOYEE SLIPPED AND FELL BY THE SWING MOTOR ON THE 480 DRAGLINE, FRACTURING HIS RIGHT FOOT.

WHILE OPENING A VALVE UNDER MCC2, SLIGHT INCLING ONT HE WAY OF THE VALVE, COVERED WITH AN EE STEPPED ONT HE MUD SLIPPED AND FELL ON HIS BUTTOCK.

WHILE CLIMBING A BULLDOZER, SLIPPED AND CAUGHT HIMSELF ON RAILING INJURING RIGHT ARM.

EE DROPPED A PIPE WRENCH & IT FELL INTO A WATER CYCLONE PUMP WATER TANK, WHILE RETRIEVING THE WRENCH HE CUT HIS LEFT ARM WITH THE LIP OF THE DISCHARGE FLUME.

EMPLOYEE SPRAINED HIS LEFT WRIST WHEN HE SLIPPED AND FELL.

WHILE HAMMERING METAL WITH A SLEDGE HAMMER A PIECE OF METAL FLEW OFF AND STRUCK HIS RIGHT LOWER LEG CAUSING A PUNCTURE WOUND.

EMPLOYEE STRAINED HIS SHOULDER WHILE TRYING TO MOVE A FALLEN OVERHEAD HOIST FROM A FELLOW EMPLOYEE WHO WAS FATALLY INJURED.

EMPLOYEE WASHED THE ROLL CRUSHER AND REMOVED THE ROLL COVERS. AS HE STEPPED DOWN HE SLIPPED AND FELL BACKWARDS, STRIKING A J-BAR WITH HIS BACK.

WHILE DRIVING HAUL TRUCK HE HIT A POT HOLE AND HURT LOWER BACK.

WHILE A PASSENGER IN VEHICLE, DRIVER TRIED TO AVOID MATERIALS NEXT TO TRACK & HIT A STANDING TRAIN. HE FELT PAIN IN UPPER LEFT LEG & KNEE.

EE WAS UNLOADING EQUIP FROM FLATBED TRK. HE USEDTHE HANDLE GRIP ON THE TRUCK TO STEADY HIMSELF &TO PULL ON AS HE CLIMBED ONTO THE TRUCK BED. HE STATED THAT HE DID NOT SLIP OR FALL WHILE CLIMBI NG BUT MAY HAVE PULLED TOO HARD OR OUT OF BALANCE

WHILE TRANSFERRING CASE OF D45 FROM ONE TRUCK TO ANOTHER I DROPPED A CASE AND SMASHED MY HAND AGAINST THE TAILGATE PINNING MY HAND UNDERNEATH.

EE CUT LEFT INDEX FINGER WITH A CABLE WINCH.

SCREEN BECAME UNHOOKED FROM THE LATCH WHILE EMPLOYEE WAS LIFTING IT AND HIT HIM ON THE HAIRLINE.

WHILE CLOSING RAILCAR DOOR WITH A LINING BAR EE FELT PAIN ON HIS BACK.

WHILE GETTING OUT OF HIS TRUCK ON UNEVEN PAVEMENT, EMPLOYEE FELT PAIN ON LEFT KNEE.

WHILE CUTTING OFF BOLTS USING OXYGEN ACETYLENE EQUIPMENT THE HOSE BECAME UNCOUPLED AT THE TORCHEND CAUSING AN EXPLOSION THAT BURNED EMPLOYEE'S FACE ON THE LEFT SIDE.

EE WAS STARTING HIS SHIFT AS A TRUCK DRIVER WHILE CLIMBING THE LADDER HE FAINTED & FELL TO THE GROUND.

EE DROPPED A PIECE OF STEEL ON HER LEFT MIDDLE FINGER.

GOT OUT OF THE TRUCK PUT FOOT DOWN. THEN EVERYTHING SHIFTED INCLUDING HIS ANKLE

EMPLOYEE WAS WELDING WHEN A PIECE OF SLAG FELL INTO HIS BOOT AND BURNED HIS RIGHT FOOT.

EMPLOYEE SUSTAINED A LOWER BACK PAIN WHILE MOVING ELECTRICAL POWER CABLE.

EMPLOYEE WAS CUTTING WITH AN OXYGEN ACETYLENE TORCH WHEN HIS POANT LEG CAUGHT FIRE AND BURNED HIS LEFT LEG.

EMPLOYEE WAS REACHING TO TURN OFF AC UNIT IN CAT LOADER WHEN HE SLIPPED AND HIS RIGHT INDEX FINGER WAS LACERATED BY AC FAN BLADE.

EMPLOYEE WAS USING HIGH PRESSURE HOSE TO CLEAN PLANT AND HOSE RUPTURED, CAUSING EMPLOYEE TO SLIP AND FALL LACERATING HIS RIGHT INDEX FINGER REQUIRING 8 STITCHES.

EMPLOYEE WAS RESOCKETING DRAG CABLE WHEN CABLE SLIPPED AND SPRUNG BACK STRIKING HIS LEFT ARM.

EMPLOYEE WAS REMOVING SOME OLD LOCKERS FROM EMPLOYEE LOCKER ROOM AND HIT HIS RIGHT INDEX FINGER BECAME CLOSED IN ONE OF THE LOCKERS SEVERING THE TIP OF HIS FINGER.

EE DEVELOPED BACK PAIN WHILE OPERATING BULLDOZER.

EE WAS OPERATING HIGH PRESSURE WATER HOSE CLEANING PLANT AREA & TWISTED HIS RIGHT KNEE
