DRIVER OF BULLDOZER WAS WORKING AT EDGE OF LAKE ""PADDING"" THE SLIP FACE FOR THE DRAGLINE USED BY RINKER, WHEN BULLDOZER FELL INTO LAKE RESULTING IN DEATH TO DRIVER.

WHILE ASSISTING IN THE INSTALLATION AND WIRING OF AN ELECTRONIC GENERATOR, INSIDE OF A 7820 MARION DRAGLINE, A MECHANIC WAS POSITIONING AN OVERHEAD 10 TON HOIST WEIGHING ABOUT 1160 POUNDS TO LIFT THE GENERATOR ARMATURE IN ORDER TO ALIGN THE BOLT HOLES, WHEN THE HOIST FELL ON THE EMPLOYEE CAUSING FATAL BLUNT TRAUMA.

Employee was adjusting the drag cable on the drum when EE's finger became pinched between the wire rope and the drum. This resulted in a fracture with stitches to the left index finger.

While completing a bucket exchange, EE was inserting a pin into the hitch extension. A finger became pinched between the head of the pin and the hitch extension. This resulted in a nondisplaced fracture of the distal phalanx of the right ring finger.

Employee reported the bed of EE's 777 truck slammed while retracting after dumping the load at the main crusher. EE did not feel any pain until April 28th, 2023. The initial report of injury did not require any restrictions, however, on EE's second visit, after an MRI EE received restrictions on 05/08/2023

While troubleshooting A/C system on a locomotive, EE left hand index finger came into contact with a cooling fan. Resulting in cuts that required stitches.

The employee was using a magnetic drill to drill holes in metal plates. When EE attempted to clean the metal shaving, EE's gloved hand became entangle in the drill bit causing a traumatic left thumb injury.

EE was loading used wire rope into a dumpster. Forklift become unstable and turned over. EE received a laceration to the head that required medical attention, stitches. EE returned to work on the next scheduled shift. MSHA site was down for service Feb. 15 thru Feb 19.

On 11-15-2018, employee reported pain in lower back while lifting and carrying crusher liners. On 11-16-2018 employee was medically evaluated and put on restricted duty.

EE was using a sledge hammer to drive a pin in. The sledge hammer was harden steel. The pin is soft blow material. As the pin seated the hammer made contact with the area around the pin. This resulted in a small piece of metal entering ee's lip. When the metal was removed it required 2 stitches.

Employee was exiting a diesel truck. As they stood and twisted to exit the employee felt something in their L thigh. Employee was sent for diagnosis. Dr.stated no treatment needed. EE reported pain persisted. Sent for follow ups. EE missed first day of work on July 5, 2017.

During the PM day on unit 105, (Marion 7820 Dragline) an electrician was replacing the light fixtures on the boom when the fixture slipped and caused a small cut on his right hand on the middle finger at the knuckle. After providing first aid the employee was taken to see the Doctor who placed two stitches on the wound.

EE was placing safety cone behind truck. During this task he felt a sharp pain on his back. He reported the incident and an entry was made on the First Aid Register. He came back the next day Fri. and the next day Sat. On Monday he complained his back was bothering him. He was taken to the Dr. who put him on restricted duty.

A mechanic from the Mobile Shop received a laceration in his left hand. He was changing a seal on a PTO. The tool that he used slipped off his hand. He received the cut on the palm of his hand close to his thumb.

Employee was closing a water valve on a water pump. While closing the valve the flat bar slipped causing him to loose balance, fall forward onto the valve sticking him in the ribs. He reported it on First Aid and informed his supervisor. He continued to work until 1/28/2008 when he informed his supervisor that the pain had continued and we seeked medical attention.

EE & other were replacing impact idlers. They were in process of installing fourth idler. When they both simultaneously lifted the idler it slipped and fell. He then attempted to prevent the idler from falling & had his finger pinched in between the frame of the conveyor & the frame of the falling idler. He was wearing gloves.

Employee was walking along the peak of the excavated stockpile in order to measure heights and calculate volume of the pile for production. Employee lost his balance when material gave way underneath him while climbing. Employee rolled down the entire face of the stockpile approximately 90 feet before coming to rest.

EE was assigned to replace a bearing on the lower bend pulley (BP) of Conveyor 22B. Crouching on the catwalk with his back to the midrail he attempted to remove the bearing cover by pulling on it, at which time it slipped from his hands causing him to fall backwards against the midrail. The midrail broke and he fell aproximately 12 ft landing on his left shoulder.

EE was walking with a 5 lb. GPS rover pole in his hand. He looked at the GPS to give directions. He was walking on fallen trees. The bark was smooth, wet and slippery. His foot slipped off and rested on soft and uneven ground 18"" to 24"" below, causing him to sustain a sprain of his left ankle. This Report was filed on 11/20/06, I called MSHA to inform internet problem.

EE WAS REPLACING THE HIGH PRESSURE FITTING ON THE AC COMPRESSOR ON A CAT 16H GRADER. ON HIS WAY DOWN THE LADDER HE MISJUDGED HIS LAST FOOT PLACEMENT FALLING BACKWARDS HITTING THE BACK OF HIS HEAD ON THE CONCRETE FLOOR. THIS RESULTED IN A LACERATION ON THE BACK OF HIS HEAD. HE WAS TREATED AND RELEASED FROM THE PALMETTO GENERAL HOSPITAL.

He was attempting to dislodge a rock in the winged tail pulley with a long piece of flat bar. He pulled down on the bar, the end of the bar rolled out of the tail area, the flat bar smashed his left index (pointing) finger resulting in a broken bone, 5 stitches. He was wearing gloves on his hands at the time.

Employee was greasing rollers on dragline with an air accuated grease gun. Hose developed pin hole & grease filled his glove and (L) thumb because of extreme pressure. Employee had surgery to remove grease from around bone. He is off work presently.

He walked down the first set of stairs from 992 G cab level to the ballast block level without any incident. He faced the engine compartment, held on to both handrails and using his right foot, took one step down. The right side support bracket and bolts were broken. As the step dropped, employee knocked his right shin against the walkway edge.

Employee was climbing down from the fines recovery dragline and when he stepped onto the ground from the ladder he twisted his right knee.

While installing a ladder on the 4600 dragline, used in the plant for cleaning sand, it slipped from his right hand, catching the tip of the left pinky finger, causing 2 cuts to either side of his finger. (He was a First Aid on the 16th of Feb. He was taken to Workers Compensation on the 21st of Feb., at his request, where he received treatment.)

The employee was taking a survey point on the excavated material Pile on Lake 6 north.As he started to descend the pile, getting towards the bottom, a rock 10"" to 12"" in diameter rolled down the pile and struck him in the back of the legs, causing a bruise and an abrasion on the calves of his legs.
