On December 16, 2016, Miner was attempting to split a 2' seam off a rock that was lying flat on the forks of the loader. As the 2' rock spilt, the remaining rock on the bottom side had an unknown seam that broke in half causing the rock to fall off the forks and roll onto ee right ankle. EE was taken to the hospital and was diagnosed with a fracture to right ankle.

Showing all 3EE was operating a Stonemason 70 rock chopper. While spinning the rock to go into the machine the EE got Left small finger caught between the rock and the frame of the machine. The resulting injury was a laceration to the small left finger which required sutures.
Finger under a thin rock as operator lowered top jaw to cut the rock did not move finger from under rock causing the middle finger on the end to be smashed. Resulting in lacerated finger requiring bandages and antibiotic cream and checks for infection.
