Operator of dozer was starting a new slot and as the material fall away ahead of the blade, the dozer operator quickly reversed and tracked over the previous slot embankment edge. The dozer laid over onto its left side, operator was wearing their seatbelt and was able to self-extricate. Operator was then sent for evaluation at an off-site clinic where they were issued restrictions.

Showing all 21Employee was using a 6" grinder with a cut off wheel cutting flat plate on a set of stands when the grinder grabbed and kicked back toward the individual making contact with EE's left mid to upper thigh resulting in a laceration and 11 stitches.
Employee was operating a haul truck and was being loaded by the production loader and felt some discomfort in their back, the employee was reassigned to a different loading unit and finished the shift but after leaving the following morning, called their Supervisor and reported that they needed to visit the ER. The restrictions would not allow return to work for the next shift.
Employee was backing a dozer on the main dump and contacted a rock with the back of the dozer, this caused a sudden stop which jarred EE and caused neck pain. First day of work missed 02/19/2025.
Employee was cutting a bolt off with a DeWalt portable bandsaw. When the saw cut through the bolt, it dropped down coming into contact with the left thumb causing a laceration.
While installing Feed pipe to the #2 skid at TSF4 an excavator was being used to position the pipe in place. With the stored energy left in the pipe it swung back approximately 10 feet striking the employee in the lower right leg.
Employee was loosening a bolt with an impact gun and had hand on the socket while activating the impact when a sliver of steel caught EE's finger causing a laceration.

EE was using a Hytorc wrench to tighten mill shell bolts. EE's left ring finger tip was caught between the mill shell and the wrench.
While cutting angle iron with a 4" grinder, employee lacerated right index finger and required eight stitches.
Employee was welding a bracket on a drill in the pit and felt pain in right shoulder.
Employee was putting rigging into mantle while lining up eye bolt, crane hook drifted down and smashed left thumb, nail was removed and 3 stitches applied.

An employee was starting to cut out a bearing on a pump barrel when the grease flashed in the bearing and a fire ball of grease shot out of the bearing and deflected off the face shield of the employee cutting the bearing and struck another employee standing nearby in the facial area with hot grease.
Employee was torqueing bolts on a 789 front suspension. During the torqueing process the tooling slipped, this caused the reaction arm of the gun to come around and contact one of his fingers. He sustained a cut of the fingertip and fractured the tip of the bone.
Employee strained left shoulder when nut came loose and impact fell back off socket. The doc checked him out and gave him a full duty release. It was still hurting him so we took him to a shoulder specialist on 10/2/15 who gave him light duty
Employee was using a cordless hand drill in an awkward position and the drill got caught up on a piece of metal snagging the drill and quickly twisting his arm.
