Mine operator twisted ankle while exiting a transport van. Medical examination assigned work restrictions. Operator returned to work next shift but declined to perform restricted duties. Operator reports ongoing physical therapy for injury and has not returned to site in any capacity.

Employee was stepping over a pipe when EE struck EE's right knee on the flange connection and cut through EE's pants, cutting EE's knee.
Two employees were working together to clean up some using a forklift to maneuver drill steels. The employee on the ground had their hand on top of a drill steel when another shifted and moved towards the other steel catching their finger between the two drill steels and crushing it.
Operator was walking on uneven ground and felt pain in their knee. After evaluation employee was placed out of work by the medical provider beginning on 12/10/2025.
Mechanic was moving a loader cutting edge from a pallet onto the forks of a forklift and when dropping it onto the fork EE's finger was caught smashing EE's right index finger and causing a laceration which required sutures.
A Mobile Maintenance Mechanic was removing cutting edges off of a forklift, as they set it down to flip it over their finger got caught between the cutting edge and corner of forklift smashing their finger, employee received sutures to close wound.
A Haul Truck Operator was climbing from truck to the ground using the handrails and 3-step boarding ladder when they fell from the second step to the ground. They were transported to the hospital for evaluation and was placed out of work for a suspected concussion. Their first shift of lost time was 10/19/2025.
The victim was preparing the area to lower a vulcanizing hot press down to the lower level by removing grates on the 2nd floor of the number 2 ball mill feeder belt. In the process, the victim, who was not wearing fall protection, fell 10 feet to the lower level.
Employee was cutting flex using a utility knife. Employees knee slipped causing them to make contact with upper right knee, causing about a 1inch laceration.
A maintenance mechanic was inside the discharge chute of Ball Mill #5 and was using a 12-pound sledgehammer to break a chute liner loose when EE's footing slipped mid swing and the hammer hit EE's right knee causing a chip fracture.
A Lube Technician was dismounting a dozer in the pit after servicing it, when stepping down they felt pain in their ankle and felt a pop. EE was sent to a provider for evaluation on 09/11/2025 and was diagnosed with a sprain injury and placed on crutches, first day of lost time was 09/12/2025.
A mechanic was stepping onto a barge from the ground when their muddy rubber boots caused them to lose footing and slip and caught themselves by grabbing the handrail and hurting their right shoulder.
A production loader operator was coming off the boarding ladder of the loader when EE stepped on a rock and felt pain in EE's leg. On evaluation EE was diagnosed with a muscle strain and issued restricted duty, EE's first shift of restricted duty was 09/05/2025.
A haul truck operator noticed redness and swelling on EE's forearm assumed to be from an insect bite or sting, on evaluation EE was referred to medical evaluation for a possible allergic reaction. EE was was diagnosed with an acute, localized reaction and prescribed a steroid and light duty for the course of medication.
THE DRIVER WAS HAVING TROUBLE REMOVING THE HOSE AND THEY PULLED ON THE HOSE, THAT WAS WHEN THE HOSE WENT STRAIGHT TO THEIR BOTTOM LIP, IT CUT BOTTOM LIP, PUSHING THE BOTTOM TOOTH INSIDE, CAUSING BLEEDING AND PAIN.
While waiting for the shift to begin the employee sat in a chair next to a cork board mounted on a wall. The cork board fell and struck the employee on the left shoulder.
Employee was replacing a drive sprocket on a excavator. While attempting to install the sprocket fell off of the forklift and landed on ankle. Took employee to the emergency room and had x rays taken. Results were no broken bones or torn ligaments. Ankle is fine just some swelling and bruising.
While waiting for shift to start employee was rubbing eyes when they felt a sharp pain in their eye after touching welding jacket. Employee was seen at the ER where a foreign object was removed from their eye.
As The operator was dismounting the haul truck, right hand stuck to the handrail as their foot reached the ground. At that time they felt a sharp pain from the front of right shoulder. Issued 2 lb lift restriction, no driving, no operating equipment, no climbing ladders. Began restricted duty on 06/13/2025.
Operator of dozer was starting a new slot and as the material fall away ahead of the blade, the dozer operator quickly reversed and tracked over the previous slot embankment edge. The dozer laid over onto its left side, operator was wearing their seatbelt and was able to self-extricate. Operator was then sent for evaluation at an off-site clinic where they were issued restrictions.
Employee was using a 6"" grinder with a cut off wheel cutting flat plate on a set of stands when the grinder grabbed and kicked back toward the individual making contact with EE's left mid to upper thigh resulting in a laceration and 11 stitches.
Employee was walking from the Main Gate to the Administration building and there was a pickup next to the gate, they walked around the pickup and turned to talk to the driver. As they turned back to continue walking, they stepped into a dip in the pavement, twisted an ankle, and fell resulting in an ankle fracture.
Operator went to fuel island and the pump 2 hose had been rolled back up. In the process of pulling hose back to the east side of fuel island, operator went to get more hose out of reel and in doing so strained their back as they were turning their body. First missed shift was 04/25/2025.
Operator of drill 557 was leaving tie down attempting to go down ramp to Castle dome pit, during that walk Haul truck 531 was attempting to pass drill, When haul truck attempted to pass it struck rear of drill mast causing damage to both and injuring the drill operator. First missed shift was 05/30/2025.
Employee was attempting to retrieve tools from the back of employee's pickup and cut right hand on an exposed, sharp, piece of metal located at the top of the tailgate. Employee received a cut to the top of right hand above the thumb which required 5 sutures.
While removing the wear caps on a loader bucket, they struck the cap with a hammer which caused a metal fragment to fly off and embed in this left hand between the thumb and index fingers through a leather glove and the embedded metal had to be surgically removed. First missed shift occurred on 04/02/2025.
Employee was operating a haul truck and was being loaded by the production loader and felt some discomfort in their back, the employee was reassigned to a different loading unit and finished the shift but after leaving the following morning, called their Supervisor and reported that they needed to visit the ER. The restrictions would not allow return to work for the next shift.
Employee was backing a dozer on the main dump and contacted a rock with the back of the dozer, this caused a sudden stop which jarred EE and caused neck pain. First day of work missed 02/19/2025.
As the employee was dismounting the haul truck, EE stepped off the last rung of the ladder and stepped on an unseen small rock and rolled EE's knee inward. First day of missed work was 02/06/2025, following revised restrictions from a follow-up visit.
Employee was priming holes, as employee moved to push wire down the lath / stake a splinter came from the wood and entered employee's hand between the thumb and pointer finger, it then pushed back into the back of the hand.

Employee was cutting a bolt off with a DeWalt portable bandsaw. When the saw cut through the bolt, it dropped down coming into contact with the left thumb causing a laceration.
While installing Feed pipe to the #2 skid at TSF4 an excavator was being used to position the pipe in place. With the stored energy left in the pipe it swung back approximately 10 feet striking the employee in the lower right leg.
Employee was helping to adjust the gap on Crusher #5 using a hydraulic ram. When the ram began to retract, it slipped off of the crusher tooth and the employee grabbed it, crushing their Right ring and pinky finger between the ram body and head.
Maintenance employee was working to install a bearing on a conveyor pulley and cut the second knuckle on EE's right hand middle finger.
Maintenance employee was placing a training idler onto a conveyor cartridge that was being rebuilt. While placing the idler into the frame, it shifted catching employee's pinky finger on left hand between the idler and frame.
Employee was removing an inline valve on the tailing pump discharge line and when the last bolt was removed, the valve shifted releasing pressurized tailings slurry which got in employee's face/eyes.
EE was tightening bolts on a rebuilt pump when EE felt a pain in EE's back.
Employee was removing brush and weeds while standing on a pallet and slipped causing EE to fall to EE's knee. New restrictions issued on 10/14/2024, first missed shift is 10/16/2024.
Employee was loosening a bolt with an impact gun and had hand on the socket while activating the impact when a sliver of steel caught EE's finger causing a laceration.
Employee was boarding employee's haul truck and for an unknown reason, chose to utilize the emergency egress ladder rather than the normal boarding ladder/stairs. EE reached the third rung from the top when EE fell approximately 11 feet to the ground. EE received a head laceration, fractured right thumb, fractured collarbone, and fractured C6 lamina.
Employee was climbing Scaffolding felt a pop and sharp pain in the left knee. Employee was diagnosed at medical facility with strain/sprain to the knee. Was referred to physician for further evaluation.
Employee reported back strain after climbing the boarding ladder/stairs of employee's haul truck. Pain reported the following day on 07/05/2024. Employee issued restrictions of no driving, stairs, or uneven ground by medical provider.
Tire technician was removing a lock ring from a haul truck tire when the lock ring pinched EE's fingertip. *EE was diagnosed with a secondary infection due to improper wound care during a follow-up visit on 8/5/24 resulting in restricted duty work beginning 08/05/2024. Restrictions listed as wear splint, keep hand clean/dry, no lifting/push/pull over 10 pounds.
Employee was walking down a conveyor walkway and slipped on debris.
While operator was going to move plastic pallet from table to set up for next day starter sheets as operator was moving the pallet it slid of table causing employee to lose balance falling back and hitting lower left triceps, causing a laceration requiring sutures, and a bruise and swelling on elbow area.
Employee was harvesting cathodes in the tank house and had a dropped sheet inside of the cell. While trying to fish the sheet out with a handled hook, EE pulled to lift the sheet and it snagged on the side and dropped off the hook and felt a pull in EE's back and radiating down left leg. Employee was given restrictions of 5 lb lifting, no stairs, no squatting.
Employee was operating a road grader and noticed the transmission was not working correctly. Operator was inspecting for leaks, when they opened the engine compartment door the transmission fluid cap popped off causing them to be splashed with oil which got in their eyes.
Employee was performing semi annual service an HT518, was headed to back of the motor to cut zip ties on the detection line after stepped on tubing, foot slipped and worker's leg caught a bracket which cut calf on right leg.

Plate slipped off the rope falling onto employee's hand causing a laceration
EE was pouring some chemicals into a tube and a small amount got onto EE's arm creating a small burn.
Driver was placing lid on copper concentrate container and needed to slide it over. While using bar, lid shifted causing wrist to twist. Tweaked right wrist and felt a pop.
Installation of new silo components (feeder tube), when a bolt broke and the tube fell approximately 5 ft onto employees right foot (above steel toe). 2 fractures diagnosed, employee given a walking boot, crutches, restrictions of no weight bearing on right leg.
Employee was standing on loose gravel near the facility, installing signage. Employee lost footing while using prybar, causing employee to fall to the gravel and striking left knee. Employee was evaluated by a care professional and required sutures and follow-up visit. No other care required. No lost time required. Returned to full duty on next scheduled workday.
The condition contributing was a highwall failure. Employee was extracted out of right window of the piece of equipment. Employee had used their right arm and shoulder trying to break the window causing an injury.
Person was bracing against pipe with right knee, when a bobcat hit the pipe into EE's knee.
Injured Right knee and did not report until 7/7/2023 and then EE was out on EE's 7 days off and will become a lost time on 7-14-23.
Employee was using a metal shear and when activated the plate popped up catching employee's left thumb between the plate and the body of the machine.

helping with a cyclone feed pump and smashed EE's right ring finger while removing belts from the motor side
Installing 16"" casing and employee's finger was caught between mainline and bolt on elevator dog ear latch.
Operator was walking from the piece of equipment to the restroom when EE tripped over EE's feet causing EE to fall to the ground. Employee is being treated for a shoulder fracture. Employee was initially injured on June 24th however the fracture was not found until the follow up appt. on July 21st making it reportable. Employee has not lost any days.
Person was injured June 9th and worked until results showed a fracture. Is now out as of 7-7-22. Fracture was not found until 7-7-22 and has become a lost time. Employee had been released for work as part of normal job duties prior to the diagnosis.
Employee tripped while making rounds fell and cut left hand.

Jostled while being loaded causing lower back pain
Involved in a single light vehicle accident on the haul road, vehicle came to a sudden stop. Person was sitting in the back seat of the vehicle
2 Employees were cutting 1"" thick steel plate that was placed on stand making sure to pay extra attention to orientation of plate so that it did not fall after being cut. While placing a piece of angle iron on the plate the stand tipped over and the steel plate landed on EE's foot causing a laceration. The injury required 4 stitches.
Driver stared shift at 10:30 pm on 04/07/2021. While placing a seal on a container the driver was stepping from catwalk onto the container and rolled EE's ankle. EE required assistance at this point.
Employee states cleaning muck off the table and adjusting easier skirt with a hammer while belt was running. Hammer slipped and went into roller and took EE's hand in. Damages broken bones in right hand and swelling of the arm which lead to skin graphing.
Fell into open hole in grating that required stitches.

EE was using a Hytorc wrench to tighten mill shell bolts. EE's left ring finger tip was caught between the mill shell and the wrench.
Employee was using a metal bar to move plates to set a truck frame on stands. The bar came out of the employees hands and landed on the top of employees foot.
EE was walking in some mud while carrying weight and EE's ankle gave out/twisted.
The employee injured their wrist when they fell from the truck during dismount.

EE was affected by a spill from a valve that was bumped and opened on a level up above.
Employee was assisting moving a lifter toward the center of the mill to fit in the required location. Another employee was hitting the lifter with a sledgehammer to push a bolt out of the bolt holes and the first employee's thumb got in the way causing it to be hit with the sledgehammer.
Stumbled and fell on hands required stitches.

An employee was pulling the last set of cathodes in a cell at the SXEW and tried to grab a copper bar while holding the strongback. The employee felt a shock go through their body to their hand. Seen by site First Responders and flown to a hospital in Phoenix for further evaluation.
On 12/11/2018 we received notice from our workers compensation insurance company that this employee has been diagnosed with biopsy proven anthrasilicosis secondary to dust inhalation. The date of illness is 2-11-2014.
An employee was travelling in the outlying area, got out to check something on their light vehicle, then tripped and fell approximately seven feet down a hill side. They struck a cactus and received abrasions on their torso and a laceration on their hand that required stitches.
IE claims ee was struck in the face (left cheek) while attempting to open #2 standard crusher feed box in the fine crushing plant. Alleged incident occurred on 10/17/2018, the employees dentist appointment on 10/22/18 put ee off work 10/23/2018. The mine is controverting this as work related injury.

While washing down slurry from the lip of the number 6 ball mill cyclone underflow box, employee was splashed with slurry mixture in the left eye.
While stacking filter plates on a pallet, employee caught middle finger on right hand in-between 2 filter plates resulting in 10 stitches.
EE pulled the Tractor Trailer w/container up to seal placement rack. When on top of the rack there is a ""trap-door' in the catwalk. EE said EE placed the security hook to trap door to prevent it from falling. As EE was placing the seal lock on the manhole cover, the trap door fell down. the trap door hit EE in the back of EE's head pushing EE's forehead into the catwalk.
While installing and checking alignment on a dust collector belt, employee received a laceration on right index finger requiring stitches.

An employee was starting to cut out a bearing on a pump barrel when the grease flashed in the bearing and a fire ball of grease shot out of the bearing and deflected off the face shield of the employee cutting the bearing and struck another employee standing nearby in the facial area with hot grease.
Employee was torqueing bolts on a 789 front suspension. During the torqueing process the tooling slipped, this caused the reaction arm of the gun to come around and contact one of his fingers. He sustained a cut of the fingertip and fractured the tip of the bone.
Employee attempted to stand from the sitting position and felt sharp pain in his lower back.
After washing down haul truck employee slipped on wet surface causing groin strain.

Employee tasked with troubleshooting 480v belt drive motor due to poor connection at termination. As he attempted to remove splice covering to visually inspect, the knife slipped and contacted left thumb knuckle resulting in a minor laceration. Medical treatment administered, sutures were applied to close the wound.
Employee walked into a low hanging light fixture hitting his head and causing neck strain
Employee was not used to working under smaller haul trucks and stood up and hit his head causing back pain
Employee slipped on water while walking to the Lime Plant and he fell backwards hitting his head on the cement slab.
Another contractor's truck was backing up and his rear bumper struck our worker's right leg/hip. Worker was taken to local hospital for x-rays/tests. All x-rays/tests came back clear. No broken bones or injuries sustained, only bruising. Worker was released to full work duty on June 29, 2015.
Employee was instructed to install a new high voltage feed to power a pump. The cable was heavy and he sprained his shoulder when positioning the cable.
Closing 2C cyclone and stepped up on the beam to reach airline. When stepping down, did not place foot all the way on the ground, and slipped on the beam causing her to twist her right ankle. Requested no further medical attention at this time.

EE was attempting to line up an 18 inch pipe elbow. EE placed his left arm between the pipe elbow and a bracket as the elbow was being lowered resulting in his arm being caught between the pipe elbow and the bracket.
