Showing all 3Injured employee had removed a screw for repair of equipment. Employee was reinstalling the screw when the screw drill was pressed and the screw had a rough metal edge and caught EE's index finger. Cutting it 3 inches in length requiring stitches. No loss of work and was able to return the following day.
After lock out tag out of the equipment, the employee climbed to an area to be able to reach the rock that had the machine jammed. Upon releasing the rock, the rotor rolled toward the employee catching right foot forcing toes toward calf.
