Struck by... (Not Elsewhere Classified)

Injured employee had removed a screw for repair of equipment. Employee was reinstalling the screw when the screw drill was pressed and the screw had a rough metal edge and caught EE's index finger. Cutting it 3 inches in length requiring stitches. No loss of work and was able to return the following day.

Was lifting wear parts in and out of a VSI crusher when he felt a strain in the mid section He did not think much about it at the time.

Struck by flying object

EE was doing clean up under equipment with Bobcat skidsteer loader and got dust/dirt in his right eye. EE was wearing safety goggles.

WAS TRYING TO REMOVE (CUT) LID OFF THE SNAKER, WAS PULLING & PUSHING WHEN HE INJURED HIS LOWER BACK.

EE WAS TAKING A MEASUREMENT, HE WAS STANDING ON THE SHAKER. HIS FOOT SLIPPED AND HE FELL FORWARD STRIKING HIS CHIN ON PLATE AND FELL ON SHOULDER ON PLATFORM. HE CUT HIS CHIN AND BRUISED HIS LE FT SHOULDER AND RIBS.
