Employee was operating the shearer when a shield unexpectedly was advanced. This caused the employee to be pinned between the shield and panline. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

Employee stood up on the bolter to install resin and the bolt. As EE stepped down, EE's foot inadvertently went between the bolter station and the bolter frame as the bolter frame was being retracted. It did not involve any compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

Employee was assisting scoop operator in storing a shield. After EE placed a crib under the shield, EE exited the area between the shield and the rib. The shield unexpectedly shifted and pinned EE's left leg against the rib. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

Employee stepped onto the back of the CM to slide a roof strap across to the bolter while the CM was in operation. Employee slipped as employee slid the strap across the miner catching leg between miner frame & the boom as it was sumping toward the face. It did not involve any equipment or compliance issues. It also did not involve job skills, training, PPE or protective devices.

Employee was bolting, and as ee swung the bolter head a drill steel caught the bolt tray causing it to swing around striking ee in the face. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices. Employee returned to work on next scheduled shift, 4/9/18, but began missing work on 4/12/18.

While traveling on haulage, pole came off of trolley wire, then came down striking the EE on hard hat. Did not involve any compliance issues. Personal protective equipment was being used. This did not involve job proficiency or attitude. Employee called off sick on next work day. Did not know time in question was injury related until 12/26/2017 when EE brought doctors slip.

While using a track jack to re-rail, a mobile jack slid sideways causing employees finger to get pinched between the track jack and mobile. This did not involve any compliance issues. Personal protective equipment was being used and this did not involve job proficiency attitude or machinery.

Rock dust hose got caught on something while employee was dragging it causing ee to feel a strain in shoulder. This did not involve any compliance issues. Personal protective equipment was being used and this did not involve attitude, job proficiency, or machinery. Employee worked until 10/23/2017 when doctor took ee off of work.

While installing a 16' cable bolt, employee felt a pop in right shoulder. This did not involve any compliance issues. Personal protective equipment was being used and did not involve job proficiency, attitude, or machinery. Employee was injured on 3/7/2017 but the doctor did not take EE off of work until 3/20/2017.

The injured employee was using a hand grinder to remove a keeper pin on a hydraulic cylinder located at the parts car. When operating the grinder it kicked back causing a laceration to right hand. The employee was wearing gloves. The employee received 4 sutures. It didn't involve any compliance issues. It didn't involve skills proficiency,training or attitude.

While guiding cable into the monorail winch employees glove got caught between the rope and roller pulling EE's hand in toward the spooling drum and cutting the tip of EE's right index finger. This did not involve any compliance issues. Personal protective equipment was being utilized. This did not involve job proficiency, or attitude.
