EE was repairing a defective windshield wiper unit. EE saw motor linkage was disconnected. While reconnecting the linkage, the wiper motor unexpectedly activated, pinching their left ring finger between the linkage and the ball. Seen at clinic, diagnosed with fractures to the left ring fingertip, and wound closed with sutures. Released to modified duty.

Ee reported to NGM on 5/4/26 that on 4/3/26 ee was working 'in EE's columns' when a contractor and crew started hydro blasting above EE, causing debris to fall onto EE. EE reports that EE and a coworker yelled and went to the control room. Injury not reported to NGM until 5/4/26. No signs of injury to ee. Site cannot confirm incident occurred as reported by ee.

EE was sprayed by another EE with water from a water cannon. Seen at clinic, diagnosed with a contusion and on follow-up referred to PT and MRI. Released to modified duty.

Ee operating 4.5in disc grinder fitted w/cutoff wheel to remove fender from flatbed trailer. Tool unexpectedly kicked back and the rotating wheel contacted the ee's left hand, cutting through the leather work glove, and cutting the pinky finger. Seen at ER, wound sutured, ee released to full duty.

IP was removing bolts from the cutting edge of a loader bucket with an impact gun. The bolt came unseated, and a sharp edge on the bolt lacerated the IP's leg above the knee.

Ee loading a 16' bolt into resin head and centralizer. Coworker pressed button, closing centralizer on ee's hand. Ee seen at clinic, laceration to pinky. Pinky buddy taped and laceration closed with steristrips. On follow-up, antibiotics given for infection, finger redressed and buddy taped. Ee working full duty.

EE was working on a pressure washer. They plugged it in and was checking belts. They leaned over onto the power button. The washer turned on, and the pulley belt pulled their glove in, amputating the tip of their left middle finger. Seen at ER, diagnosed with traumatic partial amputation, released to modified duty.

Ee was drilling and dirt/dust went in right eye. Seen at Golden Health, diagnosed with irritation only, no injury, and given prescription antibiotic as a preventative measure.

Ee was using band saw to cut cap hooks. EE's glove got caught in the band saw and cut EE's finger. Seen at ER, laceration was closed with sutures, released with restrictions. Site offering modified duty.

Ee removing haul truck tire on CAT 795. EE was trying to get rad gun on the nut to loosen them and improperly placed their right hand, accidentally hit trigger of rad gun with left hand, pinching right thumb between gun and hub of tire. Seen at clinic, released with restrictions and referred to ortho.

EE did ride-along in shovel at the Pipeline pit. EE reports ride was rough, causing them neck and back pain. Seen at clinic, diagnosed with cervical and thoracic spine strain, released with restrictions. Site offering modified duty.

EE was sitting in cab being loaded. On last pass there was a large boulder that was too big to go through the opening of the shovel so the operator "shook it" off, the boulder hit the overshot on the bed of the truck, shaking and bouncing the truck. EE reported, asked to be seen by doctor on 5/20. Diagnosed with cervical pain and thoracic sprain, released with restrictions.

Ee was using torch to heat a part to remove it when flashback occurred. Ee burned on both arms and face. Seen at ER, diagnosed with 1st and 2nd degree burns on both arms and face, and referred to clinic. Seen at clinic following day, released to restrictions.

On Tuesday, November 5th, around 3:30 p.m., an iron worker helper injured their right middle finger while adjusting a steel I-beam. During the bolting process, the worker's finger got caught between the reaction arm of a pneumatic tool and the I-beam, causing a laceration.

Ee reports pressure washing a piece of equipment when debris shot behind ee's safety glasses into left eye. Ee was seen at Humboldt General, diagnosed with a corneal abrasion and given prescription medication. Released to full duty.

EE was removing hose from a machine with cutting wheel. EE was wearing a face shield, gloves, and safety glasses. A piece of debris got in EE's right eye. EE was seen at health clinic, referred to eyecare clinic, diagnosed with a conjunctival abrasion, prescribed antibiotics, and released to full duty.

EE hooked a 4 way chain to component with crane. When they put tension on the chain, it spun and hit them in the mouth. Broken front tooth. Seen at clinic, referred to a dentist, and released to full duty.

EE was retorquing lug nuts. EE was moving the gun to the next lug nut and struck their thumb on the edge of the wheel. Seen at ER, laceration was closed with sutures, and released to full duty.

Hydroblast cut and injection of water in the left hand between thumb and pointer finger

Ee was tightening bolts with an impact gun when the bolt spun against ee's glove, cutting the glove and cutting ee's palm. Seen at ER, laceration closed with steri-strips and glue, and on follow-up ee placed on restrictions site can accommodate.

EE was hand stabbing a bolt into the face to perform a resin test. Their right hand was in the line of fire inside the centralizer clamp and the clamp came down on their hand. The operator released the clamp, releasing the EE's hand from the centralizer. Seen at the ER, diagnosed with a fracture and laceration, and referred for an orthopedic consult. Released to full duty.

EE using 1.5 ton crane moving motor. The motor was on the manual crane approximately 4" from the cat walk, and the hoist system failed, and the motor fell, striking the EE on the head/shoulder/back. Seen at ER, diagnosed with compression fracture to spine, laceration/soft tissue injury to right leg. EE put off work, non-weight bearing.

EE reports was lying on the ground, air arcing on a piece of equipment. When EE went to change positions, EE felt pain. Seen at ER and clinic. Suspected non-occupational condition ruled out by ultrasound. EE on restricted duty.

Attempting to break top sub off tooling using chain wrench while drill was rotating. Wrench struck employee in face resulting in laceration requiring stitches.

EE was working with another EE on a Geho pump. Removing diaphragm door, took last nut off, door popped off, and hot slurry struck the EE in the face. Seen at ER, trasnported by air to Medical Center for burn consult and x-rays of arm/shoulder. No fracture, EE treated and released for burns. Released to full duty for burns effective 8/2.

Ee being loaded by hydraulic shovel and reports bucket "slammed" into bed of truck jostling ee in the cab. Reports pain in back, seen at the clinic on 7/7, diagnosed with cervical, thoracic, lumbar strains, released to restricted duty. Medical and factual investigation completed 7/26, claim accepted by workers' comp 7/27, resulting in this report.

Work Shift started on 6/01/2023. Incident occurred on 06/02/2023. Employee was using an angle grinder to buff rubber on floor of a flotation cell. The angle grinder kicked back resulting in a laceration to right knee.

A utility crew was assigned to complete rehab work at the 1600 haulage corridor, which entailed removing utility lines. A team of three employees were utilizing a scissor deck with a pipe handler to remove 10" steel pipe. They had removed one pipe section and were starting to disconnect the second pipe when they sustained injuries.

Blocked transfer line impeded (frozen product) the flow of material. In an attempt to push the product through to the receiving tank, the driver attached an airline. The pressure caused the material to mist from the air connection exposing the driver to the sodium hydroxide. The result was a chemical burn of left hand; full thickness burn of left forearm.

Ee was air arcing metal and dropped a hot arc rod on EE's left leg. Seen at clinic, diagnosed with a puncture wound, released to full duty. EE developed an infection, was seen at Renown hospital on 9/12 and admitted for care. Surgical wound cleaning done and ee certified off work.

Ee was working on a drill. EE was getting ready to nip in, and hit the spool button rather than the unspool button, catching EE's glove in the spool, pulling EE's hand in. Seen at the ER, diagnosed with a laceration to the left thumb. No fracture, sutures required. Ee released with restrictions site can accommodate.

The employee was using a core press, the ram was bent under pressure. When the employee retracted the ram, the ram launched the tube upward striking the employee in the hand resulting in a laceration & forehead (eyebrow).

Ee operating rubber tired dozer on waste dump. Reports backing over large rock with left front tire, causing dozer to bounce hard, jostling EE in cab. Reported that day, completed shift, work two more shifts. Requested doctor visit on 7/21, diagnosed with strain. Referred to physical therapy at ee's request.

When pulling a column pipe to replace a pump. The column pipe failed and fell inside the casing. This resulted in tensioning of the electrical pump cable that was being spooled. The individual was clear of event and went to assist a co-worker and was contacted by the cable coming off the spool. This resulted in abrasion to right side of torso, fractured rib and bruised lung.

In the process of pulling a column pipe to replace a pump. The column pipe failed and fell inside the casing. This resulted in tensioning of the electrical pump cable that was being spooled. This tension caused the spool to move and is when it contacted the individual next to the spooler. Resulted in facture to tailbone and bone in back, right sprained ankle and skin abrasions.

Two process employees were working to clear a plugged drain with an air lance. The plug cleared and sulfuric acid sprayed injured employee in face, arms, and wrist. Transferred to burn center; diagnosis chemical burns. Employee released on 5/8. Lost time.

Ee was grinding a weld out when the grinding wheel EE was using blew apart, sending fragments that struck EE's thigh and face. Seen at Golden Health, sutures required to close wound on thigh. Ee released to return to work full duty 04/04/22, EE's next scheduled shift. No lost time.

EE was moving air line to lock it out, and the air line came on and hit them on the knee. Reported it on date of injury, but sought treatment on 3/25. Diagnosed with abrasion/contusion/infection to right knee. Antibiotics prescribed to treat infection. Released to full duty.

EE being loaded at shovel on 191. Hoist on shovel went out, the shovel struck the bed of the truck forcefully, leaving the EE in an awkward position for a short period of time. Initial impact jostled EE in cab. Seen at health center, released with restrictions.

Injured employee placed a pipe wrench on a drill rod sub and when activated, the wrench struck the employee in the hardhat above right eye knocking employee to the ground.

On 1/8/2022, EE was wrapped up in a rotating hose after trying to straighten the hose out without de-energizing the equipment or placing controls into neutral. EE was treated for a dislocated elbow. EE received prescriptions from the facility. EE was released on light duty restriction until 3/24/2022. Final disposition information to be provided on 10/5/2022.

Ee was using an impact wrench to remove bolts. A nut on one of the bolts turned and cut the ee's left middle finger. Seen at Golden Health, stitches required to close the cut, ee released to full duty.

EE backing up haul truck, shovel bucket struck bed, stopping truck suddenly and jostling the EE in the cab. The EE was seen at health clinic, diagnosed with sprain, and released to full duty. On follow-up on 7/29/21, EE prescribed physical therapy and released with restrictions, making this reportable.

EE was using a pressure washer. Enough pressure built up in the wand, causing it to "jump back" and strike the EE in the face. EE was seen at the ER where a laceration near the right eye was closed with sutures. EE released to full duty.

Manpower employee used a battery operated circular saw to cut cardboard for sample labeling. EE cut left thumb with saw.

Employee was using a grinder to repair a piece of equipment. Later that evening EE felt something in EE's right eye and was taken to the hospital where they removed a small piece of debris.

Ee was removing bolts with impact and felt something go into right eye. Ee was wearing safety glasses. An EMT flushed ee's eye and the ee returned to work. Ee requested to be seen by eye doctor due to ongoing irritation. Seen on 10/12, a rust ring was cleaned from the eye and ee was prescribed antibiotics. Ee is released to full duty.

The EE was helping at the table to trip out drill pipe. The EE grabbed the wrench at the table and the driller moved the hydraulic cylinder pinching the finger between the table bushing and the wrench handle.

Ee was pushing down the dig face on the 5380. The dozer hit a rock in the face, causing it to stop abruptly, and the ee's teeth to bite down/clash together, fracturing a molar on the lower left side. Seen at the dentist who confirmed no unusual decay causing fracture, determined work related.
