Showing all 6While bolting in the No#5 heading, the drill operator was swinging in to put up inside bolt when a piece of rock fell off the canopy striking EE's left hand. The rock was round in diameter 2" thick x 10".
This was a roof bolter when EE let EE's canopy down the pin broke letting the canopy fall down striking EE in the shoulder and broke EE's finger.
EE was standing in the intersection of the #4 entry. The bolt machine backed out and turned right handed sharply to go through 4 right. When doing so, the rear end of the bolt machine came around quickly. The bumper of the bolter went over top of EE's right foot, pinning it to the ground. Resulting in a fracture to the metatarsal bone in right foot.
EE was installing a roof bolt and when EE raised the drill pot, to attach the wrench to the bolt head, the bolt caught and bent, mashing EE's pinky finger to the frame of the drill head.
