EE was installing a 18" split set with a Jackleg. As the EE was pulling the jackleg out of the bolt, the jackleg pivoted and pinched EE's hand between the jackleg and the wingnut.
EE was struck in the face by a rock that was dislodged from the face. The miner was installing a bolt into the fingers of the bolter when the bolt hit the face, a rock slide down the bolt and struck the employee.
While bolting a piece of rock fell through the chain link fencing and struck the employee in the arm.
EE Was removing the seals on a Wilson-Snyder pump with a 1 ton overhead hoist when the hoist fell from the hook and struck the employee on the hand.
EE received a laceration to left knee when a rock they were attempting to dislodge rolled down the rib, hit a small ledge and struck them in the knee.
While installing ground support, the employee got finger caught between the machine and the leg.
EE gets middle and ring fingers pinched between traverse slide and rotation slide cylinder nut of drill after resting hand on the side of the traverse slide not realizing EE had put two fingers in the opening for the rotation slide cylinder.
EE was using an impact wrench with a socket on it to tighten a nut on a yoke. The socket came off while in use and struck the EE in the mouth.
EE was bolting up a hole in the wire in the back at the intersection, EE said they drilled by either a bolt or through a slip. When EE pulled the machine back out of the hole it hung up. When EE got it to let go the leg was released all the way and the machine came down all at once.
EE and partner were using a jackleg drill for installation of rock bolts. While pulling hard on the jackleg to extract the drill steel from the hole the steel suddenly pulled free causing EE to fall backwards and right thumb got caught between the jackleg and a concrete slab causing crushing injuries to the thumb.
The employee was bolting the lower hole on the right rib with a Jackleg drill, when the employee started to pull the machine out of the hole he felt a pain in his lower back. The employee received medical treatment on 7/24/2015.
On 9-15-14 E was pushing a 12' dywidag up into a hole drilled in the back when a piece of material fell behind his safety glasses and got into his right eye. E flushed the eye repeatedly. When E returned to work on 9-16-14 he was taken to the eye doctor where a piece of embedded metal was removed from his eye.
EE was burned on his right bicep. EE had been bolting with a 4' split set driver and leaned it against his bicep while it was extremely hot from driving in split sets.
Employee injured his left shoulder when the drill steel he was drilling with hit an existing bolt causing the jackleg to spin.
EE assumed the operation of the jackleg drilled out for and installed four bolts then moved over to his left to begin the next row. He drilled three bolt holes and was on the 4th hole, which was about chest level, when the goose neck came loose striking him in the nose.
EMPLOYEE WAS WORKING ON THE TRACTOR AND AS HE WENT TO GET DOWN OFF IT, HIS PANT LEG CAUGHT THE CLUTCH PEDDLE CAUSING HIM TO LOSE BALANCE AND FALL. HIS LEG HIT THE FRAME OF THE TRACTOR CAUSING A SMALL FRACTURE TO HIS LOWER RIGHT LEG
Employee was bolting in a stope using a jackleg, when he attempted to pull the jackleg and steel out of a hole he just drilled he injured his shoulder.
Workers were rotating the slide on a long hole drill. As the slide rotated, it struck the drill steel. The drill steel shifted suddenly, striking the worker in the mouth.
Worker lost his balance while drilling with a jackleg. The drill began to fall and he stopped it by grabbing the saddle/hammer area. The drill rotated back on the leg, pinching his right hand.
Employee unintentionally depressed the air actuated trigger on a grease gun. Discharge from the grease gun got into the employee's right eye. The eye was flushed with eye wash underground and he was then taken to an optometrist. The optometrist prescribed an antibiotic to prevent an infection.
While removing compressor rope, using a sheave pulley that was with original compressor setup, EE was stationed outside the pinch points and was the signal person for the operator on the Mini Excavator. The injured employee is not sure how exactly he ended up with hand into the pulley. His left hand, index, middle and ring fingers were injured.
While bolting the back, the jackleg the miner was using, hung up and the drill spun around and the handle struck the miner in the mouth. The laceration required 8 stitches.
Miner was bolting back with a jackleg drill. The leg on the drill blew apart and the jackleg drill struck the miner on the right ankle. The miner was placed on light duty.
Injury was using a cutting torch to cut the bit off of a bucket when he tried to stand up he stumbled and the flame of the torch ran across his left hand. He burned 3 of his fingers.
The workers rode the Alimak to the face. The injured opened the trap door and placed his left hand on the top deck to climb the ladder. Before he could climb the trap door slammed shut on his right hand.
Worker was drilling a corner back hole on a raise round. As he was drilling the drill steel became stuck causing the drill to spin and the handle of the drill pinched the workers finger between the drill and wall.
Worker was drilling a hole to install an alimak curve rail anchor bolt. The drill steel/bit intersected a cable bolt causing the drill to spin suddenly pinching the workers left thumb between the drill and alimak rail resulting in a laceration.
A Geo-tech was grinding on some metal and a piece of metal got behind his safety glasses and became embedded in his right eye
Employee had just finished blow piping a drill hole when a small rock came out of the crown and struck him on the left forearm, causing a small laceration. The wound required sutures.
Employee was attempting to clear a plugged sand line. He changed the line w/air and water, but it didn't clear. He unhooked air and bled off pressure. Hooked water back up a went to end of line to see if it was clearing the sand. There was some pressure in a void in the pipe that broke loose and made the pipe surge. The 4" pipe struck him in the face.
EMPLOYEE WAS BOLTING WITH A JACKLEG DRILL. WHEN HE PULLED THE MACHINE OUT OF A HOLE IN THE BACK, HE LOST CONTROL AND FELL AGAINST THE RIB, HITTING HIS ELBOW. THIS CAUSED A LACERATION REQUIRING 3 SUTURES.
EE WAS ROCK BOLTING AND GOT DIRT IN EYES. THE RIGHT EYE HAD A RUST RING THAT NEEDED TO BE CLEANED UP AND HE ENDED UP WITH RESTRICTED DUTY.
EE was pulling on jackleg drill to get the steel out of a bad hole & felt something pop in his shoulder. Possible rotator cuff injury. waiting on MRI
EMPLOYEE WAS ROCK BOLTING WHEN A SMALL PIECE OF ROCK GOT IN HIS EYE. IT BECAME EMBEDDED.
EE WAS HELPING MOVE A PIECE OF STEEL BEDLINER INA TIGHT AREA WITH A PEDISTAL FAN IN THE AREA. ASHE RAISED UP, THE MIDDLE FINGER OF HIS LEFT HANDSTUCK THROUGH A DAMAGED PORTION OF THE GUARD ON THE FAN. THE FAN BLADE STRUCK HIS TIP OF THE FINGER CAUSING A LACERATION AND FRACTURE.
EE WAS ROCK BOLTING AND A SMALL PIECE OF METAL GOT IN HIS EYE.
EE WAS BOLTING THE BACK IN A SLUSHER STOPE. A ROCK CAME OUT OF THE BACK AND HIT HIS LEFT HAND, CAUSING ALACERATION TO HIS LITTLE FINGER. THE WOUND REQUIRED SUTURES.
EE WAS ROCK BOLTING RIB WALL WHEN SOME SAMLL LOOSE ROCK STARTED TO COME DOWN. AS HE MOVED OUT OF THE WAY HE HIT HIS RIGHT ARM ON THE OPPOSITE RIB CAUSING A SMALL LACERATION WHICH REQUIRED ONE SUTURE.
MINER WAS BOLTING THE BACK IN 6020 DEVELOPEMENT RAMP. HE WAS PUTTING IN LEAD BOLTS FOR DOUBLE WRAP OF WIRE. DRILLING HOLE FOR 4TH BOLT AND WEDGE SHAPED SLAB CAME OUT BETWEEN 2ND AND 3RD BOLTS. IT HIT IS LEFT FOREARM. RESULTING IN RESTRICTEDDUTY.
TWO EMPLOYEES WERE UNLOADING A BUNDLE OF 9' MATSFROM MINE TENDER. THEY WERE USING A JIB CRANE W/CHAIN. THE MATS HUNG UP ON THE TAILGATE OF MINE TENDER. HE TRIED TO FREE THEM AND AS THEY CAME A WAY FROM THE TAILGATE THEY FELL ON HIS RIGHT INDEX FINGER SMASHING IT. TIP OF FINGER WAS FRACTURED AND NEEDED SUTURES.
EE WAS ROCKBOLTING WHEN THE THERE WAS A SMALL FACE BURST. A SMALL PIECE OF FLY ROCK CUT HIS LEFT FOREARM REQUIRING 4 STITCHES.
EMPLOYEE WAS HELPING LOAD A SHOT CRETE BAG WITH OTHER EMPLOYEE OPERATING TUGGER. HE WAS HOLDING THE TUGGER CABLE AND HOOK IN PLACE AND HAD HIS HAND TOO CLOSE TO THE HOOK WHEN THE OTHER EMPLOYE E TIGHTENED THE CABLE, PINCHING HIS RIGHT THUMB IN THE HOOK, REQUIRING STITCHES.
EE WAS DRILLING AND BOLTING RIB WALL IN STOPE. THE WALL SLUFFED AND PUSHED THE JACKLEG AGAINST OPPOSITE RIB, PINCHED THE LITTLE FINGER ON THE RIGHT HAND.
A SLAB FELL FROM THE FACE WHILE HE WAS ROCK BOLTING THE RIB, STRIKING AND LACERATING HIS LEFT ARM.
