Crew was spooling in the dredge anchor cable using the winch. The cable started to become off track of the spool when the employee placed their right foot into the cable to prevent it coming off the spool completely. The employee reports their foot slipping off the cable driving their heel into the decking/floor of the dredge causing immediate discomfort to the right ankle.

Welding on rotary in maintenance building, when a spark burnt through his welding glove, causing burn to left thumb. Went to the Dr and received prescription fo anti-biotic.
The ee was operating a mobile pettibone crane and lifting a dredge pontoon when the right front outrigger sank into the sand causing the pettibone to lay over onto the right side. It was set back up.
THE EMPLOYEE WAS CUTTING METAL ON THE UNDERSIDE OF THE OVERSIZED ROCK CHUTE THAT WAS LINED ON THE TOP SIDE WITH NEOPRENE RUBBER LINER. THE TORCH MELTED SOME OF THE LINER CAUSING THE EMPLOYEE TO BREATHE THE FUMES.
THE EMPLOYEE IS A HEAVY EQUIPMENT OPERATOR AND HAS HAD PERIODIC BACK PROBLEMS OVER THE PAST 40 YEARS. ON 3/28/01 HE WAS OPERATING A SCRAPPER & HIS BACK STARTED BOTHERING MORE THROUGHOUT THE DA Y. WENT TO DR SEVERAL WEEKS LATER.
