Removing a bolt with an impactor to replace the wear plate the carriage bolt moved up. Employee pushed down on the bolt and the sharp edge on the bolt caught their glove pulling a finger down on the rotating bolt cutting their finger.

Showing all 12Employee was running a scraper and was being pushed by another scraper. EE alleges that the other scraper hit EE hard from behind and EE complained that EE's back was sore and ask to go home for the day. EE supposed to return to work the next day but asked for the day off. On 6/16/23 EE went to the Occ Health Clinic without notifying anyone.
Injured employee was removing bolts from a belt frame on a crusher screen. He had his arm underneath the frame piece when the final bolt was removed the frame piece fell pinching his arm between the piece and the box. He went to the emergency room and was given a prescription and released back to full duty.
dust and metal grindings blew into eye of miner while repairing conveyor belt. Miner was wearing safety glasses and face shield, but should have worn goggles due to windy conditions, and postioned himself upwind of the work area.

EE HAD DUST ON HIS COVERALLS AND HARD HAT. AS HE WALKED BY THE BACK OF THE GEN SET, THE DUST BLEW INTO HIS FACE FROM THE FAN OF THE ENGINE.
EMPLOYEE THINKS HE WAS USING A GRINDER AT THE TIME OF INJURY. EXACT CAUSE BELIEVED TO BE GRINDER USE.
EE WAS REPAIRING CRUSHER USING GRINDER. GOT A PARTICLE IN EYE.
