Showing all 27Two employees and a contractor crane operator back a crane into position. One employee was in front and the other was in the back. The crane operator was backing and was not aware the employee in the back had placed themselves behind the crane and the employee was pinned between the hand rail and the crane injuring their leg.
The EE was operating a 14 ton manitex boom truck in a narrow road between B-Tanks preparing to move an empty MCC panel. EE was unable to extend the main outriggers due to width of road. EE didn't like the position and began to pull boom back in and the truck begin tilting, so the injured worker jumped off the boom truck onto the ground.
Boom truck was not setup properly due to limitations caused by concrete barriers. After the boom truck flipped on its side due to outriggers not being deployed an all stop was not called to access the hazards presented by the equipment failure. After attempting to safely stabilize the hoisted load, consideration was not made for a safe clear escape path.
EMPLOYEE CAUGHT RIGHT HAND MIDDLE, RING AND PINKY FINGERS IN AIR ACTUATED KNIFE GATE VALVE ALMOST SEVERING THEM REQUIRING SURGERY.
College Summer Work employee was cutting a 2x8 board with a skill saw when the board dropped - EE reached to catch board striking saw cutting EE's left hand index, ring and pinky fingers. EE was taken to McDuffie ER then transferred to the Doctor's Hospital in Augusta, GA where surgery was performed to repair nerve and tendon damage.
THIS IS A CONTRACTOR EMPLOYEE WORKING ON OUR MINING PROPERTY WHO WAS PRESSURE WASHING PIPE WHEN EE STEPPED INTO A HOLE LOSING BALANCE. EE LOST CONTROL OF PRESSURE WASHER WAND AND IT HIT LEFT LEFG SHIN CAUSING 2ND DEGREE BURNS.
EMPLOYEE WAS WELDING AND OTHER CONTRACTORS WERE ABOVE EMPLOYEE, GRINDING. EMPLOYEE WAS WEARING PPE BUT A PIECE OF RUST BECAME EMBEDDED IN THE RIGHT EYE.
Employee was welding, cutting, and grinding plate metal and got a small metal flake in his eye. Employee reported that his eye felt irritated, so it was washed with an eye rinse solution. Employee was then sent to the doctor, and the metal flake was removed. Employee is expected to make a full recovery.
EMPLOYEE WAS CUTTING METAL WITH TORCH, WHEN SOME MOLTEN METAL WENT INSIDE HIS GLOVES, BURNING WRIST AND ARM.
Common procedure of installing PVC slip-joint pipe was being done when excavator was being used to push a 4x4 block against the pipe to put together. The 4x4 block broke and was propelled through the air and struck employee in the leg.
EMP WAS USING A BENCH GRINDER TO SMOOTH THE EDGE OF A FLAT BAR WHEN BAR KICKED UPWARDS PULLING HIS RIGHT THUMB INTO THE GRINDING WHEEL RESULTING IN HIS THUMB NAIL BEING 2/3 SNATCHED OFF ALONG WITH FLESH.
Another employee was cutting rebar on cutoff saw when a piece of rebar flew off striking injured employee on right arm causing laceration requiring stitches.
Employee and co-worker were removing motor from shelf with a hoist. Injured employee was holding motor guiding it down to level when co-worker pushed wrong switch causing motor to drop on injured employee's left little finger crushing it.

EMPLOYEE WAS WELDING AND GRINDING IN THE SHOP. HE NOTICED HIS RIGHT EYE GETTING IRRITATED. HE WASHED IT SEVERAL TIMES AND TOOK SOME SOLUTION HOME TO WASH OUT ALSO. HE WAS WEARING SAFETY GLA SSES AND A FACE SHIELD. HE WENT TO DOCTOR THE NEXT DAY.
EMPLOYEE WAS THREADING CONDUIT IN RIGID 300 THREADING MACHINE. HAD THREADED CONDUIT, WENT TO REMOVE BUT DID NOT WAIT FOR MACHINE TO STOP. PIPE HUNG IN JAWS OF MACHINE, CAUSING PIPE TO CATCH HIS RIGHT HAND BETWEEN MACHINE AND CONDUIT MASHING 4TH FINGER.
EE WAS RIDING IN GOLF CART IN EARLY MORNING AND DID NOT SEE ROPE BARRIER. GOLF CART WENT THROUGHAND UNDER ROPE BARRIER CAUSING IT TO CUT AND BRUISE LEFT EYE AND CHEEK REQUIRING 6 STITCHES.
CUTTING STEEL PLATES WITH TORCH WEARING GOGGLES WHEN FIRE (METAL) GOT IN EYE CAUSING IRRITATION.
EE ALONG WITH OTHER CO-WORKERS WERE CHANGING A SHAFT IN A BLUNGER. THEY HAD TO USE A HYDROLIFT TO LIFT SHAFT THEN TRANSFER IT TO A CHAIN HOIST. THE SHAFT SLIPPED THROUGH THE HOIST AND THE BLAD E OF THE SHAFT HIT EE'S RIGHT LEG CUTTING IT REQUIRING FIVE STITCHES.
EE WAS INSTALLING SUPPORT BRACKETS TO HOLD WATER LINE. HE WAS USING GRINDER AND GRINDING OVERHEAD AND PARTICLE OF METAL GOT IN LEFT EYE. HE WAS WEARING SAFETY GLASSES WITH SIDESHIELDS.
REMOVING MOTORMOUNT OFF ENGINE OF BACKHOE #75. AIR WRENCH DIDN'T TURN OFF; BOLT CAME OUT CAUSING MOTORMOUNT TO SLASH RIGHT INDEX FINGER. SUTURES REQUIRED.
