Employee was cutting down a broken tree. It twisted and hit employee on the side of the head.
Employee was burning bolts off machinery with a torch. A washer went down his right boot and burned his ankle. About 2 weeks later the employee went to the hospital for treatment. EMPLOYER WAS UNAWARE OF INJURY UNTIL INVOICE WAS RECEIVED AND EMPLOYEE WAS QUESTIONED ON 6-20-11.
The employee was assisting a contract welder who was working in a feeder area of the plant. The injured employee was acting as a spotter in the event that the welder needed help due to working in a confined space.
Employee was using a hand grinder.
Tried to turn rotor in 43x40 crusher; right foot got caught between field plate and breaker bar.
Was using torch to burn head off bolt on 6x20 scrubber when hot metal residue flew back into his ear canal. ee failed to report injury until 1/15/07 at which time he was sent to medical provider.
WHILE WORKING ON THE MOTOR A PIECE OF ROCK WAS JAMMED IN THE MOTOR. WHEN HE SHOOK THE ROCK LOOSE, THE COUNTERWEIGHT PULLED THE BELT AND HIS FINGER INTO THE PULLEY.
EE WAS OPERATING A DROP BALL ATTACHED TO A TRACK DRIVEN SHOVEL. WHILE MOVING A LEVER IN PREPARATION TO MOVE SHOVEL THE LEVER JERKED FROM EMPLOYEE'S HAND CAUSING STRAIN TO LEFT SHOULDER.
