While swinging the 103 hoist crane around with a load on the hoist, EE was pulling from a rope and co-worker was assisting to swing the crane by pulling from the hoist chain. The rope broke. EE fell backwards onto the Nordberg crusher. Adjustment ran. EE fractured left side ribs.
While cutting steel in a band saw a small amount of steel particles built up close to the blade drive pulley. Employee lift guard and went to clear build up with finger, saw blade caught glove and pinched index finger tip of the left hand on edge of saw frame.
Employee felt and heard a tearing sound from the left shoulder while working the lever back and forth of a 1/4 ton chain come along. Employee was in the act of removing a rod assembly from a hydraulic cylinder. The hydraulic cylinder was laying horizontally in the shop on the cylinder work bench.
While EE was filling 5 gal bucket with soap and exiting an area where other containers were stored, his foot was caught on said containers and caused ee to trip. As he was falling his left forearm contacted a hot muffler on steam cleaner. This caused 2nd degree burns to effected area.
EE was replacing the rear spray bar. On water truck 21-1003, after cutting old spray bar off. He was grinding to prepare for new spray bar. He was wearing safety glasses and face shield when foreign object got in right eye.
While changing batteries in a truck using a socket and butterfly air ratchet, the socket fell off of the air ratchet. When trying to put the socket back onto the rachet, he accidentally bumped the air rachet trigger mechanism, causing the socket to spin lacerating the palm of his right hand resulting in four stitches.
EE was adjusting dust control water valve at the blue rock jaw crusher as a loader dumped rock into the plants feed hopper, a small flying rock struck and scratched his left eye. EE's prescription glaases did not come with side shields.
EE was adjusting the open setting of a 48" gyradisc crusher to assist with the unplugging of the machine. When changing the position of the crusher's hydraulic ram his left long finger got caught between the cylinder and the ram head injury was diagnosed as a left long finger crush injury on 5/12/06. EE was placed on medical duty.
WHILE OPERATING ENGINE LATHE, A LONG, SHARP SHAVING FLEW OUT AND LACERATED EE'S RIGHT MIDDLE FINGER.

CUTTING WITH A TORCH AND A PIECE OF MOLTEN METAL BURNED THROUGH SOCK AND BURNED FOOT.
(EE WAS OPERATING LATHE.) CHIP WOUND DOWN INTO CHIP PAN, GRABBED THE CHIPS IN PAN AND THE ROTATING CHUCK ON THE LATHE CAUGHT THE CHIPS AND PULLED THEM OUT ONTO EE'S HAND, CAUSING CUT.
EMPLOYEE WAS IN TRUCK SHOP DEBURRING PUNCH HEAD ON BENCH GRINDER. PUNCH SLIPPED OFF OF GRINDING WHEEL AND LEFT LITTLE FINGER CONTACTED GRINDING WHEEL, CAUSING ABRASIONS TO HIS LEFT LITTLE FINGER.
DURING PLANT REPAIRS USING 3/4" DRILL, THE DRILL TWISTED & WENT INTO SHARP STEEL. CUT HIS RT. PINKY FINGER & HAD TO HAVE 4 STITCHES.

EMPLOYEE WAS USING A HAND HELD GRINDER ON A SPLICE WHEN THE GRINDER KICKED OUT CAUSING LACERATION ON LEFT INDEX FINGER. EMPLOYEE HAD REMOVED GUARD FROM THE GRINDER PRIOR TO STARTING THE JOB.

WHILE EE WAS CHIPPING THE FEEDER PAN A LARGE ROCK FELL FROM THE EDGE AND STRUCK HIM IN THE BACK.
EE WAS DISASSEMBLING TURNBUCKLES FOR HAUL TRAILERS DUMP DOORS WITH A ROSEBUD TORCH AND A LARGE PIPEWRENCH. HE WAS HEATING THE TURNBUCKLE JAM NUT WITH TORCH IN LEFT HAND WHILE TURNING THE NUT W ITH THE WRENCH IN HIS RIGHT HAND WHEN HE PULLED THE TORCH AWAY FROM NUT. THE FLAME PASSED OVER HIS RIGHT HAND.
WORKING TO REMOVE ESSENTRIC WITH IMPACT TOOLS & PRY BAR, SHOULDER STARTED ACHING.
GRINDING STEEL PLATE FOR SIDE OF BUCKET. GRINDING DUST FELL IN LEFT EYE. HE THOUGHT IT CAME OUT, BUT EYE STARTED TO FEEL WORSE OVER THE WEEKEND.

WHILE DRILLING ON THE DRILL PRESS A PIECE OF SHAVING WAS GATTING BIGGER AND BIGGER IN AN ATTEMPT TO PUSH IT OUT OF THE WAY IT CURLED AROUND MY FINGER RESULTING IN A SEVERE CUT.
EMPLOYEE WAS GRINDING ON A PIECE OF STEEL, WITH SAFETY GLASSES IN PLACE WHEN A PARTICLE SOMEHOW FOUND ITS WAY INTO HIS LEFT EYE. THE EMPLYEE WENT TO THE DOCTOR AND HAD THE PARTICLE REMOVED.
TAKING OFF A CUTTING EDGE ON LOADER #17-365, HOLDING THE BOLT WITH HAND AND USING IMPACT WRENCH TO TAKE BOLT OFF AND THE BOLT CUT THOUGH MY GLOVE AND CUT MY HAND.
WHILE WORKING ON THE PRESS AT THE MACHINE SHOP, A PIECE OF STEEL SLIPPED OUT AND HIT HIM IN THE LEFT HIP.
