While packing cement the employee was not aware the cement bag was not in the #2 cradle properly as it filled. The bag pushed EE's right foot under the #1 cradle, cutting the top of EE's right foot. Right foot required 6 sutures.

EE was cutting a piece of metal on metal shear. EE was lining up piece to be cut when ee pushed down on pedal causing plunger to engage and drop down. When ee plunger dropped down left pinky finger was under plunger, smashing finger on tip and pulling off nail and breaking bone at tip. EE had fingertip skin sewn up and no lost time was involved.

Employee was preparing to use a hand wood router, setting the depth of the router bit to the wood. Employee turned on the router, holding the wood down with his hand; the wood slipped pulling his left index finger into the router bit, resulting in cutting his finger and severing the top tendon.

Employee was beginning to waterblast the #5 T/A duct, when hot clinker sand blew out the port hole. The hot sand blew up the sleeve of his aluminized coat causing 2nd degree burns to his right wrist and forearm.

Cleaning brick saw and nicked his left pinkie finger. Washed out at first aid station. Then driven to chanute memorial hospital to assure no infection and had 4 stitches, then he went back to work with no loss of time.

Cotter pin that holds socket in place caught employee's glove which extended the pin. This pin cut employee's left index finger. Finger received 6 stitches.

EMPLOYEE WAS TIGHTENING BOLTS ON THE DUCKWORK WITH AIR IMPACT WHEN SAFETY PIN WHICH HOLDS THE SOCKET SLIPPED OUT WHILE SPINNING. EMPLOYEE RECEIVED 14 STITCHES.

ERECTING NEW DUCT IN PLACE BY CRANE. THE DUCT WAS HUNG UP BETWEEN STEEL & WHEN IT CAME FREE, THE EE'S LEG GOT CAUGHT BETWEEN STEEL & CRANE CABLE. TAKEN TO DOCTOR. RETURNED TO JOB & WAS PLACED ON OFFICE DUTY FOR OBSERVATION. OBSERV. PERIOD WAS NOT COMPLETED: EE WAS INCARCERATED FOR UNRELATED ACTIVITY. POST-INJURY DRUG TEST: RESULTS POSITIVE.

EMPLOYEE STATED HE WAS TURNING CHUCK KEY ON THE MAY DRILL WHEN HE ACCIDENTLY HIT THE DRILL WITH THE KNEE/ LEG. THIS ACTIVATED THE DRILL, CAUSING ARM TO TWIST AND CATCHING HIS GLOVE IN THE CHUC K KEY CHAIN. EMPLOYEE HAD NOT UNPLUGGED EQUIPMENT BEFORE REPLACING DRILL BIT.

WORKER WAS AIR LANCING FROM STAGE #4 CONE ON PREHEATER #5 PLATFORM, WHEN AN AIR PURGE CAUSED HOT DIRT MATERIAL TO BLOW OUT THE OPEN DOOR. THIS CAUSED BURNS TO BOTH FOREARMS AND THE FACE.
