Showing all 10Operator was feeding raw material into a feed bin when a small granular piece of graphite flung out of the feed bin striking the operator in the eye. The operator was wearing prescription glasses at the time of the accident. Operator requested a hospital visit due to pain in the left eye.
Employee was operating a stretch wrap machine when the material they were wrapping became unbalanced toppling over and pinning the employee against a nearby railing. Employee requested medical attention, ER doctors report minor swelling on employees back.

THE EMPLOYEE WAS BAGGING MATERIAL AT THE PACKER, WHEN THE BAG BLEW APART, BLOWING MATERIAL UNDER HIS SAFETY GLASSES AND INTO HIS EYE.
The employee was driving the sweeper by the jet mill room. The forklift came out of the jet mill room and the two collided. The injured employee banged his head off the frame guard of the sweeper.
Employee was driving the sweeper, when the steering wheel spung around and caught the ee's thumb.
