Employee was welding, when welding the employee received flash burn to both eyes from another welder in the same area. The employee reported the flash burn 16 hours later, we are not exactly sure on the time of exposure.

EE NOTIFIED PLANT MANAGER DURING LUNCH PERIOD THAT HIS LOWER BACK WAS HURTING. SORE BACK - NOTHING SERIOUS & FINISHED SHIFT OPERATING A HITACHI EX300 BACKHOE-EXCAVATOR. NEXT MORNING WED. JULY 17 BACK WAS STILL SORE. REQUESTED TO SEE DOCTOR. SENT TO DOCTOR.

EMPLOYEE WAS HELPING DISMANTLE AND MOVE 36" FIELD TRUSS/CONVEYOR TO ANOTHER PART OF PIT. A HYDRAULIC CRANE WAS LIFTING 60' SECTIONS OF CONVEYOR. TAG LINES WERE ATTACHED. AS THE CONVEYOR WAS LI FTED THE CONVEYOR SWUNG AROUND TOWARD EMPLOYEE. EMPLOYEE MOVED OUT OF THE WAY QUICKLY SPRAINING RIGHT KNEE.
