The employee was preparing to grease the skid steer with an air grease as he was unrolling the grease line he started walking backwards not realizing how close the skid steer was parked he tripped backwards onto the bucket. The result of fall caused a left hip /lower back contusion

Employee was diagnosed with Carpal Tunnel Syndrome on June 19th by the Company's Workers Comp Doctors. This process of determination of CTS had been going on over a period of 2-3 months while the employee continued to work with no Lost Time and or Restrictive Duty. The Plant Management Team was not made aware of this diagnosis by Company Personnel until July 10th 2014

Employee was welding, when welding the employee received flash burn to both eyes from another welder in the same area. The employee reported the flash burn 16 hours later, we are not exactly sure on the time of exposure.

On 10/16/12, EE was exiting the cat skid steer. Upon exiting, EE hit her shin on the cab which possibly cause here to slip. EE reported the accident to another EE on 10/23/12. On 10/24/12 EE showed me how she thought she may have hurt her thumb-wrist area-EE was taken to the doctor where she was given a brace-she was cleared to return to her full duty as an operator.

Injured was stepping over the tailgate of his pickup truck, he placed his left foot on the bumper of the truck and his left hand on top at the tailgate. He swung his right leg over the tailgate. His left foot slipped off the bumper causing him to fall on his left side.

EE was adjusting the CB antenna, as he walked back across the walkway of the loader, he tripped over a shovel that was laying on the walkway, lost his balance and hit his knee on the hood handle.

AS THE CONVEYOR WAS LI FTED THE CONVEYOR SWUNG AROUND TOWARD EMPLOYEE. EMPLOYEE MOVED OUT OF THE WAY QUICKLY SPRAINING RIGHT KNEE.

EE WAS RESETTING E STOP (SAFETY SWITCH) AFTER CONVEYOR HAD STOPPED. HE WAS PULLING HIMSELF UP ON CONVEYOR FRAME AND SLIPPED, AND FELL INTO SKID LOADER BUCKET THAT WAS BELOW HIM. CONVEYOR WAS 3 -4' OFF GROUND.

EE WAS ENTERING LUNCH ROOM DOOR TO EAT AND WIND WAS BLOWING STRONGLY. HE GRABBED THE OUTSIDE OF THE DOOR TO CLOSE IT WITH HIS LEFT HAND AND THE WIND BLEW IT SHUT STRIKING HIS LEFT MIDDLE FINGE R BETWEEN THE METAL DOOR AND METAL DOOR JAM BROKEN FINGER.
