During the installation of a blower for #1 Sly the injured was checking the direction of the rollers when his right index finger was sucked into the rollers resulting in a deep laceration.

Pulling a 21' joint of 4" black iron pipe with an electric wench. The start/stop switch stuck and pulled pipe against right hand. Caught right hand between pipe and steps.

A top sheet failed to cut off completely. Employee opened the gate to the restricted area, overrode the electrical safety switch by blocking it off and proceeded to dislodge the top sheet while the machine was operating. The top sheet dispenser returned trapping his arm and shoulder against the metal structure.

TOP SHEET DISPENSER HAD A PIECE OF PLASTIC HANGING FROM CLAMP. EMPLOYEE PULLED IT OFF AND ATTEMPTED TO ADJUST TOP SHEET ROLL WHEN CLAMP CAUGHT HIS ARM AND HELD HIM UNTIL IT COULD BE MANUALLY BE RELEASED.
