Fall from ladders

Employee was walking stumbled and fell sideways 14 feet from a step over ladder they had traversed landing on their right side with pain on that side and shoulder 911 was called and the employee was transported to Hospital.

EE was using EE's personal knife to remove a band from a stack of bags. In cutting the strap the force used the knife to continue on toward the EE left forearm causing a laceration. EE was taken to a local urgent care facility were the wound was treated and stitches applied. EE may return to work tomorrow.

Absorption of radiations, caustics, toxic and noxious substances

Employee was dropping off a loaded trailer in the truck parking lot. While cranking the dollies on the trailer down, a bee landed on the employee's right side. This resulted in the employee being strung by the bee and the employee having to self administer an Epipen as employee is allergic to bee venom.

Forklift came into to remove pallet of product, causing the pallet to shift which resulted in the bags of product twisting the employee's left knee.

While walking on an elevated walkway employee did not observe that an access opening in the grating was open and subsequently stepped through it causing a contusion to his lower leg.

Employee was operating a machine that fills polypropylene bags with pulverized limestone when his left hand got caught in one of the straps attached to the bag, as the forklift operator was backing out with the pallet that the bag was sitting on resulting in the employee's left arm being pulled causing a strain to the machine operator's left shoulder.

Employee was leaving plant to go on a break, employee stepped backwards to speak to a fellow employee and in doing so tripped and fell into a nearby railing.

During the installation of a blower for #1 Sly the injured was checking the direction of the rollers when his right index finger was sucked into the rollers resulting in a deep laceration.

while working on the belt line, employee was picking up a bag of dust when another bag fell striking his hand and pushed it into the belt causing an avulsion of the nail right hand ring finger.

While removing lug nuts from a tire, the socket employee using came free when struck with a sledgehammer traveling backwards striking employee's face and mouth.

In attempt to remove a 50 lb bag from the production line, the bag tilted over forcing employee's right hand and wrist to be strained.

While replacing a brake chamber on a commercial truck, the employee unsafely attempted to disassemble the factory sealed portion of the brake chamber. The spring pressure discharged the brake chamber causing pieces of the spring to strike the employee over his left eye, causing a very deep laceration.

Employee tripped over wooden crib block on a concrete floor causing his left knee to strike the floor.

Employee on lunch break wiped his left eye with the sleeve of his clothing and dust from the sleeve entered his left eye allegedly scratching it

Fall from scaffolds, walkways, platforms

(Worker breached safety barrier and fell 4 feet to warehouse floor.) A roll of plastic covers was being installed with a forklift. EE was positioned inside a safety perimeter attempting to position the locks on the shaft of the roll. For whatever reason he exited the safety perimeter and fell off the elevated work area about 4 feet to the warehouse floor.

Employee was tightening straps on truck for load, hands were wet from snow, hands slipped off strap bar and bar flew up and hit him in chin. Employee was using a DOT approved strap bar.

Employee disregarded safety proceedures and labels by staying on forklift and attempting to release reclaim bin lever instead of dismounting and releasing the lever. The safety cage and fork assembly came down trapping right arm between cage and release lever.

Employee was going down the steps into the pit and a bolt had fallen and gotten lodged in the step and he did not see it and stepped on the bolt and twisted his ankle.

Employee was stacking 40lb bags and a bag fell and he bent to pick up the bag and hit his left eye on the pallet.

Employee was stacking dust on pallet and a 50 lb bag of dust came off assembly, employee caught bag and went to swing it onto the pallet and tripped and caught self with left arm.

Employee was using a lady slipper bar and was jabbing it between the belt and metal platform and the lady slipper bar slipped thru with his left pinky going thru also. Was told not to by co-worker.

Employee was pushing a hose clamp up on a hose with a screw driver and screw driver slipped hitting him in the right eye.

Employee left premises for lunch. Upon returning and still on break he was talking to other employees and he fell face forward hitting his face on the conrete.

Employee was stacking dust, picked up bag and next bag came off belt line and landed on bag that EE was picking up and shoved his right hand 2 fingers between the belt and metal plate.

Operator speeding on forklift, hit concrete slab, swerved, overcorrected and forklift went up on side at 35 degree angle and came to rest against pallet of product.

Employee stated that he had a toothache and was going to the emergency room. When he got to the emergency room he told them that he got hit with a fork lift. He did not report this to his supervisor and the fork lift operator stated that he knew nothing about the incident. We fill this report with prejudice and that the indicient was not reported to supervisory personnell.

A top sheet failed to cut off completely. Employee opened the gate to the restricted area, overrode the electrical safety switch by blocking it off and proceeded to dislodge the top sheet while the machine was operating. The top sheet dispenser returned trapping his arm and shoulder against the metal structure.

TOP SHEET DISPENSER HAD A PIECE OF PLASTIC HANGING FROM CLAMP. EMPLOYEE PULLED IT OFF AND ATTEMPTED TO ADJUST TOP SHEET ROLL WHEN CLAMP CAUGHT HIS ARM AND HELD HIM UNTIL IT COULD BE MANUALLY BE RELEASED.

Forklift operator was putting tarp on top of loaded trailer. Tarp was heavier than usual because of rain and mud. He twisted his left wrist.

Was loading dump truck #121 with 920 loader got in #121 to park on back lot. When exiting #121 stepped out and missed step.
