While performing mechanic duties, HOLT employee and Weisman Equipment contractor, was embedded in the left chest area by a small piece of metal shard from a piece of equipment. The employee was taken to a medical facility to receive medical treatment and the foreign body was removed from the chest.

Showing all 5While standing at the control panel of a portable screen a large rock fell from the top surface and struck the top of EE's hard hat. The hard hat shell contacted EE's head, causing a cut to EE's scalp. The impact knocked EE down and caused pain to EE's back and right shoulder.
While using a hand held grinder, employee slipped and hit left hand lacerating palm.
EE was using an impact wrench to remove a rusted bolt when it jerked causing him to smash his finger.
