Struck by falling object

While standing at the control panel of a portable screen a large rock fell from the top surface and struck the top of EE's hard hat. The hard hat shell contacted EE's head, causing a cut to EE's scalp. The impact knocked EE down and caused pain to EE's back and right shoulder.

Struck against a moving object

While dumping dirt from a 775 haul truck, the bed lowered rapidly and jostled the operator in the cab, causing what was diagnosed as cervicalgia. However, they did not report the incident until 28 July 2025.

Struck against a moving object

While driving loader 600-181 down to the city base pit, the bucket slowly dropped until it dug into the road, bringing the loader to an abrupt stop. EE reported the incident to EE's supervisor after it happened but did not need or want medical attention at that point. On 7 May EE stated that EE's right shoulder was very sore and requested to have it examined.

Struck by falling object

EE was cleaning a ladder at the secondary wet screen when material fell from the ladder and went into EE's right eye.

Struck by rolling or sliding object

Employee climbed on pump to disconnect motor from pump. Pump was disconnected from pipe, pump rotated, employee fell, pump pinned leg between pump flange and pipe.

Fall onto or against objects

Closing the lid of the crusher, EE was moving a rock that was in the way with a metal rod. When the rock moved, EE's weight shifted as well, and EE fell into the lid retainer wedge holder with EE's upper right chest.

Struck by flying object

The miner was cleaning the catwalk along the truss conveyor near the chute transferring material from the overland, when EE was struck in the right eye by a small piece of fly rock.

Caught in, under or between a moving and a stationary object

Replacing a tail pulley in a screen feed belt, the new pulley assembly was suspended in place when the maintenance employee was using emery cloth to smooth the shaft. The pulley assembly shifted, trapping employee's thumb between the shaft and the structure. The crush injury resulted in a tuft fracture of the right thumb, and loss of the thumb nail.

Caught in, under or between a moving and a stationary object

While moving a supporting block beneath a conveyor assembly under construction, the block was left crooked. Employee attempted to straighten the block and the conveyor assembly dropped unexpectedly and caught the meat of EE's right little finger. The crush injury caused a 15 cm laceration.

Struck by falling object

Clearing out a stoppage of the crusher, employee placed forearm on the frame and peered in the open access doors. One of the access doors was jarred and fell onto employee's forearm, fracturing right distal radius and ulna.

Fall from machine

Offloading a 40' auger from a flatbed, EE placed chocks against the auger and removed ties securing it in place, then moved to secure it to the forklift with a come-along winch. When the auger started to roll the EE jumped clear of the auger and upon landing on the ground EE's right knee was twisted and EE's left wrist was broken.

Employee fractured and lacerated the end of the middle finger on left hand when replacing a conveyor belt tail pulley bearing assembly.

Employee was using utility knife to cut notches in skirt board rubber for a conveyor. Employee was cutting towards body when knife slipped and hit employee in the leg cutting it. Cut required 4 stitches.

Operator was descending the steps on the front of a 775 haul truck. When EE placed right foot on the ground, it slid back on some loose material. EE fell forward onto the steps/bumper of the truck hitting left elbow/forearm and chin.

Customer truck driver hauling material for CKJ Trucking LP. Climbed into bed of truck on tar plane of the haul road as he was climbing down from the top of the truck he slipped and fell approx. 5 feet to the pavement below, landing on his right side.

While using a hand held grinder, employee slipped and hit left hand lacerating palm.

Employee slipped on last rung of the step ladder he was using to fuel a scraper. His foot landed on the fuel hose laying on the ground next to the scraper causing him to twist his right knee damaging the ligaments.

EE was transferring crusher shoes and anvils from a pallet to the bed of a truck. EE pulled/strained muscle in lower back. EE reported injury to super.at the end of the shift 1/25/12. EE requested medical attention the following day 1/26/12 at approx. 3:00p.m. EE was taken to local area medical provider at this time for exam.

EE was standing on a conveyor belt removing some over head bolts with a wrench. The belt was wet and EE's foot slipped. He caught himself by grabbing the over head beam with his left hand and dislocated his left shoulder as a result of stopping the fall. EE was wearing proper fall protection and was properly tied off when the injury occurred.

Struck by flying object

Employee was shoveling next to tunnel feed conveyor belt when a 2 inch rock bounced off the belt and struck him on the left side of the neck in the soft tissue between the jaw and collar bone. Employee was taken to the hospital to be checked by a physician as a precautionary measure.

Opening crusher door on VSI crucher, employee was pushing up from under side of door while other employee was lifting from top side with rock bar. The bar slipped, the door started to come down, employee tried to pull hand out of the way but the door caught pinkie of right hand pinching and lacerating finger.

EE slipped and fell going down stairs to crusher control house, severely spraining right ankle.

WHILE ASSISTING A FELLOW MAINTENANCE WORKER, THE EMPLOYEE WAS BURNED ON THE WRIST BY A CUTTING TORCH WHEN IT SLIPPED OFF THE PIECE OF METAL THEY WERE CUTTING.

EE WAS CONDUCTING A PRESHIFT INSPECTION OF THE SECONDARY PLANT. WHILE CLIMBING UP A FIXED LADDER THE BOTTOM PORTION CAME LOOSE CAUSING THE EE TO FALL ABOUT 3 FT TWISTING HIS KNEE.

EE STATED THAT HE ENTERED THE OFFICE BLDG AND A FEW MINUTES LATER AS HE WAS LEAVING THE BLDG SLIPPED AND FELL ON THE LANDING AREA. THE AREA CONSISTS OF A LEVEL CONCRETE FLOOR COVERED BY A ROOF . THE EE WORKED UNTIL 1-28-04 AND LEFT ON FAMILYMEDICAL LEAVE FOR AN UNRELATED ILLNESS RX ONLY.

EE WAS ATTEMPTING TO FUEL EQUIPMENT. HE ENERGIZED THE FUEL PUMP. HE THEN GRABBED THE FUEL NOZZLE & FUEL LINE FROM OFF THE METAL HOOK. THE KEROSENE HEATER WAS PRODUCING A LARGE AMOUNT OF HEAT. EE WALKED PAST THE HEATER WHILE CARRYING THE FUEL HOSE & CLIMBED THE LADDER. THE PRESSURIZED FUEL HOSE CONTACTED THE HEATER CAUSING THE FUEL LINE TO SEPARATE & SPRAY FUEL ONTO THE HEAT SOURCE.
