Showing all 22While pull starting a chainsaw the miner felt a pop in the left wrist. X-Rays were taken and came back as negative but the miner was placed on restricted duty by the doctor to rest the wrist.
Miner was cutting/grinding a 20'x5" Channel Iron when it fell from the work bench catching left glove on the way down. This resulted in a strain to EE's left shoulder when it jerked EE's left arm. Miner has been placed on restricted duty and a MRI of the shoulder is pending.
EE was using an impact gun to remove a 3/4" nut from a bolt. As the nut loosened, the impact gun kicked back and struck EE above the left eye (between his safety glasses and hard hat) causing a laceration above his left eyebrow. EE was taken for medical treatment, received 4 stitches for the laceration and was released to work with no restrictions.
While operating the water pull the miner was looking back at the sprayers located behind the operator stations view. As the miner turned back forward she felt a pull in her back like it was getting a spasm. Miner described pain as in the lower part of her back shooting down towards her leg.
Employees were taking turns as one welded the heal plates into the loader the other would then grind. Employee was injured by indirect exposure of UV rays from his coworker who was welding He was wearing an auto darkening welding hood; The hood was on shade level #5 for grinding.
While trying to install the basket on the K400 cone crusher, the basket moved toward the engine housing pinning the employees arm.
He was cutting the screen bolts off and when he shifted to cut the next bolt the torch tip got caught in his glove.
While watching over his crew perform various welding, cutting, and using the plasma ark through out the day, he injured his eyes.
Was welding he got flash burns.
As he was drilling a hole in a belt, the drill slipped and went into his finger.
He was holding a piece of wood and as he proceeded to shoot a nail through the wood he missed and the nail went through his thumb.
HE WAS ASSISTING IN A LINER CHANGE AND WHILE REMOVING BOLTS HIS CO-WORKER WAS USING AN IMPACT GUN TO REMOVE THE BOLTS, SUDDENLY A NUT FLEW TOWARD HIM STRIKING HIN IN THE MOUTH.
HE WAS USING A HIGH PRESSURE AIR HOSE TO CLEAN DIRT AND DEBRIS FROM THE NEW JAW LINERS WHEN SOMETHING FLEW INTO HIS EYE.
WHILE HE WAS LOOSENING NUT ON THE CRUSHER, A WIRE WHICH HELD THE SOCKET ONTO THE IMPACT GUN CAME AROUND AND CUT HIS LEFT HAND FROM THE THUMB AROUND TO THE 1ST FINGER.
EE WAS CUTTING REBAR & A PIECE SPRANG BACK & HITTHE TORCH. THIS CAUSED THE TORCH TO TURN & HIT EE ON THE NECK CAUSING 2ND DEG BURNS
HE WAS GRINDING ON A METAL PLATE WHEN A GUST OF WIND SHIFTED THE DEBRIS INTO HIS RIGHT EYE.
HE WAS WELDING AND GRINDING BOLTS TO INSTALL NEW PULLEYS FOR THE HOPPER BOOT. A PIECE OF METAL FLEW INTO THE EE'S RIGHT EYE.
HE WAS INSTRUCTED BY HIS SUPERVISOR TO WELD A GURARD FOR THE DRY SCREEN, THE WAS NOT PROVIDED WITH ANY TYPE OF PROTECTIVE EQUIPMENT FOR THIS JOBTASK. HE PROCEEDED TO WELD FOR APPROX.30 MINUTES AFTER COMPLETING THE JOB HE WENT BACK TO HIS REGJOB OF OPERATING THE LOADER. AT APPROX. 7PM HE BEGAN TO FEEL BURNING IN BOTH HIS EYES.
AS HE WAS SHOVELING SPILLED MATERIAL ONTO THE RETURN BELT A LARGE ROCK BOUNCED OUT OF THE CONE STRIKING HIM ON THE RIGHT SIDE OF HIS HARD HAT.
EE WAS OBSERVING THE DRIVE BELTS IN THE CONE CRUSHER TO SEE IT THEY WERE SLIPPING, THE BELT BROKE HITTING HIM IN THE FACE AND HAND.
EMPLOYEE HAD HIS HAND ON I-BEAM WHILE USING THE IMPACT GUN; IT CAME OFF THE SOCKET AND SLAMMED DOWN ON HIS FINGER.
