The EE was working on Tower No. 1. The chute over the scalping screen stopped up and spilled rock out of the chute. The EE saw the material begin to fall, and a 6" rock hit EE in the back causing a Contusion. Doctor put EE on restricted duty for one week, thus causing the accident to be reportable.

EE was working on the scalping tower installing new chutes when a neighbor began firing an automatic weapon in the direction of the screening tower from a neighboring property. One of the bullets hit the EE in the right, upper thigh. The EE was taken to the hospital, treated and released for THE injuries. EE missed two days of work due to the incident.

EE was attempting to put a pin back into a Euclid R-90 Haul Truck. Employee was using air tools. During the process, got something in left eye. The next day employee went to the doctor and they had to remove a piece of metal from eye. Employee was wearing safety glasses at the time of the accident.

EE was cutting holes in a piece of metal for bolts to go through with an oxy/acet torch. The EE tried to pull the gas hoses closer to him and they got caught on a piece of metal. He tugged at the lines and the torch fell from his hands. When the torch fell, it turned and burned his left forearm and wrist, causing 2nd and 3rd degree burns.

EE WAS TAKING BOLTS OUT OF A SCALPING SCREEN. EE WAS PULLING SLACK OUT OF HOSE WHEN THE TORCH FLIPPED OVER, BURNING HIS RIGHT HAND.

WHILE USING THE AIR WRENCH TO TIGHTEN BOLTS ON DOZER BLADE, THE EMPLOYEE'S FINGER WAS CAUGHT BETWEEN THE SOCKET AND BASE OF THE IMPACT WRENCH, CUTTING THE TIP OF HIS MIDDEL FINGER ON HIS LEFT HAND.

WHILE STEAM CLEANING THE D-7 DOZER, THE STEAM CLEANING HOSE BECAME ENTANGLED ON THE DOZER BLADE, CAUSING THE EE TO LOSE CONTROL OF THE WAND AND SPRAYING HIS RIGHT FOOT AT THE BOOT LACES, HOT W ATER ENTERED HIS BOOT BURNING HIS ANKLES.
