Employee was operating a Cat D-8 dozer on a ore stockpile. He doesn't know when the injury happened. His back was sore and having spasms on 11-16-08. He went to the doctor and was not allowed to return to work. He contacted his supervisor on 11-21-2008 about his injury.

Showing all 10Employee was operating a 16G Motor Grader. She was backing up after making a pass. She was looking over her right shoulder when her neck started to hurt. She initially thought it would be OK and didn't want to go to a Dr. On August 14, 2008 she decided to go to the doctor. She was then placed on light duty.
Employee was operating a drill press. While operating the drill the glove on his right hand was caught in the rotating drill bit. He sustained a cut on his right hand. He did go to the emergency room and received 6 stitches.
Employee was installing bed liners on a 785 haul truck. He was using a portable 9" grinder. In doing so he had gotten metal in his eye. He wasn't sure if he had metal in his eye or not. So on 11-22-07 I took him to the emergency room where the metal was removed. Employee did go to an eye doctor on 11/23/07 and again on 11/26/07. His eye is o.k. and he did not miss any work.
Employee was welding on bed liners of a haul truck. In doing so, hot slag burned his left elbow and it got infected. He went to the doctor on Tuesday the 30th and the doctor decided not to let him return to work until the infection was gone.
EE WAS ATTEMPTING TO LIFT A TORQUE CONVERTER THAT WAS PARTIALLY BOLTED INTO A 992 LOADER. THE PICKING EYES BROKE CAUSING THE CRANE BLOCK TO JUMP SEVERAL FEET IN THE AIR AND ONE OF THE CABLES TO COME PARTIALLY OFF THE SHIV. HE DID NOT NOTICE THE FAULTY SITUATION & CON'T HIS JOB ABOUT 3' UNDER THE 200# BLOCK. THE BLOCK DROPPED ONTO HIS HARD HAT PINNING HIM AGAINST THE LOADER.
EE WAS ATTEMPTING TO INSERT 8" PLASTIC PIPE INTO THE PIPE FUSER. AS THE PIPE SLID FORWARD, EE GOT RIGHT LITTLE FINGER BETWEEN THE PIPE AND THE FUSER. FINGER WAS LACERATED REQUIRING STITCHES AT THE EMERGENCY ROOM
EMPLOYEE WAS USING A WIRE FEED WELDER WHEN THE STRINGER SLIPPED OUT OF HIS RT.HAND. HE ATTEMPTED TO GRAB IT W/HIS LEFT HAND & THE WIRE PIERCED HIS GLOVE & HAND. THE WIRE STABBED HIS INDEX FINGER TENDON CAUSING AN INFECTION. DR. NEEDED TO CUT & DRAIN THE INFECTION.
MINE MECHANIC WAS USING A HAND HELD PNEUMATIC DIE GRINDER TO CUT A PIECE OF METAL ON A HAUL TRUCK ENGINE. THE 3 INCH GRINDING WHEEL BROKE AND A PIECE OF IT STRUCK HIM ON THE LOWER LIP. THREE STITCHES WERE PLACED IN HIS LIP TO CLOSE THE LACERATION.
EE WAS DRILLING 1/2" DIAMETER HOLE THROUGH A PIECE OF PLASTIC RACEWAY. HIS RT. HAND WAS STABILIZING THE PLASTIC AND THE DRILL BIT PUNCHED THROUGH THE MATERIAL AND LACERATED THE PALM OF HIS RT. HAND. HE IS LEFT HANDED. 2 STITCHES REQUIRED
