Employee was blowing flu's at waste heat with a crooked lance. Employee did not inform supervisor or office of x-rays. Did not know it was reportable until hospital sent bill for x-ray taken.
Employee uses airlance to blow dirt from boilers. Also uses rebar to break caked on dirt. He states there is a lot of hand, wrist & shoulder movement to this task. This task is required every day. Employee was given medication first. Then Dr. decided he needed thearpy. That was done on 10-12-04. Reported as soon as it became reportable.
Employee was changing brass on the rolls on # 3 Kiln. He picked up a 1 1/2" impact to tighten the bolts. Felt a pain in shoulder. EMPLOYEE DID NOT REQUEST MEDICAL ATTNETION UNTIL 5/5/04. AT THAT TIME HE WAS SENT IMMEDIATELY TO THE COMPANY DOCTOR.
EE IS BREAKING IN ON KILN OILER, DURING THIS TIME RINGS WERE BEING SHOT IN THE BACK OF #4 KILN. AFTER FIRST SHOT DUST THAT HAD SETTLED ON OVERHEAD PIPES FELL INTO THE EYE OF EE.
EE WAS DOING MAINTENANCE WORK ON #4 KILN (GIRTH GEAR). HE WAS REAMING HOLES 8 HOURS A DAY FOR 3 DAYS. DURING THIS TIME HIS LAST TWO FINGERS ON LEFT HAND BEGAN TO BECOME NUMB.
EMPLOYEE WAS HELPING UNLOAD COKE RAILCARS. WHEN THE CAR WAS EMPTY HE WAS TO UNHOOK THE CHAIN CONNECTING THE CAR SHAKER TO RAILCAR. HE DID NOT DO SO AND WHEN THE HOIST WAS LIFTED AND CAR STARTE D TO MOVE THE TENSION ON THE CHAIN CAUSED IT TO SNAP AND SWING IN HIS DIRECTION AND COME IN CONTACT WITH HIS LEFT ARM, FACE, AND RIBS.
EE WAS BLOWING OFF CLOTHING AFTER A JOB. EE WAS USING AN APPROVED AIRLINE. A PIECE OF DEBRIS LODGED IN HIS RIGHT EYE.
EE WAS PLACING JACK & STAND UNDER #2 KILN TIRE. A FAN WAS CIRCULATING AIR IN THE WORK AREA WHEN CLINKER DUST WAS BLOWN INTO EE'S RIGHT EYE.
EE WAS BLOWING DUST OFF OVERHEAD DUCT WORK. HE WAS WEARING FACE SHIELD & SAFETY GLASSES. DUST HAD ACCUMULATED ON BILL OF HARD HAT. WHEN HE LOOKED UP, THE DUST FROM BILL FELL UNDER FACE SHIELD, BEHIND GLASSES AND INTO EYE.
EE WAS USING AN AIRLANCE THAT IS REINFORCED BY REBAR ON BOTH SIDES OF LANCE. HE WAS USING THIS LANCE TO POKE HOLES IN THE DIRT BUILDUP ON BOILER. AS HE WAS PERFORMING THIS TASK HE FELT A POPPI NG IN HIS BACK. MIDSECTION. THE AREA HE WAS WORKING ON WAS A LITTLE LOWER THAN HIS KNEES.
EE STATES HE WAS KNEELING/SITTING IN AN AWKWARD POSITION TO WELD A CRACK ON NO. 4 KILN. HE TWISTED HIS BACK CAUSING GREAT PAIN.
EE WAS IN THE PROCESS OF SHOOTING A BALL IN #4 COLLER. THE BLASTERS WERE SHUT OFF AS WELL AS THE FAN. HOWEVER, WHEN INSPECTION DOOR WAS OPENED CLINKER DUST PUFFED BACK IN HIS FACE, GETTING INT
EE WAS REMOVING CASTABLE FROM AROUND THE BLASTERNOZZLES IN THE COOLERS USING A SMALL JACKHAMMER. JACKHAMMER BIT GOT STUCK BETWEEN THE CASTABLES AND A CASTABLE ANCHOR. WHEN HE TRIED TO PULL TH E JACKHAMMER OUT HE HURT HIS NECK AND DOWN BETWEEN HIS SHOULDER BLADES.
EE WAS STANDING ON THIRD RUNG OF LADDER. HE WAS CUTTING WITH A TORCH. THE TORCH BACKFIRED AND HE LOST HIS BALANCE. TRYING TO REGAIN HIS BALANCE HE GRABBED AHOLD OF THE LADDER AND HURT HIS RIGH T SHOULDER.
