Showing all 9Turning of drive belts. Right middle finger pinched between motor pulley and drive belts. EE jerked EE's hand quickly which severed the tip of finger. No bone loss. Y flap to cover wound.
Breaking oversized rock with excavator. While placing tool onto rock, the boom pressure remained, causing excavator to flip onto its side. EE had lap belt on, but upper body thrown into cab sides, fx clavicle. EE had to jump off about 8ft, spraining both ankles.
While grinding on steel (flat stock), the tool rest broke, causing his LT hand to contact the grinding wheel.
Loosening the closed side setting. His hand was on the hydraulic ram, used to make the adjustment. It retracted, pinching his LT ring finger with 1300 psi of pressure.
EE was operating track mobile to position onto tracks to load railcars. As he was positioning the track mobile to go back on rail, left rear tire rim failed. A piece of the rim swung up against side of the cab, striking cab & employee. The side glass was open and employee leaned out to line up wheels when rim failed causing employee to be struck in the face. Trauma/contusions.
Employee was tightening screw bolt with electric impact. A foreign body flew between safety glasses and his left eye. Employee worked the rest of the day. Next day had to go to the company clinic. Employee was diagnosed with conjunctivitis, irritation and laceration in his left eye. Employee was given a prescription. No lost or restricted time.
Employee was removing the pivot joint bushing on the Cat. 769 Haul Truck and replacing the bit on the air hammer. Employee placed his hand on top of the bit to push it in place, the trigger on the air hammer was accidently pulled causing the tool to be activated, resulting in a contusion of his right hand. Employee was restricted for 5 days.
EMPLOYEE WAS WELDING A ROLL CRUSHER USING AN AIRCHISEL. SLAG WENT UNDER HIS SAFETY GLASSES INTO HIS RIGHT EYE. THE FOREIGN BODY WAS EMBEDDED AND SURGICALLY REMOVED.
